Incidence and Risk Factors of Early-onset Glaucoma following Pediatric Cataract Surgery in Egyptian Children: One-year Study.

AIM: To study the incidence and risk factors of glaucoma occurring within 1 year following pediatric cataract surgery in Egyptian children. MATERIALS AND METHODS: This is a prospective nonrandomized study conducted at Aburich Children's Hospital, over a period of 1 year on a cohort of Egyptian patients with congenital and infantile cataract. One hundred and fifty eyes of 88 patients were enrolled in this study. All the patients underwent anterior approach removal of lens matter, whereas primary intraocular lens (IOL) implantation was carried at the age of 1 and 2 years for unilateral and bila

Intraocular pressure response to the replacement of pilocarpine or carbachol with echothiophate.

In 20 patients with open-angle glaucoma in aphakia or pseudophakia whose intraocular pressure had remained uncontrolled on their current medical therapy, the medication was changed from pilocarpine or carbachol to echothiophate iodide. In all, 12 patients (60%) showed a statistically significant improvement in pressure control, 7 (35%) showed no change, and 1 had higher pressure. One-third of the patients with improved intraocular-pressure control eventually required laser or incisional surgery after a mean of 23 months, whereas the remaining subjects were controlled for the duration of the fo

Keratoplasty and intraocular lenses.

This paper describes the surgical results in 163 patients in whom intraocular lenses (IOL) had been implanted, exchanged, or removed during penetrating keratoplasty (PKP). They were divided into the following groups: PKP, cataract extraction, and IOL implant; PKP in aphakia and IOL implant; PKP and IOL exchange; PKP and IOL removal. In all groups clear transplants were obtained in over 92% of the cases, with best optical and visual results obtained in cases of Fuchs' dystrophy with cataract extraction and IOL implant (98% clear grafts and vision better than 20/40 in most cases). There were no

[Retinal light damage--summary of experimental and clinical observations].

The article presents a comprehensive review of the literature on light damage to the retina with special reference to clinical aspects. A definition of the different modes of action of light on the retina and a brief summary of current theories on possible mechanisms of light-damage are presented. Experimental studies applying ophthalmological light sources to the primate eye are described as well as investigations on photochemical light-damage to the human eye. Possible implications of these observations for some types of retinal degeneration are discussed. Finally, the importance of consider

Rate of ocular trauma in children operated on for unilateral cataract in infancy-data from the Infant Aphakia Treatment Study.

To determine whether the fellow eye of children who have undergone unilateral cataract extraction in the first year of life are at increased risk of injury and vision loss, the 10.5-year data on 109 of 114 children enrolled in the Infant Aphakia Treatment Study were examined. Based on this limited data, it was estimated that the fellow eye is at greater risk of injury than the operated eye. Our data do not support the risk being higher in children with the worst vision in the treated eye.

The Infant Aphakia Treatment Study Contact Lens Experience to Age 5 Years.

PURPOSE: To describe our experience treating a cohort of unilateral aphakic infants with contact lenses in the Infant Aphakia Treatment Study (IATS). MATERIALS AND METHODS: Fifty-seven of the 114 infants in the IATS were randomized to contact lens wear; all were followed until age 5 years, although a few had lapses in care. An examination under anesthesia, including keratometry, was performed at the time of enrollment and at approximately 1 year of age; keratometry was performed again at 5 years of age. A traveling examiner assessed visual acuity at approximately 1 year of age and again at 4.

Implanting phakic IOLs (PIOLs) in phakic eyes is regarded as an established option in refractive surgery for many years when treating high myopia, hyperopia or ametropia. Even for eyes which show a combination of refractive errors (including astigmatism) this can be an appropriate option. The surgery itself requires certain anatomical and refractive requirements, since, as with any intraocular procedure, there are different possibilities of complication depending on the chosen IOL. Therefore, any potential candidate for refractive surgery should be treated in accordance with the current recomm

Cataract, ocular surgery, aphakia, and the chromatic expression of the painter Jovan Bijelić.

BACKGROUND/AIM: Approaching art from the standpoint of optics and the artist’s eye pathology can sometimes explain the shift of the spectral colors in the work of some artists with cataract and aphakia. This may not be obvious in the paintings of other artists with the same eye pathology. The aim of this study was to create a timeline from the recently obtained details of the cataract surgery, his best corrected aphakic visual acuity, and the last paintings of the artist Jovan Bijelić. METHODS: The research included primary and secondary source material: Bijelić’s paintings from all stages of

Using catheter needles to deliver an intraocular lens for intrascleral fixation.

UNLABELLED: A new method for delivering intraocular lens (IOL) haptics through sclerotomies for intrascleral fixation using catheter needles was evaluated in pig eyes. A bent catheter needle was used to penetrate the surgical corneal wound from the presumed sclerotomy. Extraocularly, the leading haptic of the IOL was fixated between the external tube and the internal needle of the catheter needle. The fixation was strong enough to support the IOL and to extract the leading haptic through the sclerotomy site. The trailing haptic was fixated using another catheter needle extraocularly and was su

Phototoxic maculopathy after secondary intraocular lens implantation.

We report phototoxic maculopathy in 2 aphakic patients after secondary, scleral-sutured intraocular lens implantation. This rare complication may have been triggered by drugs that increased the photosensitivity of the tissue. Careful preoperative assessment is necessary to question the receipts of photosensitizing drugs and identify diseases that predispose patients to phototoxic injuries.

A simplified technique for stable transscleral suture fixation of posterior chamber intraocular lenses.

We describe a technique for implanting ciliary sulcus posterior chamber intraocular lenses (PCIOLs). This technique uses radial keratotomy (RK) markers to facilitate PCIOL centration, 2-point scleral suture fixation for each haptic to prevent PCIOL tilt, and partial thickness sclerotomies to prevent suture erosion. Postoperative results of 20 eyes with PCIOLs sutured to the ciliary sulcus were reviewed. Suture placement was determined using a Mendez degree gauge with Bores axis marker (Katena USA, Denville, NJ). Two-point scleral suture fixation without a scleral flap was used for each haptic.

[Sandwich intraocular lens implant: a concept for aphakia correction in children].

BACKGROUND: In the management of congenital cataracts the correction of aphakia is still an unsatisfactorily solved problem. As far as surgical techniques and materials are concerned, the implantation of an IOL seems to be justified even in younger children; but choosing the refractive power of the lens is somewhat difficult regarding the expected growth of the eye. MATERIALS AND METHODS: A new type of IOL is presented as a solution for this problem. Being composed of a PMMA-fashioned optic and haptic it bears a silicone lens which is fastened on top of it. The supporting lens is made of PMMA

Is the phakic eye normal in monocular pediatric aphakia?

This retrospective review of 97 pediatric patients who underwent monocular surgery for congenital or developmental cataracts studied the incidence of abnormalities in the contralateral phakic eye. Fifty-nine percent of patients had a normal phakic eye. However, 40 patients showed at least one abnormality in the phakic eye: reduced vision (21%), nystagmus (19%), cataract (15%), iris heterochromia (9%), myopia (6%), microphthalmos (6%), pupillary miosis (2%), congenital glaucoma (2%), optic nerve abnormality (2%), aniridia (1%), and corneal opacity (1%). Not all abnormalities were detected at th

[Refraction epikeratoplasty in correction of high ametropia and treatment of keratoconus].

The results of 48 surgeries (refraction epikeratoplasty) for aphakia, high ametropia, and keratoconus evidence the efficacy of such operation in the afore-said conditions. Further studies are necessary to elucidate the pathogenesis of complications, improve the technique of surgery and technology of the transplant preparation in order to make it more safe and improve its functional results.
