Ventilator modes: old and new.

CPPV is an accepted standard of treatment in patients with acute respiratory failure. Because of rapid technologic advancements, new modes of ventilation have been developed. Unfortunately, this occurs at a time when there are still many controversies about the relative benefits and modes of action of CPPV. Before the proliferation of use of these newer modes in the intensive care units, documentation of the beneficial effects and safety of these modalities under controlled condition is of utmost importance. Although some of the "new" ventilatory modes (PSV, PAV) improve patient comfort and sy

One-Lung Jet Ventilation for Robotic-Assisted Tracheal Resection in Close Proximity to the Carina: A Case Report.

This case report describes a novel ventilation method to safely accommodate robotic-assisted tracheobronchial surgery. The patient with a tracheal tumor 2.5 cm above the carina underwent a robotic-assisted tracheal resection. Single-lung jet ventilation was performed using the Monsoon Jet Ventilator during the tracheal resection and anastomosis. This allowed the surgeon to operate without interruption by eliminating an intermittent apneic technique. Through a small chest incision, the surgeon robotically advanced a Laserjet 40 tube into the transected end of the distal trachea. The endotrachea

Using airway resistance measurement to determine when to switch ventilator modes in congenital diaphragmatic hernia: a case report.

BACKGROUND: Congenital diaphragmatic hernia is a deficiency of the fetal diaphragm resulting in herniation of the abdominal viscera into the thoracic cavity. The best method of respiratory management of congenital diaphragmatic hernia is unclear, but high frequency oscillatory ventilation is often used as the initial ventilator mode for severe congenital diaphragmatic hernia. When it becomes impossible to maintain the pre-ductal saturations, the timing of successful switching of the ventilation mode from high frequency oscillatory ventilation to conventional mechanical ventilation remains uncl

The Tritube: Facilitating Upper Airway Surgery With an Ultrathin Cuffed Airway Device.

The Tritube is a narrow-bore cuffed tracheal tube (outer diameter 4.4 mm and inner diameter ~2.4 mm) that permits effective alveolar gas exchange using flow-controlled ventilation. Constant gas flow delivers physiological minute volumes, within preset pressure limits, and applies suction to the airway during expiration. The technique has attracted interest for laryngotracheal microsurgery as it provides superior surgical exposure and avoids many of the complications associated with high-frequency jet ventilation. Cuff inflation protects the lower airway and produces a motionless operating fiel

Respiratory function testing for guiding ventilator mode conversion in congenital diaphragmatic hernia.

INTRODUCTION: For patients with a congenital diaphragmatic hernia, conventional mechanical ventilation (CMV) and high-frequency oscillatory ventilation (HFOV) are used in initial ventilatory management. HFOV has recently been recommended as a rescue therapy; however, we use HFOV for initial ventilation management, with a preoperative challenge test for CMV conversion and respiratory function testing at the time of CMV conversion. We aimed to compare patient characteristics between CMV conversion- and HFOV-preferred treatment groups. METHODS: Ventilator settings and blood gases were retrospect

[Cardiovascular and hemodynamic changes after artificial pulmonary ventilation].

The cardio-respiratory interactions include mechanical, reflex and humoral mechanisms. However, in the organism they mutually overlap, thus mating their separate investigation is problematic. Mechanical effects of conventional artificial ventilation (AV) and high frequency ventilation (HFV) are elicited by increased intrapulmonary pressure during lung inflation, as well as during application of positive end expiratory pressure (PEEP). The increase in intrapulmonary pressure compresses the pulmonary vessels and increases the pulmonary vascular resistance. These changes deteriorate the right ven

Successful removal of a large intratracheal tumor using the injection-time-controllable manual jet ventilator via translaryngeal approach -a case report.

BACKGROUND: Removal of intratracheal tumors is challenging due to the difficulty in securing a patent airway before surgery. We report a case of successful removal using jet ventilation with an injection-time-controllable manual jet ventilator. CASES: A 3.3 cm-long intratracheal mass was located 5 cm below the vocal cords and obstructing 70%-80% of the trachea. Following induction, a rigid telescope under suspension laryngoscopy was used to guide the careful insertion of a hard and long catheter (inner diameter: 1.8 mm; outer diameter: 3 mm; length: 50 cm) beyond the tumor, enabling jet venti

Oscillation transmission and volume delivery during face mask-delivered HFOV in infants: Bench and in vivo study.

OBJECTIVE: Noninvasive high frequency oscillatory ventilation (NHFOV) has not been studied beyond neonatal age and with interfaces other than nasal prongs. We set up a preliminary study to investigate feasibility, oscillation transmission, and volume delivery of face mask-delivered NHFOV in a bench model mimicking a normal 1-year infant without any lung disease and then in vivo in a series of infants with same characteristics. DESIGN: A mannequin with upper airways was connected to an electronic active lung simulator ventilated through NHFOV with varying parameters. Volume delivered by oscill

Efficacy and safety of supraglottic jet oxygenation and ventilation to minimize sedation-related hypoxemia: a meta-analysis with GRADE approach.

INTRODUCTION: Hypoxemia is a common complication of sedation. This meta-analysis aimed to evaluate the efficacy and safety of supraglottic jet oxygenation and ventilation (SJOV) in preventing hypoxemia during sedative procedures. METHODS: Randomized controlled trials (RCTs) that compared SJOV with conventional oxygen therapy in sedated patients were searched in five databases (MEDLINE, EMBASE, Cochrane Library, China National Knowledge Infrastructure [CNKI], and Google Scholar) from their inception to March 2024. The primary outcome was the proportion of patients who developed hypoxia (SpO2 <
