Ultrasound Imaging of the Superficial Fascial System Can Predict the Subcutaneous Strength of Abdominal Tissue Using Mean Gray Value Quantification.

BACKGROUND: This study is the first to correlate sonographic findings of subcutaneous tissue to structural and biomechanical properties. METHODS: Precisely sized tissue specimens (59 samples) were collected from five abdominoplasty procedures. A Philips Lumify L12-4 linear array probe, connected to an Android tablet, was used to obtain ultrasound images of the superficial fascial system. A no. 1 Vicryl suture on a CTX needle placed though a needle guide within a three-dimensionally printed template ensured equal bites of subcutaneous tissue across specimens. Suture pull-out strength was measu

"It's not just hacking for the sake of it": a qualitative study of health innovators' views on patient-driven open innovations, quality and safety.

BACKGROUND: Open do-it-yourself (DIY) health innovations raise new dilemmas for patient-oriented and service-oriented scholars and healthcare providers. Our study aimed to generate practical insights into quality and safety issues to patient care raised by two volunteer-run, open DIY solutions: Nightscout Project (patient-driven, open-source software for type 1 diabetes management) and e-NABLE (volunteers who design and three-dimensionally print upper-limb assistive devices). To this end, we examined the views of health innovators who are knowledgeable about medical devices standards and regul

Aerodynamically driven phonation of individual vocal folds under general anesthesia in canines.

OBJECTIVES/HYPOTHESIS: We previously developed an instrument called the Aerodynamic Vocal Fold Driver (AVFD) for intraoperative magnified assessment of vocal fold (VF) vibration during microlaryngoscopy under general anesthesia. Excised larynx testing showed that the AVFD could provide useful information about the vibratory characteristics of each VF independently. The present investigation expands those findings by testing new iterations of the AVFD during microlaryngoscopy in the canine model. STUDY DESIGN: Animal model. METHODS: The AVFD is a handheld instrument that is positioned to cont

High tibial osteotomy with virtual planning and patient specific instrumentation: a narrative review.

BACKGROUND AND OBJECTIVE: High tibial osteotomy (HTO), particularly through a medial opening-wedge technique [medial opening-wedge high tibial osteotomy (MOWHTO)], is a well-established surgical intervention for correcting varus malalignment and unloading the medial compartment in younger patients with early osteoarthritis or secondary ligamentous or chondral deficiency. However, conventional methods pose high technical demands for accurate alignment, often requiring repeat intraoperative fluoroscopy. Patient-specific instrumentation (PSI), developed through three-dimensional (3D) imaging and

Cranioplasty with a low-cost customized polymethylmethacrylate implant using a desktop 3D printer.

OBJECTIVE: Cranioplasty implants should be widely available, low in cost, and customized or easy to mold during surgery. Although autologous bone remains the first choice for repair, it cannot always be used due to infection, fragmentation, bone resorption, or other causes, which led to use of synthetic alternatives. The most frequently used allogenic material for cranial reconstructions with long-term results is polymethylmethacrylate (PMMA). Three-dimensional printing technology has allowed the production of increasingly popular customized, prefabricated implants. The authors describe their

Clinical outcomes of internal iliosciatic fixation using a conventional reconstruction plate via an anterior approach for acetabular fractures involving the posterior column.

BACKGROUND: Acetabular fractures involving the posterior column remain surgically challenging because of the complex three-dimensional anatomy and restricted operative corridor. As an alternative to traditional posterior approaches, internal iliosciatic fixation through an anterior approach has gained increasing acceptance. Reconstruction plates, which offer the advantages of low cost and convenient intraoperative bending, are extensively used in clinical settings. METHODS: A retrospective case series was conducted on 36 consecutive patients with acetabular fractures involving the posterior c

Development and validation of a pregnant phantom with adjustable fetal presentation for region-specific fetal radiation dose assessment during pelvic radiography.

