Inferior vena cava filter-related thrombosis: Mechanisms, clinical features, prevention, diagnosis, and risk-stratified management.

Inferior vena cava (IVC) filters can provide mechanical protection from pulmonary embolism in selected patients who cannot receive anticoagulation, but filter-related thrombosis is an important complication. A uniform definition is lacking. Thrombus in the filter region may represent emboli captured by the filter, extension of pre-existing deep vein thrombosis, or local thrombus formation around the device. Filter-induced flow disturbance and vessel-wall responses may contribute to local thrombosis, although direct mechanistic evidence in humans remains limited. Reported occurrence varies subs

Flow-Controlled Atherectomy Treatment for Severely Calcified Femoropopliteal Lesion; Seat Belt and Air Bag Technique.

BACKGROUND: Distal embolization is a known risk during Jetstream atherectomy for severely calcified femoropopliteal lesions. AIM: This study aimed to evaluate the effectiveness of a novel "flow-controlled atherectomy treatment (FCAT)" using proximal protection to prevent distal embolization during Jetstream atherectomy, both in vitro and in vivo. METHODS: The study consisted of in vitro experiments and a retrospective, single-center clinical analysis. The clinical study included 18 patients with severely calcified femoropopliteal artery lesions. The FCAT procedural steps were as follows: (1)

Cerebral embolic protection (CEP) during transcatheter aortic valve replacement (TAVR) is associated with a lower rate of stroke at a high-volume center.

BACKGROUND: Cerebral embolic protection (CEP) reduces strokes during transcatheter aortic valve replacement (TAVR) but is not standard of care at most centers. AIM: To assess the impact of CEP use in real-world practice at a tertiary center using CEP as a standard of care during TAVR. METHODS: In-hospital outcome of 2173 patients was compared to 328 (13.1%) patients who could not receive CEP during TAVR due to anatomical or technical reasons. In a secondary analysis, adjusted stroke risk was compared using propensity score matching. RESULTS: Non-CEP patients had significantly higher Society

Transcatheter Aortic Valve Replacement in a Patient With Right-Side Dominant Double Aortic Arch.

BACKGROUND: Double aortic arch (DAA) is a rare congenital vascular anomaly that is typically diagnosed in infancy. In adult patients undergoing transcatheter aortic valve replacement (TAVR), an undetected DAA can present unique anatomical and procedural challenges. CASE SUMMARY: A 71-year-old woman with end-stage renal disease and severe aortic stenosis was incidentally found to have a DAA during preprocedural TAVR evaluation. The device was inserted via the right femoral artery, as it provided the largest vessel diameter and most favorable anatomy for device passage. The pronounced curvature

Bioabsorbable, elastomer-coated magnesium alloy coils for treating saccular cerebrovascular aneurysms.

Cerebral aneurysm embolization is a therapeutic approach to prevent rupture and resultant clinical sequelae. Current, non-biodegradable metallic coils (platinum or tungsten) are the first-line choice to secure cerebral aneurysms. However, clinical studies report that up to 17% of aneurysms recur within 1 year after coiling, leading to retreatment and additional surgery. It would be ideal for the aneurysm coiling material to induce acute thrombotic occlusion, contribute to a tissue development process to fortify the degenerated vessel wall, and ultimately resorb to avoid leaving a permanent for

1-Month Results From a Prospective Experience on CAS Using CGuard Stent System: The IRONGUARD 2 Study.

