[METANEPHRIC ADENOMA DIAGNOSED PREOPERATIVELY AS RENAL CELL CARCINOMA: A REPORT OF TWO CASES].

Metanephric adenoma is an extremely rare benign tumor. We report two cases of metanephric adenoma that were diagnosed preoperatively as renal cell carcinoma (RCC).Case 1 was a right renal tumor found by ultrasonography in a 57-year old woman who presented for a medical examination. Abdominal CT revealed a 26-mm mass that was enhanced weakly in the early phase and enhanced strongly in the late phase, in the right kidney. Based on a clinical diagnosis of RCC (cT1aN0M0), laparoscopic partial nephrectomy was performed. Case 2 was a left renal tumor incidentally found during an annual examination o

Value of PAX2 immunostaining in tumor diagnosis: a review and update.

PAX2 is a member of the PAX family of transcription factors that, together with PAX8, is involved in the regulation of the organogenesis of the kidney and the Müllerian system. Recent investigations have demonstrated that, among tumors, PAX2 is commonly expressed in epithelial tumors of the kidney and female genital tract. Although PAX2 expression has also been reported in B-cell lymphomas and rhabdomyosarcomas, especially alveolar rhabdomyosarcomas, it has been suggested that the positivity in these tumors was most probably due to a cross-reactivity of the anti-PAX2 antibody used in those inv

Computerized tomography in the diagnosis of transitional cell carcinoma of the kidney.

We present 8 patients with transitional cell carcinoma of the kidney who underwent preoperative computerized tomographic examination of the lesions. The computerized tomographic findings are discussed and are correlated with the conventional radiographic appearance. The tumor presents on computerized tomographic examination as a solid mass in the renal pelvis and/or in a ballooned calix, with displacement and compression of the renal sinus fat. It shows attenuation values of 8 to 20H and enhances minimally after intravenous injection of iodinated contrast material. Trapping of contrast materia

Incidental capture of rarely diagnosed pediatric tumor: an infant boy with clear cell sarcoma of the kidney.

Clear cell sarcoma of the kidney (CCSK) is an uncommon neoplasm that accounts for less than 5% of all pediatric renal tumors. Compared with Wilms' tumor, CCSK has a higher rate of relapse, greater propensity for bone metastasis, and poorer overall survival. We present the case of a 19-month-old boy with a large renal mass diagnosed incidentally by ultrasonography during surveillance for vesicoureteral reflux. This report describes the rare occurrence of an incidental radiologic capture of CCSK and provides a brief review of disease pathology.

Epithelial-myoepithelial parotid carcinoma after kidney transplantation.

The occurrence of a second malignant neoplasm (SMN) in patients who have been submitted to kidney transplantation is increasing and causes concern; parotid carcinoma is rarely reported after transplantation and may be related to long-term chemotherapy.Salivary gland carcinomas displaying exclusively myoepithelial differentiation-myoepithelial carcinomas (EMC) are rare, being less than 1% of all salivary gland tumours. EMC arises most commonly in the parotid gland and usually occurs in women. Their histopathologic features, immunohistochemical profile and clinical behaviour remain controversial

Epithelioid angiomyolipoma of the kidney: a case report.

Renal angiomyolipoma is a distinctive benign neoplasm that occurs either sporadically or in patients with tuberous sclerosis complex. A 45-year-old woman was admitted with history offlank pain and vomiting. There were no signs suggestive of tuberous sclerosis either in the patient or her family. At operation, she had a left renal mass with nephrolithiasis and hydronephrosis. Histopathology revealed epithelioid angiomyolipoma of the left kidney with chronic pyelonephritis. Immunohistochemistry confirmed the diagnosis of angiomyolipoma. This case is presented to highlight the epithelioid variant

A case of metastatic rectal adenocarcinoma diagnosed by thyroid cytology.

