Securing the Airway in Pretracheal Dermal Metastases From Oropharyngeal Carcinoma: A Case Report.

Dermal metastases reflect the ominous and aggressive spread of oropharyngeal squamous cell carcinomas. The rampant proliferation of these metastatic tumors to the neck results in respiratory distress and impending airway obstruction. We report a case of a patient with massive neck dermal metastases requiring urgent airway control for intermittent stridor. Awake tracheostomy is generally regarded as the gold standard to manage the compromised airway. However, in this unusual case, after discussion between surgeon and anesthesiologist, because of the anticipated formidable difficulties in perfor

Effects of the jaw-thrust manoeuvre in the semi-sitting position on securing a clear airway during fibreoptic intubation.

Securing a clear airway is important for successful fibreoptic intubation. We investigated whether the jaw-thrust manoeuvre in the 25° semi-sitting position improves airway clearance compared with the supine position in 88 anaesthetised patients randomly assigned to the two positions. After induction of anaesthesia, the fibreoptic bronchoscope was advanced into the mouth along the dorsum of the tongue during the jaw-thrust manoeuvre. Airway clearance was assessed at the level of the soft palate and epiglottis. Patients in the 25° semi-sitting position had clearer airways (judged subjectively b

Graphene oxide-deposited microfiber: a new photothermal device for various microbubble generation.

Hypospadias is a congenital anomaly of the penis with an occurrence of approximately 1 in 200 boys, but the etiology of the majority of hypospadias has remained unknown. Numerous genes have been reported as having variants in hypospadias patients, and many studies on genetic deletion of key genes in mouse genital development have also been published. Until now, no comparative analysis in the genes related literature has been reported. The basic knowledge of penile development and hypospadias is mainly obtained from animal model studies. Understanding of the differences and similarities between

Awake video laryngoscope intubation: case report of a patient with a nasopharyngeal mass.

Difficult airway management remains central to anesthesia practice. Video laryngoscopes have been an adjunct to airway management since the early 2000s. They have been shown to improve visualization of the glottic opening and have become a useful aid in managing difficult airways. To date, the preferred method for difficult airway management remains awake fiberoptic intubation. The purpose of this article is to summarize the use of a video laryngoscope for an awake intubation and to suggest alternative uses of these devices in other awake intubation scenarios. The case report presented offers

Airway management in the patient with potential cervical spine instability: continuing professional development.

PURPOSE: Securing the airway of a patient with a potentially unstable cervical spine (C-spine) is a complex and challenging task. The objective of this continuing professional development module is to review the current knowledge essential for airway management in the face of potential C-spine instability and, at the same time, to underline areas of uncertainty and limitations in the literature. PRINCIPAL FINDINGS: In low-risk patients-defined by strict criteria derived from large multicentre studies-the C-spine can be considered stable or "cleared" without imaging. In all other patients, at

Nonresolving pneumonia.

A 58-year-old man admitted for evaluation of weight loss and dry cough. Initially, he was diagnosed with community-acquired pneumonia. Despite receiving antibiotics, his clinical status deteriorated and was intubated. Fiber optic bronchoscopy revealed a significant amount of mucopurulent secretions. Bronchoalveolar lavage showed marked amount of sulfur granules identified on stain. Microbiology culture was compatible with Actinomyces israelii. Pulmonary actinomycosis is a rare but important and challenging diagnosis to make. It presents with wide spectrum of clinical and radiologic characteris

[Use of intubating laryngeal mask airway in combination with fiberoptic intubation in a patient with morbid obesity and unexpected lingual tonsillar hypertrophy].

Lingual tonsillar hyperplasia is rare, but may cause difficult or impossible tracheal intubation. We administered anesthesia to a female patient with a body mass index (BMI) of 47 kg x m(-2) with unexpected lingual tonsillar hyperplasia. A 32-year-old woman was scheduled for surgery to repair a ventral hernia under general anesthesia. After inducting anesthesia, three anesthesiologists were needed to ventilate via a facemask. At direct laryngoscopy, after achieving muscular relaxation, the arytenoids and epiglottis could not be identified because of markedly hypertrophied tissue. Next, we atte

[Combined use of the airway scope and fiberoptic bronchoscopy for tracheal intubation in a patient with a large epiglottic cyst].

A 26-year-old man was scheduled for surgical extraction of a large epiglottic cyst. Mask ventilation was possible under propofol anesthesia without muscle relaxant. It was difficult to see the glottis using either a Macintosh laryngoscope or by fiberoptic bronchoscopy. When the AWS laryngoscope (Hoya, Tokyo Japan) with a part of the blade removed, was inserted orally, it became possible to see the glottis with a part of the epiglottic cyst. A reinforced tube was inserted nasally, and a fiberoptic bronchoscope was passed through the tube into the trachea. The tube was then passed over the fiber

A grading scale for pediatric vocal fold nodules.

OBJECTIVE: To design a grading scale for vocal fold nodules in pediatric patients. STUDY DESIGN AND SETTING: We conducted a prospective study in which a grading scale for vocal nodule size and contour based on static fiberoptic images of pediatric larynges was developed to achieve the scale presented here. RESULTS: Twenty-eight health care professionals each rated 28 images of pediatric vocal fold nodules. The intraclass correlation for nodule size was strong (0.77; 95% confidence interval, 0.67-0.87). The kappa statistic for nodule contour was mild (0.35; 95% confidence interval, 0.33-0.37)

Flexible fiberoptic laryngoscopy in children: effect of sitting versus supine position.

