Adrenal androgen hypersecretion either produced by genetic defects or reticular disfunction, is reduced by exogenous glucocorticoid administration and, as with any suppression therapy, it should relapse when the therapy is discontinued. A 48 years old male is reported. He presented with lower limb progressive and severe polyneuropathy, hypertrichosis, endocrinological alterations (hypothyroidism and hypogonadism) and organomegaly (hepatosplenomegaly and lymphadenopathies). This syndrome was associated with an osteosclerotic myeloma. The patient died two months after admission.

The approach to adrenal hyperandrogenism, due to genetical or non-genetical enzymatic alterations, has changed dramatically during the last 50 years. To allow a better understanding of the subject, we focused it from a historical perspective, defining four stages analyzed in detail. Now there is consent on the existence of a functional adrenal hyperandrogenism, which has an important role in acne, hirsutism and menstrual disturbances.

Hepatitis C virus infection in chronic hemodialysis patients is associated with several unresolved problems. We report a 85 years old female patient in chronic hemodialysis and treated with erythropoietin, that during the course of an Herpes zoster, presented severe malaise, weight loss and muscle weakness. Two weeks later, a slight rise in serum transaminases was detected. The patient had negative antibodies for HIV and hepatitis C virus and negative hepatitis B surface antigen. Nocardia asteroide infections, an aerobic actinomycete, have several forms and lungs, skin and brain are the organs most frequently involved. When the infection suspected, special staining methods must be ordered to identify the agent. We report three immunocompetent patients with disseminated nocardiosis, two presenting with nodular lesions of skin and lungs and one presenting with pulmonary involvement and brain abscesses. The importance of clinical suspicion and early diagnosis of nocardiosis is emphasized.

ELISA techniques using monoclonal antibodies have been recently developed for the detection of Cryptosporidium parvum fecal antigens. The aim of this work was to compare the yield of these ELISA techniques with Ziehl-Nielsen and Safranin stains in formaldehyde-salt fixed samples. Between 1991 and 1994, eight external quality assessments of Cryptosporidium parvum infection diagnosis were performed. Stool specimens were sent to 74 laboratories, to be stained and examined. In 40 laboratories (54.1%), the diagnosis was correct in all evaluations, whereas in 34 (46%), the diagnosis disagreed with the parasitology reference laboratory in one occasion or more. Between December 1987 and July 1992, we performed a balloon mitral valvuloplasty to 300 patients aged 48 +/- 23 years, with pure or predominant, symptomatic mitral stenosis, with an hemodynamic area < 1.5 cm2 and a mean echocardiographic score of 8.8 +/- 1.3 (6-13). Young subjects with mobile and flexible valves as well as elders with highly damaged valves were included. A transeptal technique employing 2 balloons was used in 97% of cases. Six patients with severe rosacea were treated with oral isotretinoin during 3 to six months and followed for two years. All had a good response with quick remission of papules and pustules. Collateral effects were minimal and tolerable. Four patients has an ophthalmological assessment (one with a severe keratitis), confirming a beneficial effect of the drug. During follow up, patients remained asymptomatic during a mean of 14.8 months.

Vertebral artery dissection seems to be a frequent cause of stroke in young adults. Hepatitis C virus antibodies were studied in sera coming from 39 patients with porphyria (cutanea tarda in 17, variegate in 8, intermittent acute in 4, coproporphyria in 2 and protoporphyria in 8). Six patients with primary degenerative dementias and frontal or frontotemporal hypoperfusion are reported. Five were diagnosed as Frontal Progressive Dementia (FPD), characterized by striking changes in personality and social behavior. The last patient had a progressive aphasia and a moderate cognitive impairment. We discuss the heterogeneity of FPD that includes Pick's disease, frontal lobe type dementia (Neary) and progressive subcortical gliosis. The usefulness of SPECT and the need of a brain bank are remarked.

We report a non alcoholic, Chinese, 65 years old male that presented in august, 1992 with painful hepatomegaly of rapid installation and malaise. We report a woman with insulin resistance associated with hyperandrogenism and acanthosis nigricans (HAIR-AN syndrome) treated during 30 days with the prolonged action opioid antagonist Naltrexone. During its administration, decreases in basal blood glucose and serum insulin, insulin and glucose response to a glucose load and plasma testosterone were observed. These findings suggest that opioid activity could play a critical role in the physiopathology of hyperinsulinemia in hyperandrogenic women.

We report a 29 years old man with a choledochocele or type III choledochal cyst, an extremely rare condition. This was found during a cholecystectomy using a transcystic intra operative cholangiogram. A transduodenal sphincteroplasty was performed. The cyst diameter was 1.5 cm approximately and two 1-2 mm stones were extracted from its lumen. The chemical composition of these was different from those located in the gallbladder. The postoperative period was uneventful and the patient remains asymptomatic.

We report four patients with primary hypothyroidism (3 female, aged 47, 46, 49 and 34 years old) whose principal manifestation was muscle involvement. The diagnosis was oriented by the finding of myoedema and a slow relaxation phase of tendon reflexes. In one patient, an electromyographic and histological study was performed. Thyroid substitution lead to an improvement in myopathy in all patients.
