Pseudohypoparathyroidism (PHP) is a condition characterized by hypocalcemia, hyperphosphatemia, and an impaired phosphaturic response to exogenous parathormone (PTH). A minority of patients with PHP have associated bone disease, and in some the radiological appearances have been suggestive of rickets. Tertiary hyperparathyroidism (THP) is a rare complication in patients with hypophosphataemic rickets (HR), usually related to long-term management with active vitamin D analogues and oral phosphate salts. OBJECTIVE: To clarify the prevalence and etiology of ectopic calcification in patients with systemic lupus erythematosus (SLE), especially under the active vitamin D3 administrations and to reveal the risk factors of ectopic calcification.
METHODS: Sixty patients with SLE, excluding the patients on dialysis were studied. We examined radiographs of hands, forearms, upper and lower extremities, and pelvises of all patients to evaluate ectopic calcification.

In 19 chronically hemodialysed patients (11M + 8F, mean age 52 +/- 15 yrs) with secondary hyperparathyroidism (sHP), selected markers of bone turnover in the course of pulsative treatment with alphacalcidol were evaluated. Vitamin D deficiency is common in the elderly, especially in countries where effective sunlight or exposure to sunlight is limited. Two regimes for vitamin D supplementation--low-dose daily oral administration and intermittent high-dose administration--were examined with regard to safety and effectiveness. Eleven papers reporting studies in 449 elderly subjects were reviewed. The basal values of plasma vitamin D metabolites were evaluated in patients with primary hyperparathyroidism (1 degree HPT, n = 31), hypoparathyroidism (HP, n = 7), pseudohypoparathyroidism (PHP, n = 4) and normal controls (n = 21). Radiologic bone lesions in 20 long-term hemodialyzed patients with chronic renal failure are described. During chronic dialysis the bone lesions increased. Treatment of 8 patients with 1 alpha-OH-D3 for 6 months prevented progression of the bone lesions or improved them.

Six women (mean age, 62 yr; range, 48--77 yr) who were considered to have postmenopausal osteoporosis, as demonstrated by radiological evidence of vertebral crush fractures, low intestinal calcium (Ca) absorption, and bone biopsies consistent with this diagnosis, received a pharmacological dose of 25-hydroxyvitamin D3 (25OHD3; 20 microgram/day) for 3 months. Dietary intake, sunlight exposure and 25-hydroxy-vitamin D levels were estimated in 62 patients admitted to a geriatric assessment unit. There was a significant correlation between vitamin D intake and 25-hydroxy-vitamin D levels but none between sunlight exposure and these. This suggests that in old age dietary deficiency may be a more important cause of metabolic bone disease than limited sunlight exposure.
