Single port thoracoscopic sympathectomy for primary palmar hyperhidrosis in adolescence.

AIM: Hyperhidrosis is defined as excessive sweating beyond the physiologic needs of a person. Palmar hyperhidrosis in the adolescent period may have an impact on school work and may cause psychological problems. In this study we aim to increase awareness of this disregarded problem. MATERIALS AND METHODS: We explicated the early outcomes of 7 consecutive adolescents, where single port video-assisted thoracoscopic sympathectomy was performed for primary palmar hyperhidrosis. Patients were evaluated for symptom resolution, which was defined as complete dryness, patient satisfaction, operative c

Lung-sparing approach for an intrapulmonary bronchogenic cyst involving the right upper and middle lobes.

Intrapulmonary bronchogenic cysts (IBC) represent 20% of abnormal budding of the respiratory tract. Lobectomy is the recommended treatment for IBC in symptomatic adults. We presented a case of a patient with an IBC involving the right upper and middle lobes (RUL-RML). A 27-year-old woman presented with a 2-month history of thoracic pain, cough and haemoptysis. An opacity was found on the chest X-ray. High-resolution CT/MRI showed a 7×4.5 cm marginated mass with an air bubble inside. A video-assisted thoracoscopic surgery was performed. The cyst was neither palpable nor visible. An intraoperati

Extended videothoracoscopic thymectomy in nonthymomatous myasthenia gravis.

Myasthenia gravis (MG) is an uncommon, organ-specific, autoimmune chronic neuromuscular disorder involving the production of autoantibodies directed against the nicotinic acetylcholine receptors (anti-AchRab). It is characterized by weakness and rapid fatigability of voluntary muscles. Thymectomy is performed early in the course of the disease and is indicated for adults less than 70 years old. For many years, the clinical efficacy of thymectomy has been questioned and so far, its benefits in nonthymomatous MG have not been firmly established. Furthermore, the precise mechanisms of action of t

The future in diagnosis and staging of lung cancer: surgical techniques.

Surgical techniques remain central to the diagnosis and staging of lung cancer. Clinical situations which invoke the role of surgery include the diagnosis of solitary pulmonary masses, staging of the mediastinum, restaging of the mediastinum and the assessment of resectability. The techniques available include cervical mediastinoscopy, anterior mediastinotomy, video-assisted thoracoscopy and different procedures for intra-operative mediastinal lymph node assessment including systematic nodal dissection, lobe-specific nodal dissection and sentinel node mapping. The staging of lung cancer is con

Pneumocephalus after thoracoscopic excision of posterior mediastinal tumor.

Pneumocephalus is a very rare complication of video-assisted thoracoscopic surgery (VATS). A 66-year-old man developed pneumocephalus secondary to thoracoscopic excision of a neurogenic tumor in the posterior mediastinum. Pneumocephalus was diagnosed by brain computed tomography. Neurosurgical intervention was performed after conservative therapy had failed. The patient's condition had resolved without any neurologic sequelae 2 years later. When a patient has a relatively large amount of chest tube drainage and neurologic symptoms after VATS, the possibility of pneumocephalus due to cerebrospi

[Mini-invasive treatment of pectus excavatum in adolescence. Initial experience].

BACKGROUND: Personal preliminary experience with Minimally Invasive Repair of Pectus Excavatum (MIRPE), "Nuss" procedure, using VATS is reported. METHODS: From January 2001 to February 2002, MIRPE has been performed on 5 patients (age range 13-18 y; mean 14.8 y). Under general anesthesia, a curved steel bar is inserted into the retrosternal tunnel between 2 bilateral midaxillary line incisions. The tunnel passes initially under the pectoral muscles and enters the pleural space at level of the mammilary line. Under thoracoscopic vision, the bar is passed through the tunnel with the concavity f

Multicystic mesothelial cyst of the mediastinum: report of a case.

We report the rare case of a multicystic mesothelial cyst of the mediastinum. A 43-year-old man was referred to our department after a routine chest X-ray showed an anterior mediastinum mass. The lesion was resected by thoracoscopy, which revealed multicystic, thin-walled tense masses arising in the anterior mediastinum. The cysts were not adherent to the pericardium or to the thymus of the right lobe. Immunohistochemistry revealed that the cyst was lined with a layer of mesothelial cells and there were no malignant foci. The lining cells were positive for keratin and negative for the lymphocy

The Evolving Role of Medical Thoracoscopy for the Management of Malignant Pleural Effusion.

MT is a minimally invasive endoscopic procedure which is a well-established tool for the management of pleural malignancies, which commonly cause pleural effusions. MT allows for pulmonologists to perform diagnostic and therapeutic maneuvers at the same time with high diagnostic sensitivity and can also shorten the hospitalization duration. MT, which is video-assisted, is performed by pulmonologists, and is not the same procedure as surgical thoracoscopy or video-assisted thoracoscopy surgery (VATS). To perform MT, pulmonologists use non-disposable rigid or semi-rigid telescopes in the endosco

Bilateral Video-Assisted Thoracoscopic Resection for a Patient With Horseshoe Intralobar Pulmonary Sequestration: A Novel Minimally Invasive Approach.

