Primary ALK-positive anaplastic large cell lymphoma of the urinary bladder in a pediatric patient: a case report and review of literature.

BACKGROUND: Primary lymphomas of the urinary bladder are rare, most of which are low-grade B-cell lymphomas. Primary anaplastic large cell lymphoma (ALCL) with bladder involvement is extremely rare, with most cases seen in adult patients. Here, we report a case of ALK-positive ALCL in a 14-year-old male. CASE PRESENTATION: A 14-year-old male presented with several months of gross hematuria that had become painful. Computed tomography (CT) urogram showed a mass-like thickening of the anterior bladder wall. Cystoscopy showed a 3.5-cm pedunculated lesion at the bladder dome; transurethral excisi…

Evaluation, etiology, and management of gross hematuria following radical cystectomy and orthotopic neobladder.

OBJECTIVE: To assess the etiology, evaluation, and subsequent management of patients who developed gross hematuria following radical cystectomy (RC) and orthotopic neobladder (ONB) urinary diversion. METHODS: All patients who underwent RC and ONB between 2017 and 2021 and later developed gross hematuria necessitating cystoscopy were identified. Patient demographics, cystoscopy reports, cytology findings, and relevant imaging were reviewed to determine the etiology of hematuria, the subsequent management, and the ultimate outcome. Causes of hematuria were categorized as recurrence, inflammatio…

Accuracy of pre-operative fistula diagnostics in anorectal malformations.

BACKGROUND: Surgical safety during posterior sagittal anorectal plasty (PSARP) for anorectal malformations (ARM) depends on accurate pre-operative fistula localization. This study aimed to evaluate accuracy of pre-operative fistula diagnostics. METHODS: Ethical approval was obtained. Diagnostic accuracy of pre-PSARP symptoms (stool in urine, urine in passive ostomy, urinary tract infection) and examination modalities (voiding cystourethrogram (VCUG), high-pressure colostogram, cystoscopy and ostomy endoscopy) were compared to final intra-operative ARM-type classification in all male neonates …

Ureteroarterial fistula embolization by transradial approach: A case report.

Ureteroarterial fistula is a rare condition wherein a communication develops between a ureter and the common, internal, or external iliac artery. Localizing the fistula can be difficult, as cystoscopy, CT angiography, and conventional angiography have low sensitivity in identifying the fistula. Provocative maneuvers within the ureter, however, can aid in the visualization of fistulae on angiography. Prior reports of endovascular repair have utilized transfemoral access, which makes performing concurrent provocative maneuvers in the ureter challenging. We present a case of successful endovascul…

Bilateral bladder erosion of a transobturator tape mesh.

BACKGROUND: The transobturator tape procedure is reported to be an effective procedure with low complication rates. CASE: A 45-year-old woman underwent surgery for prolapse and incontinence. The surgery included transobturator tape. Intraoperative cystoscopy was not performed. Postoperatively, a mesh erosion into the bladder on the left side and a large cystocele were diagnosed. The patient underwent a combined transurethral and suprapubic mesh resection. Six months later, she had another mesh erosion on the contralateral side. This time, a complete vaginal resection of the mesh was performed…

Pancreas transplantation and dysuria.

Bladder drainage of the pancreatic graft remains the most common technique in pancreatic transplantation. Graft and patient survival is superior to duct injection and bowel drainage techniques. However, several urological complications develop as a result of bladder drainage. We report 6 cases of the dysuria syndrome after combined kidney/pancreas transplantation. Retrograde urethrography or cystoscopy showed extravasation from the bulbous urethra in all cases, which was most likely caused by activated proteolytic exocrine enzymes from the transplanted pancreas. Minor urethral trauma due to Fo…

Urothelial papilloma: a rare cause of gross haematuria in childhood.

Bladder urothelial papilloma is extremely rare in the paediatric population. It usually presents as painless gross haematuria and its diagnosis implies a high index of suspicion as other causes of haematuria predominate in this age range. We describe a 9-year-old boy with two episodes of gross haematuria occurring 1 year apart with spontaneous resolution after 2 days. Bladder ultrasound revealed an endovesical papillary lesion of 24×24 mm suggestive of bladder tumour. The diagnosis was confirmed by histopathological examination of the specimen obtained by cystoscopy with transurethral resectio…

Metastatic breast cancer presenting as detrusor overactivity.

Breast carcinoma metastatic to the bladder is rare and accounts for approximately 3% of all secondary bladder neoplasms. We examine a case of breast cancer metastatic to the bladder with normal findings at cystoscopy. A 53-year-old woman with a history of breast carcinoma presented with a 6-month history of severe urgency and urgency incontinence. Treatment with multiple antimuscarinic therapies was unsuccessful. Vaginal examination demonstrated a non-mobile uterus with a suggestion of parametrial thickening. Urodynamic studies confirmed detrusor overactivity. CT showed a thickened bladder wal…

The first case of vesico-vaginal fistula in a patient with primary lymphoma of the bladder - a case report.

BACKGROUND: Primary lymphoma of the bladder is a rare condition with less than 100 cases reported in the literature. CASE PRESENTATION: Here we present the case of a 64 year old woman with a 9 month history of haematuria, frequency, urinary incontinence and weight loss. Cystoscopy revealed a solid tumour throughout the whole bladder wall and a vesico-vaginal fistula. This was confirmed on CT scan which also showed no other organ involvement and bilateral hydronephrosis. Trans urethral biopsies taken at cystoscopy revealed Non-Hodgkin's Lymphoma. The patient had a nephrostomy inserted and is c…

Enterovesical fistula caused by a toothpick.

We present a case of enterovesical fistula caused by an accidental ingestion of a foreign body. A 23-year-old man presented to our hospital with pneumaturia, fecaluria, and abdominal pain but no recent possible causes of enterovesical fistula at anamnesis. Cystoscopy, cystography, and also colonoscopy were not able to detect the fistulous tract. Computer tomography (CT) revealed a fistula between bladder and bowels caused by a toothpick accidentally swallowed 2 years earlier. We tried to remove the foreign body endoscopically by cystoscopy and colonoscopy but with no success. The failure of en…

Diagnosis and treatment of bladder cancer.

Bladder cancer is the sixth most prevalent malignancy in the United States. The most common type of bladder cancer is urothelial (transitional cell) carcinoma, and cystoscopy remains the mainstay of diagnosis and surveillance. Fluorescence cystoscopy offers improvement in the detection of flat neoplastic lesions, such as carcinoma in situ. Non-muscle-invasive bladder cancer is typically managed with transurethral resection and perioperative intravesical chemotherapy. Intravesical bacille Calmette-Guérin therapy is preferred over mitomycin for those at high risk of disease progression. For musc…
