Systolic and diastolic function assessment in fabry disease patients using speckle-tracking imaging and comparison with conventional echocardiographic measurements.

BACKGROUND: Fabry cardiomyopathy is characterized by progressive left ventricular hypertrophy (LVH) associated with diastolic dysfunction and is the most common cause of death in Fabry disease (FD). However, LVH is not present in all subjects, particularly early in disease progression and in female patients. Direct assessment of myocardial deformation by strain and strain rate (SR) analysis may be sensitive to detect subclinical Fabry cardiomyopathy independent of the presence of LVH. METHODS: Systolic (longitudinal, circumferential, and radial systolic strain and SR) and diastolic (SR during

Systematic approach: an evidence management strategy for better decision-making.

Evidence-based medicine aims to apply best evidences to medical decision making. Although evidence are established using the scientific method, decision making has received less priority. Decision making is a mental process requiring a systematic analysis of evidences for a specific use, leading to a final choice of action. In many scientific disciplines involving decision and control, this process has been significantly improved by making it as a system (a set of interacting entities forming an integrated whole). Hypothesizing that most medical decisions can be described as a system, we prese

Electrical delay in apically positioned left ventricular leads and clinical outcome after cardiac resynchronization therapy.

INTRODUCTION: In recent studies, an anatomical apical left ventricular (LV) lead pacing location has been associated with deleterious outcome after cardiac resynchronization therapy (CRT). The differential impact of the LV lead electrical location in these patients remains unknown. METHODS AND RESULTS: Thirty-one consecutive CRT patients (mean age 71.7 ± 12.7 years, 55% left bundle-branch block [LBBB] morphology) with an apical LV lead and LV lead electrical delay (LVLED) were studied. Anatomical LV lead location was determined via review of coronary venography and chest radiographs. Electric

Transventricular edge-to-edge repair of the mitral valve during surgical ventricular restoration: review of the literature.

BACKGROUND: Mitral valve surgery for functional ischemic mitral regurgitation (MR) in high-risk patients, including those requiring multiple concomitant cardiac operations, carries a significant risk of morbidity and mortality. In patients undergoing surgical ventricular restoration, transventricular edge-to-edge repair provides an effective alternative to conventional mitral valve surgery. We report such a case. METHODS: A 67-year-old male with ischemic cardiomyopathy and severe left ventricular dysfunction presented to our institution with a 3-month history of dyspnea on exertion, angina, a

Coexistence of multiple and widespread cardiovascular complications in a patient with Marfan syndrome.

Inherited connective tissue diseases such as Marfan syndrome are frequently associated with cardiovascular manifestations. Aortic involvement with dilation and dissection is the most common finding and the major cause of death in Marfan syndrome patients. We report the echocardiographic study of a 53-year-old male patient with uncommon coexistence of cardiovascular abnormalities typical of connective tissue disease at first clinical presentation in acute clinical setting: dissection of the descending aorta associated with severe mitral regurgitation due to leaflet flail and massive aortic insu

Resynchronization in a patient with normal systolic function & intermittent left bundle branch block with recurrent left ventricular failure.

A 50 year old lady presented with recurrent hospitalizations for Left ventricular failure (LVF) associated with rate related left bundle branch block (LBBB). Though the baseline ECG at rates less than 90bpm demonstrated narrow QRS, normal LV systolic function and no LV dysynchrony, at rates greater than 90bpm, LBBB was noted with concurrent LV dyssynchrony. After multiple hospitalizations with LVF, as a last resort, cardiac resynchronization therapy (CRT) was performed. At 22 months' follow up the patient continues to be asymptomatic.

Assessing global and regional left ventricular myocardial function in elderly patients using the bidimensional strain method.

UNLABELLED: Biological and anatomical alterations in the elderly result in modifications of the myocardial deformation detected previously by magnetic resonance imaging (MRI) technology and could have consequences on speckle tracking's parameters in this patient population. AIM: To compare left ventricular (LV) 2D strain between elderly patients and young individuals without heart disease. POPULATION AND METHODS: Patients without history of cardiac disease were enrolled from the geriatric department. After echocardiographic examination, exclusion criteria were LV myocardial abnormality, valv

Biventricular thrombi associated with peripartum cardiomyopathy.

A 22-year old woman visited the LAMB Hospital, Parbatipur, Dinajpur, Bangladesh, in February 2010, with exertional dyspnea for three weeks. She had had a normal vaginal delivery four months ago; 2-dimensional echocardiogram showed severe left ventricular dysfunction and biventricular thrombi, which resolved without complications after anticoagulation. Biventricular thrombosis with peripartum cardiomyopathy is quite a rare finding, and its clinical course and proper management is not known. No such case has previously been reported in Bangladesh.

Methadone-induced torsades de pointes: a twist of fate.

