The cauliflower ear presents a challenge to the surgeon. Patients complain of discomfort and appearance. Three patients were treated surgically via a posterior approach to remove the hardened segment and re-sculpture a leaf of cartilage left in place. An acceptable cosmetic result was achieved and all patients are currently pain free.

Aseptic necrosis and cholesteatoma of the external auditory canal can be stages of the same pathological entity. The majority of these patients suffered from symptoms similar to those of external otitis. It is suggested that minor mechanical trauma, e.g. cotton lip swabs, plays a role in the pathogenesis of external ear canal cholesteatoma. During the last 10 years we have been surgically treating 22 patients with diagnosis of aseptic necrosis and cholesteatoma of the bony external auditory canal. All 22 patients registered significant postoperative improvement.

Extramedullary plasmacytomas are rare plasma cell tumors of the soft tissue that predominantly occur in the head and neck. Fungal infections of the external auditory canal can pose a great clinical challenge both in diagnosis and in therapy. Most infections present in patients who have undergone previous medical treatment of the external canal, which has changed the pH and normal canal flora, or in those who have diminished capacity for resisting infections. Diagnosis is made by clinical inspection and selective cultures. Therapy consists of complete cleaning followed by use of topical antimycotic agents. Systemic therapy is rarely required.

Modern optical technology has made available fiberoptic and rigid endoscopes with diameters of 2 mm and less with acceptable resolution. Endoscopes of small caliber were introduced through a strategically placed myringotomy or an existing perforation to perform exploration of the middle ear as an in-the-office procedure. This technique is now routinely used as an adjunct in the diagnostic evaluation of patients with suspected middle ear conditions. Exploratory surgery of the middle ear may be avoided or definitive procedures may be planned better based on endoscopic findings.

The authors present 15 patients with middle ear malignant tumours, of which 13 were squamous cell carcinomas, one was a rhabdomyosarcoma and one a malignant lymphoma. The malposition of an otherwise normal-appearing external ear is not uncommon in certain craniofacial syndromes. This paper presents a 10-year experience of 14 patients who underwent external ear repositioning. In this technique, a posterior incision is used to mobilize surrounding soft tissues circumferentially around the external auditory canal, which then serves as an axis for anterior rotation and elevation. All patients who have undergone obliteration of the middle ear cleft at Plymouth are reviewed. All had chronically discharging mastoid cavities and middle ears resistant to medical treatment, and all had at least one major operation to eradicate disease. All had an average hearing loss on the affected side of more than 80 decibels prior to operation. All ears have remained dry since obliteration, with a blind pit in the meatus. There have been no complications apart from a collection of fluid under one muscle flap.

We report two infants who had localised congenital tuberculous otitis. In both cases the infants presented with an ear discharge and both mothers had been diagnosed as having miliary tuberculosis. Infection is thought to have occurred in utero or during birth.

Cholesteatoma is characterized by the accumulation of a desquamating epithelium within the middle ear. Its development and spreading are favoured by chronic inflammatory and suppurative processes. The main clinical manifestations are persistent or episodic, frequently fetid purulent discharge, hearing impairment, vertigo and rarely complications (mastoiditis, labyrinthitis or meningitis). Therapy is preventive: careful management and follow-up of children with recurrent or chronic otitis media with effusion. Surgical treatment is mandatory once cholesteatoma is constituted.

The work of Japanese research committees on the relation of viral infection to sudden deafness and acute profound deafness is briefly introduced. The criteria for making a diagnosis of sudden deafness and mumps deafness are described.
