Presurgical Sleep-Wake Disturbance and Postoperative Delirium: A Prospective Cohort Study.

OBJECTIVES: Prior studies on the relationship between presurgical sleep-wake disturbance and postoperative delirium have been restricted to subjective or indirect sleep measures. This study evaluated whether objective measures of presurgical sleep-wake disturbance are associated with postoperative delirium. DESIGN: Prospective study. SETTING: Single site, tertiary care medical center. PARTICIPANTS: Adults aged 40 years or older having surgical aortic valve replacement. MEASUREMENTS: Before surgery, we assessed sleep using self-report, ambulatory polysomnography, and actigraphy. Other measu

Irregular sleep-wake rhythm disorder: From the pathophysiologic perspective to the treatment.

Irregular sleep-wake rhythm disorder (ISWRD) is an intrinsic circadian rhythm disorder caused by loss of the brain's circadian regulation, through changes of the input and/or output to the suprachiasmatic nucleus (SCN), or of the SCN itself. Although there are limited prevalence data for this rare disease, ISWRD is associated with neurodegenerative disorders, including the Alzheimer disease (AD) and the Parkinson disease (PD), which will become increasingly prevalent in an aging population. It additionally presents in childhood developmental disorders, psychiatric disorders, and traumatic brai

Wake-up Call for India on Sleep Health: Systematic Review Findings.

BACKGROUND: An Increased burden of noncommunicable diseases and their association with sleep problems exists. OBJECTIVE: The objective of this study was to estimate the prevalence of sleep problems in India. MATERIALS AND METHODS: A meta-analysis of the studies conducted on Indian data was planned using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines. To assess the methodological quality, a 10-point Joanna Briggs Institute (JBI) checklist for prevalence studies was used. RESULTS: Pooled estimates of 100 eligible studies showed the prevalence was 25.7% for i

Sleep-wake modulation and pathogenesis of Alzheimer disease: Suggestions for postponement and treatment.

Sleep-wake disorders are recognized as one of the earliest symptoms of Alzheimer disease (AD). Accumulating evidence has highlighted a significant association between sleep-wake disorders and AD pathogenesis, suggesting that sleep-wake modulation could be a promising approach for postponing AD onset. The suprachiasmatic nucleus (SCN) and the pineal hormone melatonin are major central modulating components of the circadian rhythm system. Cerebrospinal fluid (CSF) melatonin levels are dramatically decreased in AD. Interestingly, the number of neurofibrillary tangles in the hippocampus, which is

Addressing challenges to recovery and building future resilience in the wake of COVID-19.

While organisational crisis theory posits a predictable set of stages involving pre-planning and preparation, acute crisis response, adaptation and recovery, the prolonged and cyclical nature of public-health restrictions related to COVID-19 presented new challenges for institutions of higher education and conditioned students, faculty and staff to adopt a crisis mindset as their baseline. Consequently, moving from crisis to recovery posed unique obstacles at both individual (eg anxiety, exhaustion and post-traumatic stress) and organisational levels (eg transition logistics, labour market cha

The organization of sleep-wake patterns around daily schedules in college students.

The amount of time available in a day is fixed, and consequently, sleep is often sacrificed for waking activities. For college students, daily activities, comprised of scheduled classes, work, study, social, and other extracurricular events, are major contributors to insufficient and poor-quality sleep. We investigated the impact of daily schedules on sleep-wake timing in 223 undergraduate students (age: 18-27 years, 37% females) from a United States university, who were monitored for ~30 days. Sleep-wake timing and daily recorded activities (attendance at academic, studying, exercise-based, a

The Relationship Between Intraindividual Sleep-Wake Variability and Mental Health in Adolescents.

OBJECTIVE: We investigated the relationship between irregular sleep-wake patterns and other aspects of sleep and mental health among adolescents. METHODS: Community-based sample of adolescents (n = 200, mean age 16.8; 61.7% female) completed baseline assessment (i.e. sleep quality, daytime sleepiness, depressive symptoms, social anhedonia, problematic alcohol use), and 7-8 consecutive days of wearing wrist actigraphy. Irregular sleep-wake patterns were assessed via two methods, both based on actigraphy, to capture day-to-day irregularity (intraindividual variability, IIV) and weekly irregular

A New Approach - Home Waking Salivary Cortisone to Screen for Adrenal Insufficiency.

