BACKGROUND: In patients with anterior circulation large vessel occlusion stroke, hyperdense areas (HDAs) on post-thrombectomy computed tomography (CT) reflect blood-brain barrier injury and may herald malignant edema or hemorrhagic transformation. Both complications can progress to life-threatening mass effect, often necessitating surgical interventions such as decompressive craniectomy. We aimed to develop and validate a location-specific HDA score for predicting severe mass effect.
METHODS: This multicenter retrospective study analyzed 865 patients exhibiting HDAs on post-thrombectomy CT. T…

OBJECTIVE: Endovascular thrombectomy (EVT) has been recommended as first-line therapy for patients with acute large-core ischemic stroke, yet the mortality remains high. We aimed to identify independent predictors of death within 90 days after EVT in this population.
METHODS: This is a multicenter cohort study across 38 Chinese comprehensive stroke centers (November 2021-February 2023). Acute ischemic stroke patients with anterior circulation large vessel occlusion with ASPECTS ≤5, who underwent EVT were included. Independent mortality predictors of 90-day mortality were identified using mult…

BACKGROUND: The effect of postthrombectomy blood pressure (BP) management on the development of acute kidney injury (AKI) in patients with acute ischemic stroke remains largely unexplored.
METHODS: This secondary analysis of the OPTIMAL-BP trial (Outcome in Patients Treated With Intra-Arterial Thrombectomy-Optimal Blood Pressure Control) included patients with acute ischemic stroke due to large-vessel occlusion who achieved successful endovascular thrombectomy and had a systolic BP ≥140 mm Hg. Patients were randomized to intensive (target systolic BP <140 mm Hg) or conventional (target systol…

Mechanical thrombectomy can provide rapid, fibrinolysis-free restoration of venous flow in phlegmasia cerulea dolens. This case highlights an unusually severe course with extensive iliofemorocaval thrombosis, rhabdomyolysis, MRI-confirmed muscle necrosis, and arterial hypoperfusion despite anticoagulation, in which urgent intervention achieved limb salvage.

Derangement of liver function is a rare, but known, complication of percutaneous pharmacomechanical thrombectomy (PMT). PMT can cause intravascular hemolysis, which results in cell-mediated and direct hepatocellular toxicity. We report the case of a patient who suffered such a complication, and recovered with close monitoring of liver function in an intensive care setting. Further studies will elucidate the risk factors for and mechanisms of post-thrombectomy acute hepatitis, and providers should maintain a high index of suspicion in patients who have recently undergone PMT.

BACKGROUND: Mechanical thrombectomy (MT) is the standard of care for qualifying patients with acute ischemic stroke (AIS). Minimal postprocedural guidelines often result in patients being admitted to the intensive care unit (ICU) for 24 h, even if they did not receive intravenous thrombolysis (IVT). Whether or not these patients consistently use ICU resources has not been well studied. This study aims to assess the use of ICU resources in post-thrombectomy patients who did not receive IVT.
METHODS: A single-center retrospective chart review was performed for all patients with AIS who were at …

BACKGROUND: Acute pulmonary embolism (PE) with medical treatment results in high 30-day mortality rates, even when the patients present as hemodynamically stable. Catheter-directed treatment emerges as a viable first-line therapeutic approach for such patients (intermediate-high risk PE).
CASE SUMMARY: The authors describe the successful treatment of a patient with angiographically massive but clinically only intermediate-high PE, using percutaneous transcatheter mechanical thrombectomy, resulting in removal of an embolus of a rather unusually long length. The patient's clinical condition rap…

BACKGROUND: The role of coronary thrombectomy during percutaneous coronary intervention (PCI) remains controversial. While early studies showed potential benefits, subsequent larger trials failed to confirm and raised concerns about increased stroke risk. Yet, thrombectomy remains a straightforward and widely available technique that may have value in selected high-thrombotic scenarios. This study aimed to assess the safety and efficacy of selective coronary thrombectomy during PCI in a real-world, single-center setting.
METHODS: We conducted a retrospective cohort study including 2658 consec…

BACKGROUND: A higher number of recanalization attempts reduces the efficacy of endovascular thrombectomy (EVT) for acute ischemic stroke secondary to large vessel occlusion (LVO). We assessed the impact of switching EVT techniques after a failed first pass on procedural and clinical outcomes.
METHODS: This multicenter international study, conducted between January 2013 and December 2022, included patients undergoing EVT for anterior circulation LVO (internal carotid artery or M1 segments) with failed first pass recanalization. Propensity score matching identified a 1:1 matched cohort of patie…

INTRODUCTION: Poststroke epilepsy (PSE) is an important long-term complication after stroke. Data regarding predictors of PSE in patients with large-vessel occlusion stroke receiving mechanical thrombectomy (MT) are scarce. Voxel-based lesion symptom mapping on brain MRI might be a valuable tool in the risk prediction of PSE. This study aims to assess PSE risk after acute stroke treated with MT via voxel- and volumetric-based analyses.
METHODS: In this bi-center study from two tertiary-care stroke centers, we included consecutive acute ischemic stroke patients who had received MT between 2011…

BACKGROUND: Endovascular thrombectomy (EVT) is a safe and efficacious treatment of choice for acute ischemic stroke (AIS) patients due to large artery occlusion in the anterior circulation. Despite these achievements, some patients still require decompressive craniectomy (DC) even after undergoing a timely EVT. Identifying patients requiring post-EVT DC is crucial to improve the clinical outcome, even though signs and symptoms at that period may not be reliable. In this study, we aimed to investigate risk factors for DC after EVT.
METHODS: A systematic review was conducted from inception to 2…

OBJECTIVE: Mechanical thrombectomy is constantly advancing in terms of devices and techniques to enhance outcomes, and it is now commonly performed worldwide. Delayed stenosis is a potential complication that can occur following the procedure. However, its pathophysiology, risk factors, and time of onset remain unclear. We report 2 cases of delayed severe stenosis after mechanical thrombectomy.
CASE PRESENTATION: Case 1: A 70-year-old man was referred to our hospital with a right middle cerebral artery occlusion. We performed mechanical thrombectomy promptly. Complete recanalization was achie…

BACKGROUND: The transradial approach (TRA) for mechanical thrombectomy (MT) for acute ischemic stroke has been limited by the size of catheters usable in the radial artery, with the smaller access site precluding balloon-guide catheter (BGC) use. However, promising results have been reported for a TRA with a sheathless BGC (sTRA). We sought to perform a comparative study of MT with a BGC via the sTRA versus the transfemoral approach (TFA).
METHODS: A retrospective review of our MT database was conducted. Baseline, procedure-related, and outcome data were compared for patients aged ≥18 years w…
