Mesenteric masses can considerably shift positions between fractions of radiation therapy (RT). With image-guided RT, we can adapt our treatment delivery daily to target the mass while avoiding surrounding structures. We report the case of a highly-mobile mesenteric mass which required adaptive planning from a traditional “butterfly” intensity modulated radiation therapy (IMRT) plan to an anterior “rainbow” beam arrangement to permit shifting of the isocenter without compromising target coverage or increasing dose to organs-at-risk.

The internal nasal valve, the narrowest portion of the nasal airway, is prone to collapse and is often targeted for improvement in nasal reconstruction and rhinoplasty. Endonasal techniques can reduce surrounding trauma and reduce operative times compared to traditional open methods. Options include the use of spreader, butterfly and alar batten grafts, suspension and flaring sutures, and Z-plasty for scarring. A 47-year-old man with ichthyosis vulgaris presented to our hospital complaining of reduced visual acuity and ocular discomfort in the left eye. Slit-lamp biomicroscopy revealed a thinning about 2mm from the superior limbus and superficial punctate corneal lesions in the left eyes. Corneal topography was 'butterfly-like' in an area of increased elevation in the left eye. Although ichthyosis vulgaris and unilateral superior pellucid marginal degeneration are both uncommon conditions, this is first report about these two conditions in studied together.

Klippel-Feil syndrome, or synostosis of the cervical spine, is the result of an abnormal division of somites during embryonic development. This report analyses an adult male (exhumed from a Portuguese graveyard dating from the 13th to the 15th century) with malformations in the cranium and vertebral column. Besides the lesions that are typical of Klippel-Feil syndrome type II, other defects usually linked to this pathology are described (occipito-atlantal fusion, hemivertebrae, butterfly vertebrae, cervical rib, changes in normal number of vertebral segments and a possible Sprengel deformity).

Sonographic evaluation of the fetal spine in a second-trimester, low-risk patient revealed segmentary, low thoracic vertebral abnormalities, including hemi- and butterfly vertebrae, resulting in severe fetal scoliosis with fetal paraplegia and bilateral club-feet. X-ray and magnetic resonance imaging studies of the abortus confirmed the prenatal vertebral findings in addition to rib defects and transection of the spinal cord. We believe that these findings which, to the best of our knowledge, have not been previously reported, represent a new variant of occult spinal dysraphism.

The cutaneous manifestations of lupus erythematosus (LE) include LE-specific and LE-nonspecific skin lesions. LE-specific skin lesions are divided into chronic, subacute, and acute types. The representatives of the chronic and acute types are discoid lupus erythematosus (DLE) and butterfly rash, respectively. Based on the systemic manifestations, we can classify LE into cutaneous-limited LE and systemic LE (SLE). Chronic LE eruptions tend to be seen in cutaneous-limited LE, and acute LE eruptions mainly appear in SLE. Some skin lesions are related to the neurological manifestations of SLE.
