Many processes lead to anemia. This review covers anemias that are less commonly encountered in the United States. These anemias include hemoglobin defects like thalassemia, bone marrow failure syndromes like aplastic anemia and pure red cell aplasia, and hemolytic processes such as paroxysmal nocturnal hemoglobinuria. The pathogenesis, diagnostic workup, and treatment of these rare anemias are reviewed.

Multiple placental chorioangiomas in a 31-year-old woman led to a severe, acute fetal-to-maternal transfusion and maternal hemoglobinuria from hemolysis of the fetal red blood cells. Fetal distress was manifest by a sine wave fetal monitor pattern. A severely asphyxiated infant was delivered by cesarean section. The infant did well after transfusion therapy.

Case report of a 39-year-old woman treated by Danazol for a paroxystic nocturnal hemoglobinuria who developed benign intracranial hypertension and sclerosing cholangitis. Bilateral papilloedema cleared 4 weeks after Danazol was stopped. Twelve similar cases have already been reported in the literature. Danazol should be added to the list of drugs potentially inducing pseudo-tumor cerebri.

The understanding of the clinical manifestations in paroxysmal nocturnal haemoglobinuria (PNH) has made great progress. The main symptoms of this disease such as abdominal pain, renal failure or pulmonary hypertension and even the basis of the dramatic thrombophilia can be related to intravascular haemolysis and liberation of free haemoglobin resulting in NO depletion. In addition, there has been a recent great progress in elucidating the pathophysiology of clonal expansion within PNH bone marrow. In the majority of patients with haemolytic PNH, there are additional mutations within genes beyo

A 28-year-old woman who was undergoing treatment with eculizumab for paroxysmal nocturnal hemoglobinuria presented to the hospital with fevers, chills, headache, and a swollen left index finger. Blood cultures returned positive for Neisseria gonorrhoeae. We report the second case of disseminated gonococcal infection associated with the use of eculizumab.

Paroxysmal nocturnal haemoglobinuria (PNH) is a unique disorder characterised by the triad of intravascular haemolysis, thrombosis and bone marrow failure. In the early seventies it was shown that PNH is a clonal disease; and in the nineties the molecular basis of the PNH abnormality was elucidated. However, what makes a PNH clone expand is still not known. Here, we suggest that this is due to somatic cell selection, resulting from the presence in the patient of autoreactive T cells that target glycosylphosphatidylinositol (GPI) in the context of an MHC-like molecule on the surface of haemopoi

Paroxysmal nocturnal haemoglobinuria (PNH) is a disease entity that presents with intravascular haemolysis and an increased tendency for venous thrombosis. In recent years there has been a major breakthrough in our understanding of the pathogenesis of PNH. Most of the different symptoms can be tracked down to the deficiency of glycophosphoinositol (GPI)-anchored proteins in cell lines deriving from a single haematopoietic stem cell. This deficiency is caused by a mutation in the X-chromosomal PIG-A gene whose product, a glycosyltransferase, participates in the first step of the GPI-anchor bios