Adrenal insufficiency is a common and potentially life-threatening endocrine disorder that can be drug induced or endogenous and of adrenal (primary) or pituitary/hypothalamic (secondary/tertiary) origin.1,2 Of particular concern in drug-induced disease is the patient with glucocorticoid- or opioid-induced adrenal insufficiency. Adrenal insufficiency of any cause is typically diagnosed biochemically with a subnormal morning serum cortisol (the circadian, awakening peak) and serum dehydroepiandrosterone sulfate, followed by or simultaneously with an assessment of the acute (30 and 60 minutes) s

The neurons that drive infradian sleep-wake and mania-like behavioral rhythms.

Infradian mood and sleep-wake rhythms with periods of 48 hr and beyond have been observed in bipolar disorder (BD) subjects that even persist in time isolation, indicating an endogenous origin. Here we show that mice exposed to methamphetamine (Meth) in drinking water develop infradian locomotor rhythms with periods of 48 hr and beyond which extend to sleep length and mania-like behaviors in support of a model for cycling in BD. This cycling capacity is abrogated upon genetic disruption of DA production in DA neurons of the ventral tegmental area (VTA) or ablation of nucleus accumbens (NAc) pr

The health of influenza surveillance and pandemic preparedness in the wake of the COVID-19 pandemic.

The COVID-19 pandemic is the first to have emerged when Next Generation Sequencing was readily available and it has played the major role in following evolution of the causative agent, Severe Acute Respiratory Syndrome Coronavirus 2. Response to the pandemic was greatly facilitated though use of existing influenza surveillance networks: World Health Organization (WHO) Global Influenza Surveillance and Response System (GISRS), focussing largely on human influenza, and the OFFLU network of expertise on avian influenza established by the Food and Agricultural Organization of the United Nations (F

Advances in the pharmacological management of non-24-h sleep-wake disorder.

Introduction: Melatonin, a hormone that regulates circadian rhythms and the sleep-wake cycle, is produced mainly during the dark period in the pineal gland and is suppressed by light exposure. Patients with non-24-h sleep-wake disorder (non-24) fail to entrain the master clock with the 24-h light-dark cycle due to the lack of light perception to the suprachiasmatic nucleus typically in totally blind individuals or other organic disorders in sighted individuals, causing a progressive delay in the sleep-wake cycle and periodic insomnia and daytime sleepiness.Areas covered: Herein, the authors re

The psychological correlates of distinct neural states occurring during wakeful rest.

When unoccupied by an explicit external task, humans engage in a wide range of different types of self-generated thinking. These are often unrelated to the immediate environment and have unique psychological features. Although contemporary perspectives on ongoing thought recognise the heterogeneity of these self-generated states, we lack both a clear understanding of how to classify the specific states, and how they can be mapped empirically. In the current study, we capitalise on advances in machine learning that allow continuous neural data to be divided into a set of distinct temporally re-

Circadian and sleep/wake-dependent variations in tau phosphorylation are driven by temperature.

STUDY OBJECTIVES: Aggregates of hyperphosphorylated tau protein are a hallmark of Alzheimer's disease (AD) and other tauopathies. Sleep disturbances are common in AD patients, and insufficient sleep may be a risk factor for AD. Recent evidence suggests that tau phosphorylation is dysregulated by sleep disturbances in mice. However, the physiological regulation of tau phosphorylation during the sleep-wake cycle is currently unknown. We thus aimed to determine whether tau phosphorylation is regulated by circadian rhythms, inherently linked to the sleep-wake cycle. METHODS: To answer these quest

The untold story of the scorching sun-A wake up call for sun protection.

Despite extensive warnings from health authorities regarding the dangers of direct sunlight exposure, most people still turn to sun bathing to get a golden tan. Unfortunately, that pleasant tan appearance is often lost because of over exposure to sunlight, resulting in painful red sunburns. In this paper we are reporting a case with significant sunburn injuries that required hospitalization and treatment in a burn center. Concurrently a pilot study was conducted to assess the knowledge about sun protection among the adult population and the results are discussed. The results obtained from the
