An initial experience with 85 consecutive robotic-assisted rectal dissections: improved operating times and lower costs with experience.

BACKGROUND: Data are limited about the robotic platform in rectal dissections, and its use may be perceived as prohibitively expensive or difficult to learn. We report our experience with the initial robotic-assisted rectal dissections performed by a single surgeon, assessing learning curve and cost. METHODS: Following IRB approval, a retrospective chart review was conducted of the first 85 robotic-assisted rectal dissections performed by a single surgeon between 9/1/2010 and 12/31/2012. Patient demographic, clinicopathologic, procedure, and outcome data were gathered. Cost data were obtained

High-volume surgeons in regard to reductions in operating time, blood loss, and postoperative complications for total hip arthroplasty.

BACKGROUND: The purpose of this study was to clarify the volume-outcome relations for total hip arthroplasty, with a particular focus on outcome variables such as operating time, blood loss, and postoperative complications. METHODS: Using an Internet-based questionnaire survey, we identified 2965 total hip arthroplasties performed for osteoarthritis, idiopathic osteonecrosis of the femoral head, rheumatoid arthritis, or hip fractures in 289 hospitals in Japan during the period from November 1, 2006, to March 31, 2007. We examined the hospital and surgeon volumes, patient characteristics (age,

Endovascular treatment for acute traumatic transection of the descending aorta: focus on operative timing and left subclavian artery management.

OBJECTIVE: The operative timing and management of acute traumatic aortic rupture are matters of debate. We reviewed our experience with endovascular repair of acute traumatic aortic rupture, focusing on these topics. METHODS: From 1998 to 2007, 31 patients were referred to our institute for acute traumatic rupture of the descending aorta. In 11 patients (group I) an early stent graft procedure was performed, whereas in 16 patients (group II) endovascular repair was delayed. The median time from trauma was 24 hours in group I and 1.5 months in group II. Eight (25.8%) patients had a short proxi

Clinically relevant tracheomalacia after repair of esophageal atresia: the role of minimal intra-operative dissection and timing for aortopexy.

BACKGROUND: Tracheomalacia is associated with esophageal atresia (EA), but may go unnoticed from external splinting forces. Intra-operative dissection with fistula division releases external splinting, revealing tracheomalacia only post-operatively. Analysis of surgical technique may disclose an iatrogenic etiology. METHODS: From 1995 - 2004, 44 neonates underwent surgery for EA. All patients underwent pre-, intra- and postoperative bronchoscopy. Operative and bronchoscopic notes were studied for malacia, and extensive dissection of the esophagus and fistula from the trachea. RESULTS: Surgic

Transferring Surgical Expertise: Analyzing the Learning Curve of Robotic Cardiac Surgery Operative Time Reduction When Surgeon Moves from One Experienced Center to Another.

BACKGROUND: Robotically assisted cardiac surgery is performed in a team setting and is well known to be associated with learning curves. Surgeon and operative team learning curves are distinct entities, with total operative time representing the entire operative team (surgery, anesthesia, nursing, and perfusion) and cross-clamp time representing mainly the surgical team. Little is known about how a team learning curve evolves when an experienced surgeon transitions from one surgical center to another. This study investigates the dynamics of the team learning curve expressed as total operative …

Modified biplanar (0-90°) endoscopic-guided puncture technique for percutaneous nephrolithtomy: refinement with endoscopic combined intrarrenal surgery to reduce fluoroscopy and operative time.

INTRODUCTION: We aim to publish our innovative modified biplanar 0-90 endoscopic guided puncture technique for percutaneous nephrolithotomy in supine recorded with a GoPro® camera for standardization of the technique. It solves drawbacks of the fluoroscopic technique, i.e., in kidneys with complex anatomy, it may be challenging to distinguish calyces as they are often superposed, and it does not allow for all benefits of a combined endoscopic approach (1, 2). Our technique shortens puncture and fluoroscopic time and is easy to teach and reproduce. METHODS: A 77-year-old female patient had pre

Prospective single-center paradigm shift of surgical stabilization of rib fractures with decreased length of stay and operative time with an intrathoracic approach.

INTRODUCTION: Intrathoracic surgical stabilization of rib fractures allows for a novel approach to rib fracture repair. This approach can he … compared with ITP. However, there was no difference in median morphine equivalents between cohorts. Operative time was shorter for ITP with ETP (279 minutes vs. 188 minutes, p < 0.001) after adjusting for numbers of ribs fixed. CONCLUSION: In this single-center study, patients who underwent ITP had a decreased LOS and operative time in comparison to ETP in pati …

After Posterior Fusions for Adult Spinal Deformity, Operative Time is More Predictive of Perioperative Morbidity, Rather Than Surgical Invasiveness: A Need for Speed?

STUDY DESIGN: A retrospective cohort study. OBJECTIVE: The aim of this study was to determine the independent effects of operative time and surgical invasiveness on perioperative outcomes after posterior spinal fusions for adult spinal deformity. SUMMARY OF BACKGROUND DATA: Morbidity is high after posterior fusions for adult spinal deformity. Although previous reports have demonstrated an association between perioperative outcomes and the extent of correction and fusion (number of posterior levels fused, pelvic fixation, combined anterior-posterior fusion), no study has looked at the indepen…

Evaluation of Pre-Operative Cartilage and Meniscus Ultrashort Echo Time T2* as Biomarkers of Long-Term Clinical Outcomes Following Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Ultrashort echo time T2* (UTE-T2*) mapping is sensitive to the composition of deep cartilage and menisci. UTE-T2* elevations are common after anterior cruciate ligament (ACL) injuries, but their long-term prognostic importance is unknown. HYPOTHESIS: Higher pre-operative UTE-T2* values will associate with greater symptoms and radiographic knee osteoarthritis (KOA) at long-term follow-up. STUDY TYPE: Retrospective longitudinal cohort study using prospectively acquired baseline MRI and follow-up clinical outcomes. POPULATION: Twenty-two participants (13 female, mean [SD] age: 29 [

Preoperative CT-Based Pelvic Sarcopenia and Subcutaneous Adiposity Are Associated with Anaemia and Operative Time in Acetabular Fracture Surgery: A Retrospective Cohort Study.

Background and Objectives: Acetabular fracture surgery is associated with substantial perioperative blood loss and prolonged operative time. Routine preoperative pelvic computed tomography (CT) carries information about body composition that is not currently exploited for risk stratification. We tested whether (i) CT-defined pelvic sarcopenia is associated with lower preoperative haemoglobin and (ii) preoperative subcutaneous fat cross-sectional area (CSA) is independently associated with operative time, after adjustment for surgical approach, age, fracture complexity and sarcopenia status. Ma…
