Granulomatous Tubercular Hepatitis Presenting as Secondary Hemophagocytic Lymphohistiocytosis: A Case Report and Systematic Review of the Literature.

Hemophagocytic lymphohistiocytosis is a life-threatening disorder characterized by persistent pathologic activation of cytotoxic T lymphocytes, natural killer cells, and macrophages. We present details of a young patient who presented with high-grade fever, jaundice, and breathlessness. On investigations, he had hepatitis, anemia, neutropenia, and coagulopathy. He also had hypertriglyceridemia, hypofibrinogenemia, and hyperferritinemia. Bone marrow aspiration revealed histiocytosis, and transjugular liver biopsy revealed necrotizing granulomas positive for Mycobacterium tuberculosis on acid-fa

Infectious diseases detected by screening after arrival to Denmark in internationally adopted children.

AIM: To show the prevalence of selected infectious diseases among internationally adopted children (IAC) in Denmark. BACKGROUND: Each year approximately 200 IAC arrive in Denmark. These are at increased risk of infectious diseases rarely seen in Danish children. Studies from the 1990s showed that 60% of IAC had infectious diseases and that the majority of these were undetected without screening. METHODS: The study is a prospective study of medical records from children seen in the adoption clinic at Copenhagen University Hospital in the period 2009-2013. Screening was done for hepatitis A (H

[Infectious diseases in Poland in 2001].

In 2001 surveillance system of infectious diseases in Poland remained unchanged. New cases of infectious diseases were recorded in 103 positions including intoxications. Tuberculosis and sexually transmitted infections were registered in separate systems. Influenza was the most frequently reported infectious disease with 576,449 cases, 63.9% less then in the previous year. The next most numerous were foodborne infections, which were reported in 24,393 cases, including 19,788 cases of infections caused by Salmonella sp. An increase in incidence was observed in the following diseases: viral hepa

[Tuberculosis detection and immunization of long-term travellers].

Reasons why long-term travel (> or =3 months) increases health risks are many, but mainly related to travellers' behaviour. On top of immunizations proposed to all travellers, those against hepatitis B, rabies and typhoid fever should be encouraged, but require time and money. Some vaccines like meningococcal vaccine and japanese encephalitis need to be discussed according to the destination. Tuberculosis is a rare disease in travellers in general but the incidence in health care workers reaches the one of the local population at risk, and therefore tuberculosis detection is recommended.

Intestinal Tuberculosis: A Diagnostic Challenge.

Diagnosing intestinal tuberculosis (TB) with uncommon clinical manifestations is often challenging. Here, we report a case of an alcoholic patient who presented with vague symptoms and was later diagnosed with intestinal TB. This patient experienced multiorgan failure causing hemodynamic instability requiring ionotropic support; acute hypoxic respiratory failure managed with non-invasive positive pressure ventilation, hepatic failure, transudative peritoneal effusion, and transudative pleural effusion. These conditions clouded our judgment to pursue colonoscopy for a definite diagnosis and del

Hepatosplenic involvement of tuberculosis - An uncommon presentation that demands attention in specific scenarios.

Abdominal tuberculosis (TB), a potential differential diagnosis for acute and chronic abdomen, should be considered in specific situations, such as immunosuppressant or biological drug use, HIV, or in patients residing in endemic areas. Although the presence of thoracic tuberculosis may indicate abdominal TB, only 15% of patients with abdominal TB have evidence of pulmonary disease. Involvement of the liver and spleen is a common autopsy finding in patients with disseminated TB. However, the most common pattern is of fine miliary lesions. Hepatic involvement with a macronodular pattern, a rare

Generalized peliosis hepatis and cirrhosis after long-term use of oral contraceptives.

Peliosis hepatis, a condition characterized by the presence of blood-filled lacunar spaces in the liver, has been described primarily in patients on androgenic steroid medication and patients with tuberculosis. It has never been reported as a complication of the use of oral contraceptives, except in association with and localized in contraceptive-induced hepatic tumors. The present report concerns a 43-yr-old woman with generalized peliosis hepatis that developed during long-term use of oral contraceptives. Extensive examination did not reveal a hepatic tumor. Liver cirrhosis and portal hypert

One-Month Rifapentine-Isoniazid Regimen Versus Six-Month Isoniazid Monotherapy for Latent Tuberculosis: Experience from a Reference Center.

