Moving toward a standardized diagnostic statement of pituitary adenoma using an information extraction model: a real-world study based on electronic medical records.

PURPOSE: Diagnostic statements for pituitary adenomas (PAs) are complex and unstandardized. We aimed to determine the most commonly used elements contained in the statements and their combination patterns and variations in real-world clinical practice, with the ultimate goal of promoting standardized diagnostic recording and establishing an efficient element extraction process. METHODS: Patient medical records from 2012 to 2020 that included PA among the first three diagnoses were included. After manually labeling the elements in the diagnostic texts, we obtained element types and training se

The physiology of failure: Identifying risk factors for mortality in emergency general surgery patients using a regional health system integrated electronic medical record.

BACKGROUND: Emergency general surgery (EGS) patients have increased mortality risk compared with elective counterparts. Recent studies on risk factors have largely used national data sets limited to administrative data. Our aim was to examine risk factors in an integrated regional health system EGS database, including clinical and administrative data, hypothesizing that this novel process would identify clinical variables as important risk factors for mortality. METHODS: Our nine-hospital health system's billing data were queried for EGS International Classification of Disease codes between 2

Enhanced biogas production in anaerobic digestion of sludge medicated by biochar prepared from excess sludge: Role of persistent free radicals and electron mediators.

The addition of biochars to promote the efficiency of anaerobic digestion (AD) has widely received concerns. However, the role of persistent free radicals (PFRs) and the electron transfer ability of biochar in AD has not yet been noticed. In this study, biochars were prepared from excess sludge under 400 °C (B400) or 600 °C (B600) and different ratios of sludge to biochar (5:1, 10:1, 20:1) were applied in the AD of sludge. The results verified that PFRs in biochar participated in the sterilization of microorganisms in sludge, resulting in the release of organic matters. Similar to electron med

Characterization of Medication Trends for Chronic Kidney Disease: Mineral and Bone Disorder Treatment Using Electronic Health Record-Based Common Data Model.

Chronic kidney disease-mineral bone disorder (CKD-MBD) is the most common complication in CKD patients. Although there is a consensus on treatment guidelines for CKD-MBD, it remains uncertain whether these treatment recommendations reflect actual practice. Therefore, the aim of this study was to investigate the CKD-MBD medication trend in real-world practice. This was a retrospective and observational study using a 12-year period database transformed into a common data model from three tertiary university hospitals. Study populations were subjects initially diagnosed as CKD. The date of diagno

Surveillance of concussion-related injuries using electronic medical records from the Canadian Primary Care Sentinel Surveillance Network (CPCSSN): a proof-of-concept.

INTRODUCTION: Previous studies on traumatic brain injury trends in Canada have been restricted to hospitalization and emergency department visit data. However, many concussion patients may present first, or only, to family physicians. Therefore, the true burden of concussion in Canada is likely underestimated. The Canadian Primary Care Sentinel Surveillance Network (CPCSSN) collects information electronically from family physicians across Canada and can potentially be used for concussion surveillance. The objective of this study is to explore the feasibility of using data collated from CPCSSN

Calcium channel blockers as drug repurposing candidates for gestational diabetes: Mining large scale genomic and electronic health records data to repurpose medications.

New therapeutic approaches are needed for gestational diabetes mellitus (GDM), but must show safety and efficacy in a historically understudied population. We studied associations between electronic medical record (EMR) phenotypes and genetic variants to uncover drugs currently considered safe in pregnancy that could treat or prevent GDM. We identified 129 systemically active drugs considered safe in pregnancy targeting the proteins produced from 196 genes. We tested for associations between GDM and/or type 2 diabetes (DM2) and 306 SNPs in 130 genes represented on the Illumina Infinium Human E

Agreement between electronic medical records and self-reported urologic domains in the National Spina Bifida Patient Registry (NSBPR): Implications for future research.

BACKGROUND: Self-report (SR) is an efficient data collection method. However, SR data have been shown to be discrepant with medical record (MR) documentation, which raises questions about using SR to supplement retrospective chart review in research. In this study, pediatric spina bifida (SB) patients who completed SR interviews about continence status and personal bladder/bowel management were identified. We examined agreement between SR data and Urology provider notes in MRs. OBJECTIVE: This study aimed to (1) identify demographic, medical, or methodological factors that might contribute to

Electronic Health Record-Embedded Structural Interventions to Improve Sepsis Bundle Quality Measure Performance: A Quality Improvement Initiative at a Veterans Affairs Medical Center.

BACKGROUND: Though emphasized as a quality metric, sepsis quality measure (SEP-1) compliance remains low nationwide. Electronic health record (EHR)-based interventions show promise, but few have examined EHR laboratory reference value misconfiguration as a distinct driver of noncompliance. LOCAL PROBLEM: The San Francisco Veterans Affairs, a 110-bed tertiary care center, recorded 3 consecutive quarters of SEP-1 composite performance in the lowest national quintile, nadiring at 31.5%. METHODS: A multidisciplinary team applied LEAN-A3 methodology to identify root causes of low SEP-1 performanc

Artificial intelligence based multimodal fusion of electronic medical record data and ECG signals for pericarditis prediction: pericarditis calculator (p-Cal) as a screening aid.

BACKGROUND: Pericarditis may be associated with significant morbidity and mortality and can be a clinically challenging entity to diagnose. There is a clinical need for automated artificial intelligence (AI) algorithms to act as a screening aid to assist with the early and accurate detection of pericarditis. METHODS: We developed p-Cal, a browser-based pericarditis risk calculator that fuses the patient clinical data and 12-lead ECG image to estimate risk for pericarditis. The ECG encoder was designed based on a pretrained MedCLIP Vision Transformer and random forest classifier was used for t

Rural variations in primary care prostate cancer diagnosis and survival: a cohort study using linked Australian primary care electronic medical record data.

BACKGROUND: Across the state of Victoria, Australia around one-third of residents live in rural areas. There are persisting disparities and poorer prostate cancer outcomes in rural areas compared with metropolitan areas. Prostate Specific Antigen (PSA) blood tests are commonly used in primary care to investigate patients with suspected prostate cancer. Understanding variations in PSA test use and follow-up of abnormal results between rural and metropolitan areas could identify potential causes of disparities in prostate cancer detection and outcomes. METHODS: We conducted a longitudinal cohort

Adherence of studies involving artificial intelligence in the analysis of ophthalmology electronic medical records to AI-specific items from the CONSORT-AI guideline: a systematic review.

PURPOSE: In the context of ophthalmologic practice, there has been a rapid increase in the amount of data collected using electronic health records (EHR). Artificial intelligence (AI) offers a promising means of centralizing data collection and analysis, but to date, most AI algorithms have only been applied to analyzing image data in ophthalmologic practice. In this review we aimed to characterize the use of AI in the analysis of EHR, and to critically appraise the adherence of each included study to the CONSORT-AI reporting guideline. METHODS: A comprehensive search of three relevant databa
