The auditory brainstem response generated by clicks was studied in 119 normal newborns. One ear was studied in detail at 20, 40 and 60 dBnHL, the other at 20 and 30 dBnHL, increasing the intensity if no response was found. The latency for waves I, III and V in the detailed studied ear was 2.7 +/- .39, 5.4 +/- .44 and 7.7 +/- .48, respectively. 85% of newborns had thresholds for auditory brainstem response less than 40 dBnHL, 54% less than 20 dBnHL and 15% between 40 and 60 dBnHL. 77 variables with data from clinical, non invasive and invasive findings in 591 adult patients submitted to cardiac catheterization and/or coronary arteriography from 1988 to 1991 were analyzed. 51% had coronary arteriography for suspected coronary artery disease: 15% had no or insignificant disease and 8% had main left disease. Hypomagnesemia is a serious abnormality with different causes and usually associated to other disorders of electrolyte metabolism. We report a female patient developing hypomagnesemia after administration of gentamycin. This was associated to severe hypokalemia, hyponatremia and metabolic alkalosis. Possible pathogenetic mechanisms and therapeutic measures are discussed.

A possible association of cardiovascular risk factors and early diabetic nephropathy was investigated in 32 patients. Microalbuminuria (radioimmunoassay), total and HDL cholesterol and triglycerides (enzymatic methods), glycosylated hemoglobin (colorimetric methods), Apo A1 and B (immunonephelometric) and LDL were measured. Microalbuminuria was present in 28% of patients. Absolute or functional iron deficiency decreases the effectiveness of erythropoietin in patients undergoing hemodialysis. We describe a patient who developed pica associated to a ferritin level of 800 ng/ml during recombinant human erythropoietin treatment. The symptom subsided after supplementation with iron dextran. Therefore we recommend iron supplementation during the initial phase of treatment with erythropoietin until serum ferritin levels raise above 1000 ng/ml.

We report the pregnancy of a 35 year old diabetic woman with a positive Australia antigen, 3 years after a successful cadaver kidney transplant. Immunosuppressive therapy with prednisone, azathioprine and cyclosporine was maintained. Pregnancy was complicated by an acute rejection episode on week 13, mild hypertension and intrahepatic cholestasis on week 22. We conducted a double blind random study on 79 patients with gastric ulcer: 39 received sucralfate, 1 g 4 times a day (Group 1) and 40 received a single evening dose of famotidine, 40 mg (Group 2). At 4 weeks, endoscopy revealed healing of the ulcer in 46% of patients in Group 1 and 40% in Group 2 (NS). At 8 weeks, corresponding figures were 90% and 75% (NS). A patient with AIDS developed pulmonary infiltrates typical of infection by P. carinii. However, therapy for this agent was unsuccessful. Further studies including blood cultures revealed the presence of C neoformans. Specific therapy for this agent led to recovery. Conventional preparation of goitrous hyperthyroid patients using lugol and propranolol may take 2 weeks. This period may be shortened using sodium iopodate and dexamethasone. We used 500 mg of sodium iopodate and 1 mg dexamethasone for 4 days in 34 hyperthyroid patients. Sixty-three patients with severe acute pancreatitis have been studied. Pancreatitis was associated with biliary tract disease in 23 patients (36.5%) and with alcoholism in 21 (33.3%). The fistulization of a hepatic hydatid cyst to the colon is a very rare complication and only two cases have been described in the medical literature. We report a 49 year-old male in whom this complication was suspected in the preoperative period. He was submitted to a cystotomy and colonic exteriorization. Recovery was uneventful.

A 24 year old female with a stage III-B Hodgkin disease involving the mediastinum was subjected to chemotherapy and supradiaphragmatic radiotherapy. A year later she developed heart failure. LV dilatation and decreased systolic function with anteroseptal and lateral hypokinesia and a small pericardial effusion were shown by X Ray and echocardiography. An initial clinical diagnosis of radiation heart disease was changed to myocardial involvement by Hodgkin disease after performing and endomyocardial biopsy.

Popliteal aneurysm is a rather infrequent lesion which may cause severe ischemia and loss of the extremity. Between 1975 and 1990, 21 patients, all male with a mean age of 73 years, presented with popliteal aneurysm which was associated to aneurysm in other arteries in 62% and other medical problems in 95% of patients. 61% of patients had severe ischemia due to thrombosis or embolism. The sea-blue histiocyte syndrome, similar to Niemann-Pick disease, is a congenital, hereditary histiolipidosis due to an inborn enzymatic error. Accumulation of non saturated, oxidated, polymerized lipids is observed; ceroids of lipofuscin, glycophospholipids and sphingomyelin, like bulky granules 1 to 3 u in diameter, turn blue with May Grunwald staining, orange reddish with PAS and black with Sudan III and osmic acid.