We encountered a rare case of bilateral intralobar pulmonary sequestration presenting as horseshoe lung. A 44-year-old asymptomatic woman was referred to our hospital because of increased serum cancer antigen 125 and carbohydrate antigen 19-9, accompanied by an abnormal chest shadow. Preoperative imaging indicated a horseshoe-shaped bilateral intralobar pulmonary sequestration diagnosis. The surgery was initiated on the left side in a semiprone right lateral decubitus position. The aberrant vessels arising from the descending aorta were divided, and after indocyanine green administration, the

Uniportal thoracoscopic posterior basal (S10) segmentectomy using a posterior approach.

Performing a posterior basal (S10) segmentectomy through a single port is challenging because of the dorsal location of the S10 segment in the lower lobe. The vessels and bronchi to be resected are located deep and away from the major fissure, which makes exposure from the interlobar fissure difficult. To avoid unnecessary parenchymal splitting and potential misrecognition of segmental structures, we performed a uniportal thoracoscopic S10 segmentectomy via a posterior approach without extensively separating the pulmonary parenchyma from the interlobar fissure. When using this approach, placem

Risk of Pneumothorax Requiring Pleural Drainage after Drainless VATS Pulmonary Wedge Resection: A Systematic Review and Meta-Analysis.

OBJECTIVE: Omitting pleural drainage after video-assisted thoracic surgery (VATS) for pulmonary wedge resections has been shown to be a safe approach to enhance recovery. However, major concerns remain regarding the risk of postoperative pneumothoraces requiring surgical interventions. Therefore, our objective was to provide conclusive evidence whether chest tube omission after VATS wedge resection is safe and does not increase the risk of pneumothoraces requiring pleural drainage. METHODS: Five scientific databases were searched. Studies comparing patients with (CT group) and without chest t

A holistic investigation of the global outcomes of spontaneous pneumothorax during 1980-2021, including the COVID-19 pandemic A bibliometric approach.

BACKGROUND: During coronavirus pandemic, despite the increase in the number of studies on spontaneous pneumothorax (SP), there is not enough bibliometric study in the literature. In this study, it was aimed to analyze scientific articles published on SP. METHODS: Studies published on SP between 1980 and 2021 were obtained from the Web of Science database and analyzed using statistical and bibliometric methods. Spearman correlation coefficient was used for correlation studies. The exponential smoothing estimator was used to forecast publication trend for coming years. Network visualization map

[Catamenial pneumothorax].

Thoracic endometriosis syndrome (TES) is a rare disorder characterized by the presence of ectopic endometrial tissue in the chest cavity. The typical clinical manifestation is a spontaneous pneumothorax, which usually presents with chest pain, dyspnea, and/or cough. The diagnosis requires a high level of clinical suspicion and a complete gynecological history. Imaging studies can help with the diagnosis, although the gold standard is video-assisted thoracoscopic surgery (VATS). Surgical treatment in combination with at least 6 months of hormonal medical treatment has been shown to improve the

Application of "suction-guided stapling" during uniportal thoracoscopic major lung resection.

Uniportal thoracoscopic major pulmonary resection is used worldwide as a minimally invasive surgery. Occasionally, it is difficult to insert a stapler smoothly during uniportal thoracoscopic major pulmonary resection because of limited angulation. To address this challenge, we used "suction-guided stapling" to divide the bronchus or pulmonary vein. Here, we provide details of this technique, including division of the pulmonary veins or bronchus in a video. In addition, we validate this technique by showing perioperative results of uniportal thoracoscopic major pulmonary resections in our depar

Obstructive Shock Because of the Incorrect Setup of a Digital Drainage System After Pulmonary Lobectomy.

We report a case of life-threatening obstructive shock from extreme negative intrapleural pressure that occurred because of the incorrect setup of a digital drainage system after video-assisted thoracic surgery. These devices are increasingly prevalent because of their precision and portability. Those using such devices should be cognizant of their potential pitfalls.

Video-assisted thoracoscopic surgery for the treatment of pericardial hydatid cyst.

Introduction: We would like to present video-assisted thoracoscopic surgery for the treatment of pericardial hydatid cyst.Patient and Method: Thirty two-year-old female patient was referred for evaluation of a left mediastinal mass. Magnetic resonance image, computed tomography and transesophageal echocardiography confirmed a well circumscribed mass with compatible hydatid cyst at the left pericardial wall end of the differantial diagnosis. Our surgical plan entailed the endoscopic resection of pericardial hydatid cyst. We did not adopt a more precise strategy such as open procedure. We manage
