Lower gastrointestinal tract dysfunction in patients receiving long-term hemodialysis.

Severe bowel dysfunction developed in 25 of 945 patients receiving long-term hemodialysis during a ten-year period. Colonic perforation occurred in 12 patients, six of whom died due to peritonitis. In seven instances, the perforation occurred spontaneously. Ten other individuals exhibited prolonged, severe adynamic ileus that progressed to colonic pseudo-obstruction in eight patients. Medical decompression (eight patients) and surgical bowel decompression (two patients) resulted in recovery in nine. Aluminum hydroxide gel, which was taken regularly by all patients, was associated with notable

[Histopathological diagnosis of intestinal motility disorders: Hirschsprung disease and differential diagnoses].

Hirschsprung disease/aganglionosis of the colon is the most common form of intestinal motility disorder, especially in children. In this review article, which was presented at the specialist meeting of the German Paediatric Pathology Working Group, insight into the histopathological diagnosis of Hirschsprung's disease, including the technical approach to biopsies, will be provided. In addition, differential diagnoses of chronic intestinal pseudo-obstruction (CIPO), which can be identified with the help of histopathological diagnostics, are presented. These include-among others-hypoganglionosis…

Diabetic Ogilvie's Syndrome Mimicking Colonic Volvulus: A Case Report.

Ogilvie's syndrome, also known as acute colonic pseudo-obstruction, is a rare disorder marked by colonic dilation without any anatomical obstruction in the intestinal lumen. The dilation occurs typically in the cecum and right colon, with sometimes extension to the sigmoid colon, which can mimic a colonic volvulus, especially in geriatric patients. The involved mechanism in Ogilvie's syndrome is an intestinal obstruction due to an alteration in the propulsion of the gastrointestinal tract content. The diagnosis of Ogilvie's syndrome should be made only after exclusion of a specific mechanical

Multiple endocrine neoplasia 2B presenting with pseudo-Hirschsprung's disease.

Multiple endocrine neoplasia type 2B (MEN 2B) is a rare syndrome characterized by medullary thyroid carcinoma (MTC), pheochromocytoma and typical phenotypic features, such as marfanoid habitus, multiple mucosal ganglioneuromas and thickened corneal nerves. Individuals with MEN 2B may develop megacolon and pseudo-obstruction due to intestinal ganglioneuromatosis simulating Hirschsprung's (HSCR) disease. We hereby describe the clinical and genetic features of a 21-year-old male patient with MEN 2B associated with pseudo-HSCR disease. The patient had MTC, pheochromocytoma, marfanoid habitus, mult

A case of delayed oxaliplatin-induced pseudo-obstruction: an atypical presentation of oxaliplatin neurotoxicity.

Chemotherapy-induced neurotoxicity is a serious complication of cancer treatment. Oxaliplatin, a third-generation platinum drug, has become one of the first-line therapies used in the treatment of metastatic colorectal cancer. Peripheral neuropathy is a common complication of platinum-based chemotherapy. Most commonly a sensory neuropathy occurs with cold-triggered symptoms in the acute phase and numbness and painful paresthesias as a late presentation. Autonomic neurotoxicity and late presentation, months after cessation of the therapy, has rarely been described. We report a patient who clini

Intestinal neuronal dysplasia (type B) causing fatal small bowel ischaemia in an adult: a case report.

Intestinal neuronal dysplasia (IND) is a well-documented but controversial entity defined as a congenital malformation of gastrointestinal innervation caused by dysplastic embryonal development of the enteric nervous system. Mainly described as a colonic disorder mimicking Hirschsprung's disease in the paediatric population, involvement of the ileum is rare. We report a case of fatal IND of the small intestine in a 58-year-old man presenting with a recent onset of features of small intestinal obstruction. IND has been described as a cause of colonic pseudo-obstruction and chronic constipation

Colonic volvulus in children with intestinal motility disorders.

BACKGROUND: Chronic intestinal pseudo-obstruction (CIP) is a condition characterized by symptoms of bowel obstruction in the absence of an anatomical cause. Patients with CIP and chronic intractable constipation (CIC) can also develop anatomical obstruction, and the presenting symptoms mimic those of underlying pseudo-obstruction. OBJECTIVES: Our objectives were to evaluate the incidence, clinical presentation, and diagnostic investigations of colonic volvulus in children with intestinal motility disorders and to differentiate these episodes of colonic volvulus from the underlying motility di…

Phenotypic expansion of visceral myopathy associated with ACTG2 tandem base substitution.