Fine-needle aspiration (FNA) cytology of the thyroid gland has been widely used for the evaluation of thyroid nodules. Most of the nodules are primary thyroid lesions. However, up to 4% of thyroid FNA may harbor a metastatic neoplasm. The metastases are most commonly from lung, kidney, breast, and skin. Metastatic colorectal cancers are also seen in thyroid but less common. Here we report the cytologic features, differential diagnosis and clinical implications of a metastatic rectal adenocarcinoma to the thyroid gland diagnosed by FNA.

Transitional cell carcinoma of the kidney.

This paper presents 14 personal patients with transitional cell carcinoma of the kidney who were managed over a period of 13 years. Emphasis is placed on the diagnostic features, the great variety of radiological appearances of this neoplasm and the value of preoperative cytological examination of urine. In three patients the tumour started in the mucosa of peripheral calyces, and did not involve the surgical renal pelvis; this added to the diagnostic difficulty, and led to delay in diagnosis and treatment in one patient. The possibility that this tumour may be more common in Australia because

Percutaneous nephrostomy: experience in 107 kidneys.

We were successful using percutaneous nephrostomy to relieve upper urinary obstruction in 98 of 107 kidneys. In two thirds of our patients obstruction was due to neoplasm, with prostatic cancer the leading cause. Percutaneous nephrostomy proved to be ideally suited for drainage of pyonephrosis and in azotemic patients. Its use can obviate surgical nephrostomy and prolonged ureteral catheter drainage. We also are encouraged by our early experience with extensions of percutaneous nephrostomy, including percutaneous ureteral stenting, renal stone dissolution, and percutaneous pyelolithotomy.

Computed tomography of the body: initial clinical trial with the EMI prototype.

A review of the first 400 cases examined on one of three prototype EMI body scanners is presented. During the initial evaluation phases, it became obvious that computed tomography of the body has tremendous potential in the evaluation of various organs and disease states. The technique has been most useful in evaluating patients with known or suspected abdominal neoplasm, particularly lesions of liver, pancreas, retroperitoneum, and kidney. Too few cases have been studied to realistically quantitate its impact on the total field of medical diagnostic imaging.

Renal disease in adults with TSC2/PKD1 contiguous gene syndrome.

The most common renal lesions of tuberous sclerosis complex, an autosomal-dominant syndrome resulting from losses of TSC1 (9q34) or TSC2 (16p13.3), are renal cysts and angiomyolipomas. Epithelial neoplasms are less common. The TSC2 gene lies adjacent to PKD1, the major gene responsible for autosomal-dominant polycystic kidney disease. Recently, a deletion mutation disrupting both TSC2 and PKD1 has been described in young children with tuberous sclerosis complex with severe renal cystic disease. This disease has been termed the TSC2/PKD1 contiguous gene syndrome. We describe the lesions in the

[ADPKD and IPMN: Mere Coincidence or Double Trouble?].

This article constitutes a review of the existing literature on the potential correlation between autosomal dominant polycystic kidney disease (ADPKD) and intraductal papillary mucinous neoplasms (IPMN) of the pancreas. Additionally, it presents a clinical case where familiarity for both pathologies was observed, derived from the direct experience of our clinic, reinforcing the hypothesis of a possible common pathogenetic pathway. The review focuses on the potential genetic correlation between these two pathologies within the realm of ciliopathies, emphasizing the importance of targeted screen

[Acute tubule-interstitial nephritis].

Acute tubule-interstitial nephritis is a heterogeneous group of inflammatory diseases which affect renal parenchyma and tubules, mostly as a result of the immune-mediated injury. They are the important cause of acute kidney injury accounting for 5-15% of all its causes. In subsequent years, with the development of pharmacotherapy, the incidence of drug-induced nephrotoxicity has been increasing. The other causes of the acute tubule-interstitial nephritis are autoimmune and inflammatory diseases, infections, neoplasms and electrolyte abnormalities. The diagnostics are complex and treatment of t

Xanthogranulomatous inflammation of the peripancreatic region mimicking pancreatic cystic neoplasm.

