We present the case of a 77-year-old woman who had an accidental fall in her garden, resulting in a fracture of her left ankle. After manipu … . A few days later, she developed abdominal pain, and was subsequently diagnosed with a spontaneous splenic rupture. This case was interesting due to the unusual chain of events following a simple fall, and also a rare complication of anticoagulant therapy.

A 15-year-old previously healthy girl presented in full arrest after 1 week of flu-like symptoms, recent diagnosis of infectious mononucleosis, and 1 day of abdominal pain. There was no history of trauma. Focused assessment with sonography for trauma examination showed free fluid in the abdomen. The patient died despite aggressive resuscitative management and emergency laparotomy with splenectomy, which showed grade V splenic laceration. Infectious mononucleosis is a common viral illness of adolescence. Spontaneous splenic rupture is a rare but potentially fatal complication. Anticipatory guid…

AIM: To evaluate seven patients with non-traumatic splenic rupture (NSR). NSR is an uncommon dramatic abdominal emergency that requires immediate diagnosis and prompt surgical treatment to ensure the patient's survival.
METHODS: Within 11 years, seven cases were evaluated for patient characteristics, anamnesis and symptoms, method of diagnosis, findings of laparotomy, and etiology of NSR.
RESULTS: There were six (86%) male and one female (14%) patient, whose mean age was 36 +/- 12.8 (17-56) years. We report here four cases of Plasmodium vivax malaria (cases I-IV), one case of hemodialysis (c…

Spontaneous rupture of the splenic vein is rare, and is associated with high mortality. Few cases have been documented, associated with pregnancy, with hepatic cirrhosis and diseases predisposing to portal hypertension. We report a rare case with no evident symptoms or signs of liver deficiency, which was manifested with sudden massive intraperitoneal haemorrhage. An urgent laparotomy was performed and a tear of the splenic vein which was massively bleeding was found. The patient underwent urgent splenectomy and a biopsy of the liver was taken. The patient had an uneventful postoperative cours

OBJECTIVE: To evaluate the choice of treatment method for traumatic splenic rupture in children.
METHODS: A retrospective analysis of 70 children with traumatic splenic rupture was performed.
RESULTS: Of the 70 children, 10 were managed with non-surgical treatment and cured. In the 60 children receiving operations, 35 underwent partial splenectomy or prosthesis, 25 had total splenectomy, and the other 21 received autologous spleen transplantation. The follow-up study of 61 children lasting for 3 months to 5 years after operation did not reveal the occurrence of overwlrelmingpostsplenectomy i…

The anaesthetic and critical care management of blunt abdominal trauma in a patient previously diagnosed with non-compaction of the left ventricular myocardium (a rare autosomal dominant inherited disease) is reported. The management was influenced by the presence of an implanted automated internal defibrillator and treatment with anticoagulants because of the high frequency of severe arrhythmias and systemic embolism. The pathophysiology of ventricular non-compaction is reviewed briefly.

HISTORY AND CLINICAL FINDINGS: A 51-year-old asthenic patient attended the hospital with syncope, head injury, tarry stool and severe anemia. There was a history of alcohol and nicotine abuse, but no known preceding diseases of the liver or gastrointestinal tract. Except hypotension, examination of the patient did not show any further abnormalities.
DIAGNOSTIC PROCEDURE: An upper and lower endoscopy did not show any evidence of a bleeding source although the stomach was full of hematin. An abdominal ultrasound demonstrated signs of a chronic pancreatitis and a big cystic structure in the area

We report herein the case of a 26-year-old man who suffered a spontaneous splenic rupture during the course of a rubella infection. Initially, he presented with a high fever accompanied by a skin rash extending from the body trunk to the extremities and was admitted to hospital 3 days after onset for sudden upper abdominal pain radiating to both shoulders. On admission, he was pale and sweating with hypotension. An abdominal ultrasonographic study revealed a collection of fluid in the perisplenic space and exploratory laparotomy confirmed an intraabdominal hemorrhage from a ruptured spleen. Th…

Peliosis is a rare entity, formerly seen almost exclusively at autopsy, which most commonly involves the liver and less frequently the spleen. We present the sixth reported case of splenic peliosis discovered at surgical exploration and discuss the relevant findings and background of this condition, which may become an important issue for the surgeon.