Background and Objectives: Isoniazid monotherapy has been the most widely used treatment for latent tuberculosis infection (LTBI). Although effective, it has been associated with poor adherence and a higher incidence of adverse events. The shorter duration of rifamycin-based regimens has become increasingly preferable. The one month of daily rifapentine plus isoniazid (1HP) has demonstrated low toxicity and higher completion rates in HIV-infected populations. This study aims to compare the completion rate and adverse events between the 1HP and daily isoniazid for 6 months (6H) regimens in the

[The spectrum of liver disease in infection by the human immunodeficiency virus: a study of 50 liver biopsies].

BACKGROUND: The patients with human immunodeficiency virus (HIV) infection have hepatic abnormalities due to infective, neoplastic and toxic disorders. The aim of the present study was to describe the spectrum of liver disease in the HIV infection and to establish the usefulness and indications of hepatic biopsy. METHODS: The hepatic biopsies prospectively carried out in 50 patients infected by HIV with abnormal hepatic biochemical parameters, fever of unknown origin or space occupying lesions were evaluated. RESULTS: Hepatic granulomatosis was found in 37 patients (74%). In 15 (30%) the cul

"Triple infections" (fungal, bacterial and viral) in immunosuppressed renal transplant recipients.

In a period of 5 years, 8 out of 77 renal transplant patients showed simultaneous fungal, bacterial and viral infections. Candida albicans was found in all cases. The most severe bacterial complications were infections with Klebsiella, Pseudomonas and Staphylococcus aureus. Cytomegalovirus, persistent HBsAg positive hepatitis, herpes zoster, and herpes simplex infections were also found. Seven patients died of bacterial superinfection and miliary tuberculosis. The data presented show that "triple infections" are associated with high mortality and that miliary tuberculosis occurred frequently i

Congenital Tuberculosis in a Stillborn Calf.

Bovine tuberculosis is a zoonotic infectious disease of cattle caused by the bacterium Mycobacterium bovis. In adult cattle, transmission is mainly via the respiratory route, whereas, in young calves, oral infection is also common. Congenital tuberculosis is rare. The purpose of this study was to describe in utero infection of a bovine stillbirth. The fetus was necropsied and tissue samples were processed for histopathology, bacteriology and PCR; a sample of the isolated strain was genotyped using spoligotyping. The whole herd was tuberculin skin tested and the dam's serum was also assessed fo

The features of intestinal tuberculosis by contrast-enhanced ultrasound.

PURPOSE: To examine the imaging characteristics of intestinal tuberculosis by conventional ultrasound and contrast-enhanced ultrasound (CEUS). MATERIALS AND METHODS: Characteristics of the conventional and contrast-enhanced ultrasound images of 31 patients with intestinal tuberculosis confirmed by surgery were retrospectively examined. CEUS was used to evaluate the pattern of the bowel wall enhancement. RESULTS: Of the 31 patients with intestinal tuberculosis (IT), 27 had infections located at the ileocecum and 4 at the hepatic flexure of the colon. Conventional ultrasound showed that the me

[Malignant hemopathies combined with latent tuberculosis (a report of 2 cases)].

Two cases of a combination of a malignant blood disease with latent tuberculosis are presented. One of the patients was with acute leukemia and the other one was with non-Hodgkin's malignant lymphoma (Lymphosarcoma). The blood disease dominated the clinical picture and determined the severity of the course and the lethal outcome. The post mortem examination revealed hepatosplenic form of miliary tuberculosis in both patients. In one of the patients caseous tuberculosis of the bronchopulmonary, peribronchial and periportal lymph nodes was found, too. The tuberculous process had a latent course

Obstructive Jaundice Due to Periportal Tuberculous Lymphadenopathy in a Patient With Multidrug-Resistant (MDR) Tuberculosis: A Case Report.

Extrapulmonary tuberculosis is a common manifestation of tuberculosis, with lymph node involvement being the most frequent form. Among its presentations, periportal lymph node involvement leading to obstructive jaundice is extremely uncommon. Jaundice developing during anti-tubercular therapy (ATT) is frequently ascribed to drug-induced hepatotoxicity, which may contribute to delays in accurate diagnosis. We report a 17-year-old male with multidrug-resistant (MDR) lymph node tuberculosis who presented with progressive jaundice and right upper quadrant abdominal pain. Laboratory evaluation show