OBJECTIVES: This study sought to evaluate 30-day safety and efficacy of dual-layer mesh-covered carotid stent systems for carotid artery stenting (CAS) in the clinical practice. BACKGROUND: When compared with carotid endarterectomy, CAS has been associated with a higher rate of post procedural neurologic events; these could be related to plaque's debris prolapsing through stent's mesh. Consequently, the need for increased plaque coverage has resulted in the development of dual-layer mesh-covered carotid stent systems. METHODS: From January 2017 to June 2019, a physician-initiated, prospectiv

Watching 'AF'ter the Heart: the WATCHMANTM device for atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia and a leading cause of cardioembolic stroke. In nonvalvular atrial fibrillation (NVAF), thrombus formation occurs predominantly within the left atrial appendage (LAA), making it a key therapeutic target. While oral anticoagulation remains standard for thromboembolic risk reduction, contraindications, elevated bleeding risk, and therapy intolerance drive the need for nonpharmacologic alternatives. This review examines the role of the WATCHMAN left atrial appendage occlusion (LAAO) device in NVAF-related stroke prevention,

The PERFORMANCE II Trial: A Prospective Multicenter Investigation of a Novel Carotid Stent System.

BACKGROUND: Several randomized clinical trials have shown that the composite endpoint of death, stroke, and myocardial infarction (MI) is equivalent between carotid artery stenting and carotid endarterectomy. However, the risk of minor stroke has been consistently higher with carotid artery stenting. OBJECTIVES: The authors sought to evaluate the safety and effectiveness of a novel carotid stent system comprised of a stent, an adjustable integrated embolic filter and a postdilation balloon, in patients at elevated risk for adverse events from carotid endarterectomy. METHODS: PERFORMANCE II (

Primary Stenting of Right-Sided Subclavian Artery Stenosis Presenting as Subclavian Steal Syndrome: Report of 3 Cases and Literature Review.

BACKGROUND: Right-sided subclavian artery stenosis (SAS) is a rare cerebrovascular disease involving the upper extremities. Considering an endovascular approach for its management requires increased endovascular and catheterization skills when compared with the left side, due to the close approximation of the right subclavian artery origin, vertebral, and common carotid arteries. METHODS: Three patients suffering from proximal right-sided SAS were treated in our center through primary stenting. Percutaneous transfemoral and transbrachial approaches were used for vascular access, whereas in 2

A novel carotid device for embolic diversion: lessons learned from a "first in man" trial in patients with atrial fibrillation.

PURPOSE: This study was designed to investigate a novel device that was developed to direct emboli away from the internal carotid artery to the external carotid artery to prevent embolic stroke while avoiding the necessity for anticoagulation with warfarin. METHODS: A stent-like, self-expandable filter was developed consisting of fine cobalt alloy mesh. After promising preclinical animal testing, the device was implanted into the common carotid artery extending into the external carotid artery in three patients with atrial fibrillation, each of whom were at high-risk for cerebral emboli and w

Percutaneous internal carotid artery angioplasty with stenting: early and long-term results.

BACKGROUND: Cerebro-vascular accidents are the third most common cause of death. The most frequent localisation of lesions responsible for stroke are bifurcation of the common carotid artery or the proximal internal carotid artery (ICA). Surgical carotid endarterectomy (CEA) and carotid artery stenting (CAS) are the non-pharmacological methods used to treat carotid artery stenosis. AIM: To assess the efficacy and safety of CAS of ICA. METHODS: CAS was performed in 75 patients (49 males, 26 females) with a mean age of 65.2+/-9.1 years. Twenty (26.7%) patients underwent CAS with the use of the

Device-related infections: a review.

The use of surgically implanted devices has increased as a result of their beneficial effect on quality of life, and in some circumstances, on patient survival rates. They can, however, be associated with a variety of complications, the most dreaded being infection. Device-related infections are important to understand because of the morbidity and mortality associated with them. Frequently, patients are managed with hospitalization, prolonged courses of antibiotics, and surgical interventions, all of which can negatively impact on patients' quality of life. Such care is also associated with in

Sentinel Cerebral Embolic Protection in Patients with Transcatheter Aortic Valve Replacement: A Meta-analysis of Randomized Controlled Trials and Trial Sequential Analysis.