A rare but dramatic case of haemoperitoneum of non-gynaecological origin which occurred during pregnancy is reported. Obstetrical causes of shock having been excluded, laparotomy made it possible to discover an aneurysm of the splenic artery and treat it by resection-ligation and splenectomy. In such cases, the uterus is spared before the 6th month of pregnancy, and beyond that data caesarean section is mandatory. The pathogenesis of such arterial ruptures during pregnancy is unknown. The abrupt collapse accounts for the high maternal and foetal mortality. Prevention can only be achieved by su

We report a case of spontaneous splenic rupture in a patient with metastatic melanoma. Spontaneous splenic rupture without previous trauma has been observed in various pathological conditions such as infectious mononucleosis, malaria, typhoid fever and, rarely, neoplasms affecting the spleen. There have been several reported cases of splenic rupture in leukemias. Despite the high incidence of splenic metastases in metastatic melanoma, there have been only 3 cases of spontaneous splenic rupture reported in the past.

A 35-year-old man died suddenly in a clinic waiting area after repeated visits for flu-like symptoms. At autopsy, hemoperitoneum, splenic capsular rupture, and splenomegaly were found. Microscopic examination, special stains, and immunoperoxidase studies revealed findings consistent with chronic myelogenous leukemia. Spontaneous splenic rupture is an unusual presenting feature of chronic myelogenous leukemia. The symptoms of leukemia may mimic those of other "benign" disorders and misdiagnosis may lead to catastrophic consequences in some instances.

Plain film radiography, by appraisal of the soft tissues of the descending colon and left paracolonic gutter, comprises a safe, accurate, and quick determination of the presence of free fluid, which, in the proper clinical setting, indicates splenic rupture. The air enema, introduced under fluoroscopic control, is a valuable aid in eliminating false negatives and false positives, and also in distinguishing intraperitoneal clots from subserosal hematoma. The preoperative diagnosis was correct in 27 of 28 cases. The one error occurred in a case in which the interpretation of paravesical fluid (d…

Atraumatic splenic rupture is an uncommon, life-threatening abdominal emergency involving the rupture of the spleen in the absence of obvious trauma. This case report provides a summary of a 48-year-old male who presented to the Emergency Department of the University of Ghana Medical Centre with syncope and hemodynamic instability one week after receiving a yellow fever vaccine. A diagnosis of an atraumatic splenic rupture was eventually confirmed by imaging, and a splenectomy was done. This report aims to provide valuable insights into the recognition and management of atraumatic splenic rupt…

Atraumatic splenic rupture is a complex surgical pathology owing to its rarity, non-specificity of symptoms and gravity of possible outcomes. This case outlines the investigation and management of a patient with atraumatic splenic rupture secondary to undiagnosed hairy cell leukaemia. While the patient was initially managed conservatively, they went on to have a splenectomy owing to ongoing transfusion requirements. A review of the literature has also been performed and presented to highlight the potential causes of atraumatic splenic rupture and the various options for confirming diagnosis an…

Traumatic delayed splenic rupture often follows by a "latent period" without typical symptoms after injury. During this period, though there are no obvious symptoms, the injury is still present and changing. In this study, we constructed an SD rat model of delayed splenic rupture; evaluated the model by HE staining, Perl's staining, Masson trichrome staining and immunohistochemical staining; observed the pathological changes of spleen tissue in delayed splenic rupture at different times after splenic injury; we found that pathological change of injured tissues were different from non-injured, …

BACKGROUND Delayed splenic rupture is a rare complication of non-operative management of a primary splenic trauma which, without proper clinical vigilance, may result in life-threatening events. It usually occurs 4-8 days after injury and, in most cases, surgery is the treatment of choice. Since non-operative management of splenic trauma, which allows splenic salvage, has become increasingly popular, the same approach could also be applied in delayed splenic rupture. We herein present a case of delayed splenic rupture that occurred 4 months after the trauma and was successfully managed non-ope…