BACKGROUND: Accurate assessment of fetal radiation dose during diagnostic X-ray examinations remains challenging because commercially available pregnancy phantoms do not adequately reproduce fetal anatomy or permit organ-specific dose measurements. PURPOSE: The purpose of this study was to develop a pregnant phantom with adjustable fetal presentation and to evaluate its anatomical realism and applicability for fetal dose assessment during pelvic radiography. METHODS: The developed phantom comprised an anthropomorphic phantom, a whole-body fetal phantom, acrylic components, and water-filled p

3D printed intelligent scaffold prevents recurrence and distal metastasis of breast cancer.

Rationale: Tumors are commonly treated by resection, which usually leads to massive hemorrhage and tumor cell residues, thereby increasing the risk of local recurrence and distant metastasis. Methods: Herein, an intelligent 3D-printed poly(lactic-co-glycolic acid), gelatin, and chitosan scaffold loaded with anti-cancer drugs was prepared that showed hemostatic function and good pH sensitivity. Results: Following in situ implantation in wounds, the scaffolds absorbed hemorrhage and cell residues after surgery, and promoted wound healing. In an in vivo environment, the scaffold responded to the

The fitting accuracy of pre-bend reconstruction plates and their impact on the temporomandibular joint.

BACKGROUND: Various causes for bone defects of the lower jaw have been described. As a result, patients often suffer from compromised aesthetics and a loss of, or reduction in, important physiological functions, such as swallowing, breathing, and speaking. A change in the shape of the lower jaw can impair the natural occlusion and leads to an atypical or modified position of the temporomandibular joint. Titanium reconstruction plates are the standard approach to jaw reconstruction, and are used for temporary bridging of a jaw defect or fixation of a bone graft. Conventionally these plates are

Comparative Efficacy of Er,Cr:YSGG Laser, Electrolytic Cleaning, and Chlorhexidine for the Decontamination of Sandblasted, Large-Grit, Acid-Etched (SLA) Implant Surfaces: An In Vitro Controlled Study.

BACKGROUND: Predictable decontamination of exposed implant surfaces remains difficult to achieve in peri-implantitis therapy. Rough sandblasted, large-grit, acid-etched (SLA) surfaces and implant thread geometry may promote biofilm retention and limit the effectiveness of chemical antiseptics and mechanical instrumentation. OBJECTIVE: This controlled in vitro study compared the antimicrobial efficacy of three clinically relevant decontamination approaches: 0.12% chlorhexidine (CHX), GalvoSurge electrolytic cleaning, and erbium, chromium: yttrium-scandium-gallium-garnet (Er,Cr:YSGG) laser irra

Endoluminal approaches to postoperative air leaks.

Postoperative air leaks occur in up to 50% of patients following pulmonary resection, with 8-10% progressing to prolonged air leak (PAL) beyond postoperative day 5. PAL is associated with increased risk of empyema, reoperation, readmission, and prolonged hospitalization, imposing a substantial clinical and economic burden. This chapter provides a comprehensive review of the pathophysiology, risk stratification, diagnostic evaluation, and contemporary endoluminal treatment options for postoperative air leaks. Several preoperative prediction models-including the index of persistent air leak (IPA

Simulation of calcium blooming artifacts in coronary CT angiography from digital phantoms.

BACKGROUND: Calcium blooming artifacts, predominantly caused by the partial volume effect, may limit the accuracy of coronary stenosis assessment on coronary computed tomography angiography (CTA). Existing comparative tests or physical phantoms to evaluate de-blooming methods are costly, time-consuming, and limited in geometry and materials. This study developed a computational method to simulate CTA images incorporating calcified plaques from digital phantoms to help understand the characteristics of blooming artifacts. METHODS: Digital vascular phantoms with different plaque geometries and

Quinone-Based Mediator Immobilized Mesoporous Electrodes for Bioelectrocatalysis of Glucose Dehydrogenase.

Bioelectrocatalysis, which couples enzymatic catalytic and electrode reactions, is the basic principle behind enzyme electrochemical biosensors, biofuel cells, and bioelectrosynthesis systems. Electron mediators facilitate the transfer of electrons between electrodes and oxidoreductases during catalytic reactions, so ideally the mediator remains near the electrode for higher catalytic efficiency. Additionally, electrode materials with three-dimensional space at the enzyme volume mesoscale are expected to improve electrode performance (e.g., increased current density and stability). In previous
