Allopurinol is often prescribed for the treatment of hyperuricemia. It inhibits the uric acid production binding tightly to xanthine oxidase. Although it is generally well tolerated, an almost 10% prevalence of adverse reactions has been reported, particularly gastrointestinal and neurological effects. Some hypersensitivity syndromes have also been described (rash, vasculitis or exfoliative dermatitis). In these cases, if a substitute treatment is not available, a desensitization procedure to the drug must be considered. We present three patients with cutaneous hypersensitivity to allopurinol,

Specific allergen injection immunotherapy is highly effective in selected patients with IgE-mediated disease, including respiratory allergy and venom anaphylaxis. Research in this area provides insight into the immunologic basis of allergic disease and may assist in the development of more highly targeted treatment. Immunotherapy reduces immediate allergen-induced symptoms and concentrations of inflammatory mediators, including histamine and prostaglandin D2, in ragweed-sensitive patients. Similarly, nasal epithelial mast cell numbers are decreased. A characteristic feature of immunotherapy is

BACKGROUND: Hypersensitivity reactions to rifampin are relatively uncommon, but they may result in cessation of therapeutic medications.
PATIENTS AND METHODS: We report our experience with oral desensitization protocol to rifampin in a group of 35 HIV-positive patients with mycobacterial disease who had some hypersensitivity reaction to this drug.
RESULTS: Adverse reactions with this protocol were few and easily treated.
CONCLUSIONS: Oral desensitization to rifampin is safe and effective, allowing some of these patients (60%) to reintroduce the drug and to reduce the time of treatment.

Several controlled clinical trials have shown that specific immunotherapy (SIT) using incremental injections of allergens can be effective in the treatment of allergic rhinitis and asthma. Nevertheless, the risk of side effects have led to some recommended limitations of SIT. Enzyme-potentiated desensitization (EPD) is a proposed method for immunotherapy with very low doses of mixed allergens plus beta-glucuronidase enzyme, for which irrelevant or no side effects have been claimed. The aim of this study was to determine the clinical efficacy of EPD in the treatment of pollinosis. A double-blin

Systemic allergic reactions following insect stings, most often by hymenoptera (honey bee, wasp), are usually of the immediate type and thus mediated by venom specific IgE-antibodies. The most frequent clinical symptoms are urticaria, angioedema, asthma and anaphylactic shock. While in epidemiological surveys up to 4% of individuals report a history of systemic allergic reactions following hymenoptera stings, fatal cases are extremely rare. Diagnosis is based on history, skin tests and estimation of venom specific serum IgE-antibodies (RAST). For treatment of mild systemic reactions antihistam

Dendritic cells (DCs) play a pivotal role in regulating immune responses, encompassing both tolerance and activation. Dysregulation of DC function is fundamental in the development of allergic conditions, underscoring their significance as a major focus for allergen-specific immunotherapy (AIT). This review systematically delineates the intricate landscape of DC subsets, comprising conventional DCs, plasmacytoid DCs, monocyte-derived DCs, and Langerhans cells, elucidating their specific contributions to either fostering Th2-driven inflammation or driving regulatory T cell generation. Highly sensitized kidney transplant candidates are difficult to transplant because anti-human leukocyte antigen (HLA) sensitization restricts access to immunologically compatible organs, as transplantation across donor-specific antibodies (DSA) increases the risk of antibody-mediated rejection (AMR) and premature graft loss. Management should therefore move beyond antibody characteristics alone and assess whether a compatible offer remains realistic for the individual patient within the relevant allocation system. This review proposes a sequential, compatibility-first access framework. Compati

PURPOSE OF REVIEW: To outline the current advances in food allergy.
RECENT FINDINGS: Food allergies have been increasing in prevalence and significantly affect the quality of life of both patients and their families. Over the past decades, there have been major changes to the recommendations for prevention and management of food allergies. These include timely introduction, various forms of immunotherapy, biologics, and epinephrine administration.
SUMMARY: As the effectiveness of many of these therapies are optimized in childhood, it is critical to be aware options are available. Shared deci

Food allergies, particularly peanuts and tree nut allergies, can trigger severe anaphylaxis, prompting the development of novel immunotherapies. Conventional allergen immunotherapies, including therapies delivered by oral, sublingual and epicutaneous routes, show variable efficacy and are limited by adverse reactions and poor adherence. This review focuses on emerging advanced transdermal immunotherapeutic techniques, including the use of jet injectors, iontophoresis, sonophoresis and microneedles, which enable minimally invasive allergen delivery to dermal antigen-presenting cells and thus im

The therapeutic landscape of allergic diseases has been transformed by the advent of biologics targeting key immunologic pathways. These therapies offer disease-modifying potential across a spectrum of conditions including asthma, atopic dermatitis, eosinophilic esophagitis, chronic rhinosinusitis with nasal polyps, and food allergy. However, their high cost, limited long-term safety data in some populations, and evolving eligibility criteria raise critical questions about when biologics such as monoclonal antibodies are truly warranted, especially in the context of food allergy. Agents such a

PURPOSE OF REVIEW: Allergen immunotherapy (AIT) is increasingly popular as a treatment strategy for food allergy. Unfortunately, there is significant heterogeneity in reported outcomes, specifically in the dose-thresholds selected for evaluation and the symptoms used to define a "tolerated dose". These considerations are often investigator-driven and do not consider patient perspectives.
RECENT FINDINGS: A systematic review by the EAACI CO-FAITH taskforce recently flagged the need to better standardize and harmonize outcomes used in clinical trials of food-AIT. Using less objective symptoms t

INTRODUCTION: Peanut allergy is a common and potentially life-threatening condition affecting up to 2% of children in Western countries. Management traditionally relied on avoidance, but in 2020, the FDA-approved peanut oral immunotherapy (pOIT) to induce desensitization. This meta-analysis evaluated the efficacy and safety of pOIT versus placebo by incorporating newly published trials to inform clinical decision-making.
METHODS: This systematic review and meta-analysis followed PRISMA guidelines and Cochrane methodology, with registration number. Randomized controlled trials (RCTs) comparing

Venom immunotherapy (VIT) improves quality of life and is potentially lifesaving in patients with anaphylactic reactions to stinging insect venom and evidence of immunoglobulin E (IgE)-mediated sensitization. Sensitization is defined as either a positive skin test to insect venom or a serum venom-specific IgE level that is equal to or above 0.35 kU/L. With the advent of advanced diagnostics, levels of sensitization as low as 0.1 kU/L can be appreciated; however, the approach to patients with a low level of sensitization (defined as venom-specific IgE levels between 0.1 and 0.34 kU/L) remains d

Food allergy is a growing problem that can have a significant impact on both the individual, the family, and society. We are entering a new era of food allergy management with the recent US Food and Drug Administration approvals of 2 therapies for food allergy. IgE is now known to play a critical role in allergic diseases, including food allergy. Ant-IgE therapy has been under investigation for decades and is now approved for asthma, urticaria, nasal polyps, and most recently, IgE-mediated food allergy. Here, we evaluate what is known about the safety and efficacy of anti-IgE therapy as monoth