Transcatheter aortic valve replacement (TAVR) is widely used to treat severe aortic stenosis, and stroke remains a serious complication. The Sentinel cerebral embolic protection (CEP) device is designed to reduce this risk. However, it is still unclear if it truly improves patient outcomes. This meta-analysis evaluates whether the Sentinel device reduces stroke, death, or neurological complications. We searched PubMed, Embase, and Cochrane Central for randomized controlled trials comparing the use of the Sentinel CEP versus no device in patients undergoing TAVR. Outcomes included stroke within

Stenting for Left Subclavian Artery Stenosis before and after Coronary Artery Bypass Grafting Using the Internal Mammary Artery: A Report of Three Cases.

Stenosis of the subclavian artery proximal to the origin of the internal mammary artery (IMA) used for coronary artery bypass grafting may produce flow reversal (steal syndrome) and cause myocardial ischemia. We present three cases of subclavian artery stenosis proximal to the IMA before and after CABG. The first case developed symptomatic myocardial ischemia resulting from a variant of coronary-subclavian steal syndrome. The second case had asymptomatic subclavian artery stenosis proximal to the IMA used for CABG. In the third case we planned to perform CABG using the left IMA to treat cardia

Endovascular grafting for abdominal aortic aneurysms.

Despite the initial success of endovascular grafts in a very difficult patient population, many problems remain. These procedures are often time-consuming and quite complicated, requiring the close cooperation of an experienced team of vascular surgeons and interventional radiologists. Access may be difficult through occluded, stenotic, and tortuous vessels. Inadequate graft deployment may result in arterial rupture or graft migration, which could potentially lead to acute occlusion of the renal or iliac arteries. Occlusion of the inferior mesenteric artery may result in ischemic colitis. Also

Sex does not have an impact on perioperative transfemoral carotid artery stenting outcomes among octogenarians.

OBJECTIVE: Transfemoral carotid artery stenting (CAS) has been validated as an acceptable alternative to carotid endarterectomy in patients at high risk for open surgery. There are variable sex- and age-based differences in transfemoral CAS outcomes of published randomized controlled trials. The aim of our study was to evaluate sex-based differences in perioperative outcomes after transfemoral CAS performed in octogenarians. METHODS: The National Surgical Quality Improvement Program targeted vascular module was queried for all patients ≥80 years of age who underwent transfemoral CAS between 2

The role of intracoronary thrombolysis in selected patients presenting with ST-elevation myocardial infarction: a case series.

BACKGROUND: Primary percutaneous coronary intervention (PCI) is the cornerstone of management for ST-elevation myocardial infarction (STEMI). However, large intracoronary thrombus burden complicates up to 70% of STEMI cases. Adjunct therapies described to address intracoronary thrombus include manual and mechanical thrombectomy, use of distal protection device and intracoronary anti-thrombotic therapies. CASE SUMMARY: This series demonstrates the use of intracoronary thrombolysis in the setting of large coronary thrombus, bifurcation lesions with vessel size mismatch, diffuse thrombosis witho

[Stenting for cerebral infarction due to stenosis of a persistent primitive proatlantal artery: a case report].

Cerebrovascular disorders complicated by persistent primitive arteries are commonly reported, but, we herein present a 78-year-old man with cerebral infarction due to stenosis of a persistent primitive artery by itself. Cerebral angiography revealed bilateral vertebral artery aplasia to be complicated by a persistent primitive proatlantal artery which had become an important collateral circulation pathway. While the patient was hospitalized because of mild sensory impairment, he also developed visual field constriction and cerebellar symptoms due to recurrent stroke despite anticoagulation tre

Dual-layered micromesh stent technology for embolic prevention in carotid revascularization: technical experience and clinical outcomes from a high-volume interventional radiology center.

BACKGROUND: Carotid artery stenting (CAS) has become a cornerstone of carotid revascularization for stroke prevention. Despite the advantages of CAS, large-scale randomized trials involving prior (single-layer) first generation stents (FGS) demonstrated a higher risk of periprocedural cerebrovascular events compared to surgery. Dual-layer mesh-covered stents (DLSs) showed promising results in terms of 30-day embolic events in initial studies; larger-scale evidence is accumulating. This study aims to evaluate 30-day clinical efficacy of DLS against a closed-cell stent, based on large-volume dat