Xanthogranulomatous inflammation (XGI) is histopathologically characterized by a marked proliferative fibrosis, parenchymal destruction, and infiltration of foamy histiocytes intermixed with other inflammatory cells. Herein, we report a case of a 73-year-old man without symptoms who was initially diagnosed with a pancreatic cystic tumor but later with XGI in the peripancreatic region. Although XGI has been reported to occur in various organs or tissues, such as the gallbladder, kidney, bone, stomach, colon, appendix, lymph nodes, and soft tissues, XGI involving the pancreas or its surrounding

Metastatic angiosarcoma of the kidney: a case report with treatment approach and review of the literature.

Angiosarcomas are rare soft tissue malignancies. Typically they originate from the skin of the scalp or face, whereas visceral sarcomas are very rare. We report the case of a 67-year-old man affected by a large angiosarcoma of the kidney. After surgical removal, a rapid peritoneal, visceral and cutaneous diffusion developed. Palliative chemotherapy, based on anthracycline and ifosfamide, which are normally used to treat all other high-grade spindle cell sarcomas, was totally inactive. On the basis of these results and of the biological characteristics of these rare neoplasms it is mandatory to

Intermittent small bowel obstruction by jejunal enteroliths in a patient with a Crohn's disease stricture.

Small bowel obstruction is most frequently due to postoperative or inflammatory adhesions, intestinal neoplasms, hernias, or bezoars. Intermittent small bowel obstruction may be secondary to a Crohn's disease stricture or to chronic adhesive peritonitis. Enterolithiasis, usually associated with jejunal diverticulosis or with a Meckel diverticulum, should be considered in patients who have not previously undergone abdominal surgical procedures. X-ray evidence of stones in the abdominal field, outside the common sites, i.e. gallbladder, kidney, bladder, should suggest a diagnosis of enterolithia

Immunohistochemical evaluation of a complex endocrinopathy.

A patient with clinical and biochemical evidence of ectopic ACTH production is described. Autopsy revealed endocrine tumors of bronchus and pancreas with metastases to the liver, kidney, abdominal lymph nodes, and bone. Immunohistochemical methods established that the bronchial and pancreatic tumors were separate primary neoplasms each with its own hormone production; and allowed us to determine that the bronchial carcinoid was the source of ACTH and gave rise to the liver metastases, and that insulin was present in the cells of the pancreatic islet cell tumor. These methods also revealed a mi

Pineal teratoma with nephroblastic component in a newborn male: Case report and review of the literature.

Neonatal germ cell tumors are rare and comprise both benign and malignant neoplasms. Teratoma with nephroblastoma is a malignant subset defined pathologically by the presence of nephroblastoma and teratoma elements. Although teratoma with nephroblastoma is most often found in the kidney, 24 of 59 reported cases are associated with extrarenal locations, such as the mediastinum or retroperitoneum. To our knowledge, this is the first patient in the literature with intracranial/pineal teratoma with nephroblastoma, which was managed with staged transcranial approaches resulting in gross total resec

A radiological study of urinary tract disease in Port Moresby 1972-74.

During a 21/2 year period at Port Moresby, 852 patients had intravenous pyelograms. Two thirds of the patients were males and over half were Papua New Guineans. 34% of the patients had detectable urinary tract abnormalities of whom 38% had urinary calculi. Renal and ureteric calculi were particularly common in expatriate males, being found twice as commonly as in Papua New Guinean males. Although there were some Papua New Guinean male children with bladder calculi, the frequency appeared less than that found in some other developing countries. Other common findings included congenital urinary

Wunderlich Syndrome in a Patient With Coexisting Giant Angiomyolipoma and Multicystic Clear Cell Renal Cell Carcinoma: A Case Report.

Angiomyolipoma (AML) is a benign mesenchymal renal neoplasm composed of dysmorphic blood vessels, smooth muscle cells, and adipose tissue. Although most AMLs are asymptomatic and incidentally detected, large tumors may present with spontaneous hemorrhage resulting in Wunderlich syndrome (WS), a rare urological emergency characterized by spontaneous nontraumatic renal bleeding. Renal cell carcinoma (RCC), on the other hand, is the most common malignant epithelial tumor of the kidney. The synchronous occurrence of AML and RCC within the same kidney is rare. We report the case of a 64-year-old wo