Currently no standard exists as to the exact technique of flexible fiberoptic laryngoscopy (FFL) for children. Our objective was to determine the effects of examination in the sitting versus supine position on upper airway findings during FFL in children using a standardized technique and a grading system. In this prospective study, each child acted as his or her own control. Thirty children underwent FFL at a tertiary care children's hospital (mean age 4.5 years). FFL was performed during spontaneous ventilation in both positions, using a standardized sedation technique administered by a sing

[Unusual foreign body aspiration].

Tracheobronchial foreign body aspiration in adults is rare when compared to children. In this case report, 76 year old female patient who aspirated a screw of tracheostomy cannula which was taken out by fiberoptic bronchoscopy (FOB) was presented. This case showed us that education of patients about stoma care is important. The FOB can be used safely to get foreign body from distal airways.

Charcot-Marie-Tooth Disease type 1 and pediatric true vocal fold paralysis.

A case study of a child with Charcot-Marie-Tooth type 1 (CMT1) that presented with bilateral vocal fold paralysis. This is the first case of bilateral vocal fold paralysis in a child with CMT1 and it is the first case to be managed endoscopically. The surgical decision making process is discussed, and in particular the role of fiberoptic endoscopic evaluation of swallowing (FEES) in determining what surgical options should be entertained. In children with bilateral vocal fold paralysis who also have other neurologic abnormalities, the clinician should consider the possibility of CMT as the cau

Failure to detect an unusual obstruction in a reinforced endotracheal tube with fiberoptic examination.

Obstruction of an endotracheal tube (ETT) is a potentially life-threatening event. We report an unusual obstruction of a reinforced ETT. The valve-like obstruction was caused by a partial detachment of the inner coating from the embedded spiral of the ETT. It led to an increase in inspiratory airway pressure, failure to detect end-expiratory CO(2), and generated a wheezing sound in forced expiration. Fiberoptic inspection, which is a recommended procedure for a suspected ETT-obstruction, failed to identify this detachment. Exchanging the defective ETT immediately resolved the critical clinical

[Placement of a double-lumen tube using a 6 mm diameter fibro-bronchoscope and a Cook exchange catheter in a patient with unforeseen tracheal intubation difficulty].

The trachea of a 74-year-old man undergoing left pneumonectomy could not be intubated in two attempts after induction of anesthesia with midazolam, fentanyl, propofol and rocuronium. Difficult intubation had not been foreseen, but inspection through the laryngoscope revealed Cormack and Lehane grade IV conditions. Because a small-caliber fiberoptic bronchoscope was unavailable for intubation with a double-lumen endobronchial tube, we inserted a No. 9 orotracheal tube with a 6 mm bronchoscope as far as the left main bronchus. Intubation was aided by a universal adaptor for fiberoptics with a fa

Intraoperative fibreoptic bronchoscopy during neonatal tracheo-oesophageal fistula ligation and oesophageal atresia repair.

Maintenance of adequate ventilation under anaesthesia can be difficult during identification and ligation of congenital tracheo-oesophageal fistula with repair of oesophageal atresia. Anaesthesia may also be complicated by problems associated with prematurity, pre-existing aspiration pneumonitis, and difficulty positioning the endotracheal tube to prevent inflation of the stomach with increased risk of aspiration and diaphragmatic splinting. Even intubation of the fistula and gastric rupture may occur. Two neonatal cases are presented where use of a 2.2 mm neonatal bronchoscope passed through

A comparison of rigid -v- video thoracoscopy in the management of chest trauma.

Between December 1, 1994 and April 1,1998, 44 thoracoscopic procedures were performed in 42 patients following chest injuries. Indications included exploration in 15, retained haemothorax in 10, continued bleeding after chest tube placement in 3, air leak in 5 and empyema in 11. Video thoracoscopy was used in 24 cases and rigid thoracoscopy in 20, including 14 patients in whom video thoracoscopy was contraindicated. There was no difference in the operative times, length of stay or incidence of complications. Two formal and 3 "mini" thoracotomies were used in the video thoracoscopy group. Three

[Diagnosis and removal of foreign bodies of the trachea and bronchi].

The article presents results of examination and treatment of patients with foreign bodies in the tracheobronchial tree. It was noted that clinical manifestations were not leading in diagnosis of aspirated foreign bodies. The X-ray diagnosis is also not sufficiently effective, especially in cases with organic foreign bodies. Fiber bronchoscopy was used in 45 patients for diagnosis and removal of foreign bodies from the tracheobronchial tree. Practical recommendations are given on the method of removal of the foreign bodies depending on the shape, size and kind of the foreign body. The authors c

Placement of left double-lumen endobronchial tubes with or without a stylet.

PURPOSE: This study was designed to determine if leaving a stylet in the left Bronch-Cath endobronchial tube (DLT) for the entire intubating procedure improves the accuracy of placement on the initial attempt, without introducing complications. METHODS: Sixty ASA 1-3 patients were randomized to one of two groups. In Group 1 (n = 30), the stylet was retained for the entire intubation procedure and in Group 2 (n = 30), the stylet was removed once the bronchial cuff had passed the vocal cords. In both groups, the DLT was turned 110 degrees counterclockwise and advanced until resistance was encou