Familial visceral myopathy (FVM) is a rare heritable and heterogeneous condition due to impaired smooth muscle function. We identified a family segregating 11 individuals with a spectrum of visceral symptoms involving the small intestine, colon, biliary tract, urinary tract and uterus. Whole-exome sequencing revealed a novel heterozygous tandem base substitution c.806_807delinsAA (p.(Gly269Glu)) in ACTG2, encoding smooth muscle actin γ-2, in affected family members. Variants in ACTG2 were recently identified in FVM with intestinal pseudo-obstruction as well as with the congenital megacystics-m…

Intestinal ganglioneuromatosis and multiple endocrine neoplasia type 2B: implications for treatment.

Three infants, who presented with intestinal obstruction due to diffuse transmural intestinal ganglioneuromatosis, are described. Mutation analysis of exon 16 of the RET proto-oncogene revealed germline M918T and thus, a molecular diagnosis of multiple endocrine neoplasia type 2B (MEN 2B). Two infants developed medullary carcinoma of the thyroid. The third had a prophylactic thyroidectomy despite no obvious thyroid masses and normal calcitonin concentrations, but microscopic multifocal medullary carcinoma was found on histological examination. Early recognition of intestinal ganglioneuromatosi

The use of computed tomography in the detection and characterisation of large bowel obstruction.

Computed tomography (CT) has become the imaging modality of choice for the investigation of patients with clinico-radiological suspicion of obstruction. Appropriate CT technique is discussed, with recommendations on how to improve diagnostic confidence and to reduce radiation dose. The performance of CT in the investigation of LBO is compared to that of the plain abdominal radiograph and contrast enema. CT is unrivalled in its ability to detect and characterise the cause of large bowel obstruction (LBO), identify complications, and guide appropriate treatment.

[Gastrointestinal complications of acute pancreatitis (author's transl)].

Gastrointestinal complications, such as ileus, bleeding, stenosis and fistula formation, were retrospectively analysed on 180 patients with acute pancreatitis. Paralytic ileus occurred in only a quarter of patients with acute oedematous pancreatitis and only one had bleeding from a gastric ulcer. Complications occurred in the early but also postacute stage in patients with the haemorrhagic-necrotizing form. Even with early and delayed operation and adequate treatment of most complications, renewed gastrointestinal complications were not uncommon and required re-operations.

[Conservative and surgical ileus treatment].

The diagnosis ileus is one of the most common indications for an emergency laparotomy. In 70% of the cases, the small intestine is affected, and in 30% it is the colorectum. While stuck hernias are a major cause in developing countries, the most common causes in western countries are postoperative adhesions that lead to an acute bowl obstruction. The timeframe for treatment of a complete mechanical obstruction is short as acute ischemia can lead to necrosis with bowl perforation within 6 h. The perioperative lethality for an emergency laparotomy due to an ileus ranges from 5-15%. In addition t

Pseudo-obstruction in children.

Pseudo-obstruction of the gastrointestinal tract is a rare disorder of impaired gastrointestinal motility. The more common symptoms of pseudo-obstruction in the infant or child include dysphagia, nausea, vomiting, abdominal distention, abdominal pain, and constipation. The majority of children have symptoms within the first year of life. Chronic cases of pseudo-obstruction are associated with neuropathic or myopathic changes in other parts of the body. Bladder dysfunction and neurological problems have been reported. The diagnosis of pseudo-obstruction is based on history, physical examination

[Involvement of the small intestine in systemic scleroderma].

INTRODUCTION: Though impairment of the gastrointestinal tract is commonly encountered in patients with systemic sclerosis, the most frequent abnormalities are esophageal and anorectal disorders. Involvement of the small intestine is also common, reaching a 40-80% prevalence. It often leads to life-threatening complications. CURRENT KNOWLEDGE AND KEY POINTS: The occurrence of small intestine impairment and its potential relationships with other organ impairment is still unknown. However, it rarely indicates the existence of the disease (10%) which remains asymptomatic for a long period. As cli

Phenotypic overlap masking MELAS in Turner syndrome: a diagnostic challenge.

BACKGROUND: Mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) is a multisystemic disorder that can present with diverse clinical features, including cardiomyopathy and chronic intestinal pseudo-obstruction. However, diagnosing MELAS can be challenging when its manifestations overlap with other genetic syndromes, leading to diagnostic anchoring and potential mismanagement. CASE SUMMARY: A 51-year-old woman with mosaic Turner syndrome presented with concentric left ventricular hypertrophy and mildly reduced systolic function (left ventricular ejection fraction 4…