A case of methadone-induced torsades de pointes is presented to demonstrate clinical features that predispose patients to this serious cardiac arrhythmia. A patient who was receiving methadone maintenance treatment for heroin addiction presented to the hospital with dizziness and near-syncope. He was taking a relatively high dose of methadone but was not taking any concomitant cytochrome P450 inhibitor or QT-prolonging drugs. He had prolonged corrected QT interval, hypokalemia, and hypomagnesemia on admission and was later found to have severe left ventricular dysfunction. On admission to a te

[Myocardial noncompaction in a patient with kyphoscoliosis].

Myocardial noncompaction of the ventricular myocardium is a rare congenital cardiomyopathy characterized by excessively protrusive trabeculae and deep trabecular recesses in one or more segments of the ventricle. A 37-year-old male patient with prominent kyphoscoliosis presented with dyspnea. Transthoracic echocardiography showed trabeculations and deep intratrabecular recesses along the ventricular cavity in the apical and lateral segments of the left ventricle. Color Doppler imaging showed blood flow in these recesses. Diameters of the left ventricle and atrium were increased, with global hy

The immediate effects of pacemaker-related electric remodelling on left ventricular function in patients with sick sinus syndrome.

AIMS: The immediate effects of electric remodelling on the left ventricular (LV) function by dual-chamber pacemakers remain unknown. The purpose of our study was to assess the interaction between heart rates and right ventricular pacing (VP) on LV contractility and diastolic function. METHOD AND RESULTS: Twenty-five patients with dual-chamber pacemakers and sick sinus syndrome were evaluated. Echocardiographic examinations included standard and tissue-Doppler echocardiography at bilateral mitral annulus margins under either the intrinsic atrio-ventricular sequential conduction (ventricular se

Cardiac incoordination induced by left bundle branch block: its relation with left ventricular systolic function in patients with and without cardiomyopathy.

BACKGROUND: Although left bundle branch block (LBBB) alters the electrical activation of the heart, it is unknown how it might change the process of myocardial coordination (MC) and how it may affect the left ventricular (LV) systolic function. The present study assessed the effects of LBBB on MC in patients with LBBB with and without dilated (DCMP) or ischemic cardiomyopathy (ICMP). METHODS: Tissue Doppler echocardiography (TDE) was performed in 86 individuals: 21 with isolated LBBB, 26 patients with DCMP + LBBB, 19 patients with ICMP + LBBB and in 20 healthy individuals (Controls). MC was a

ICD implantation and arrhythmia-free survival in patients with depressed LV function following surgery for valvular heart disease.

BACKGROUND: Although prophylactic implantable cardioverter-defibrillator (ICD) implantation is beneficial in patients with severe ischemic cardiomyopathy, it is unclear whether patients with cardiomyopathy due to valvular heart disease have a similar benefit. METHODS: We followed 17 patients (14 men/three women, age 62 +/- 13 years, left ventricular ejection fraction [LVEF] 29 +/- 10%) who had nonischemic valvular cardiomyopathy, underwent valvular heart surgery (aortic valve replacement, mitral valve replacement, and/or mitral valve repair), and subsequently had an electrophysiology study (E

Coronary artery bypass in myocardial ischemia of the young due to hydatid cyst.

Cardiac hydatid cyst is an uncommon disease, especially in France. It is rarely responsible for myocardial ischemia, and even more rarely in the young. We report the clinical features and imaging of a 24-year-old Tunisian woman with hydatid cyst of the free wall of the left ventricle responsible for myocardial ischemia, associated with a hepatic hydatid cyst. Management consisted of a combination of surgery with cystopericystectomy and coronary artery bypass graft and prolonged medical treatment as for treatment of hepatic hydatid cyst. Three-year follow-up revealed no recurrence. This case il

Measurement of left ventricular dyssynchrony in patients with ischaemic cardiomyopathy: a comparison of real-time three-dimensional and tissue Doppler echocardiography.

BACKGROUND: Real-time three-dimensional echocardiography (RT3DE) is an alternative modality to tissue Doppler imaging (TDI) for assessment of intraventricular dyssynchrony but its role is yet to be defined. OBJECTIVES: To (1) compare RT3DE and TDI for assessment of intraventricular dyssynchrony; (2) determine whether the two techniques agreed regarding the magnitude of dyssynchrony and identification of the site of maximal mechanical delay; and (3) investigate the reason for disagreement. PATIENTS: 100 patients with ischaemic cardiomyopathy. SETTING: Tertiary referral cardiac unit. MAIN OU

Long-term support with milrinone prior to cardiac transplantation in a neonate with left ventricular noncompaction cardiomyopathy.

A 2-week-old female infant presented with acute decompensated left ventricular failure. Echocardiography diagnosed left ventricular noncompaction cardiomyopathy with dilated phenotype and a left ventricular shortening fraction <10%. The infant was mechanically ventilated for 2 weeks and then successfully extubated. She was maintained on intravenous milrinone for 6 months until she underwent successful orthotopic heart transplantation. Young children can be supported with inotropes for prolonged periods while awaiting heart transplantation.
