Over a 2-year period, 31 patients underwent prolonged hydrostatic bladder distension for benign and malignant bladder disease in this unit. Analysis of acid-base-metabolism in cerebrospinal fluid is not easy, because pO2, pCO2 and pH change already within a short time after sampling. Acid-base-values in CSF have been measured in 50 healthy patients within 20 seconds after withdrawal. Our results (pH 7.298 +/- 0.007; pCO2 46.94 +/- 4.84 mmHg; HCO3 22.78 +/- 2.15 mVal/l; BE-2.4 +/- 2.39 mVal/l; pO2 50.27 +/- 14.06 mmHg) demonstrate, that CSF is more acidic than often claimed. This is important, whenever possible precipitations by intrathecal injection of local anaesthetics are discussed.

Intrathecal morphine provides effective postoperative pain relief in major orthopaedic surgery. In use, however, is associated with unpleasant side effects like nausea and vomiting. The effect of different premedications on postoperative emetic sequelae induced by intrathecal morphine was studied in a prospective, double blind study. Sixty patients scheduled for arthroplasty surgery of the lower extremity were anaesthetized with spinal anaesthesia with a combination of isobaric bupivacaine 20 mg and morphine 0.3 mg. Although it has been known for a long time that hearing impairment may be a late complication, only a few case reports have been published, and not all have included audiograms.
