Acute traumatic patellar dislocation.

Inaugural traumatic patellar dislocation is most often due to trauma sustained during physical or sports activity. Two-thirds of acute patellar dislocations occur in young active patients (less than 20 years old). Non-contact knee sprain in flexion and valgus is the leading mechanism in patellar dislocation, accounting for as many as 93% of all cases. The strong displacement of the patella tears the medial stabilizing structures, and notably the medial patellofemoral ligament (MPFL), which is almost always injured in acute patellar dislocation, most frequently at its femoral attachment. Latera

[Arthroscopically assisted treatment for acute patellar dislocation].

OBJECTIVE: To evaluate an improving operative procedure and the clinical results of arthroscopically assisted treatment for acute patellar dislocation. METHODS: Between April 2006 and March 2009, 22 patients (25 knees) with primary acute complete dislocation of the patella underwent an improving arthroscopic operation, release of lateral retinaculum and suture of medial capsule and retinaculum structure. There were 5 males and 17 females with an average age of 23.6 years (range, 14-34 years). Three patients had bilateral procedure. Eleven left knees and 14 right knees were involved. The disea

[Posterior dislocation of the hip and posterior cruciate ligament avulsion in an 8-year-old boy].

Traumatic hip dislocation is a relatively rare entity in children. Posterior cruciate ligament (PCL) avulsion is also very rare in children and only a few cases have been reported. Because of the mechanism of injury, coexistent PCL avulsion with posterior dislocation of the hip is frequently seen in adults. This coexistence, however, has not hitherto been reported in children. We present an eight-year-old boy with traumatic posterior dislocation of hip and PCL avulsion. He was brought to hospital after a car accident in which his knee hit to the front part of the car when he was driving a bicy

Indications of Arthroscopy in the Treatment of Patellar Instability in Children.

Purpose Understanding the relevant risk factors for patellar instability and the clinical and radiographic tests necessary to determine optimal treatment. This case series intends to evaluate arthroscopic indications in the treatment of patellar instability in children. Methods From 2013 to 2021, 33 patients (seven to 16 years of age) with 35 knees sustaining first-time dislocation with loose bodies, recurrent dislocation or subluxation, and habitual dislocation were arthroscopically operated on according to the flow chart. Periods of follow-up were two to 10 years (avg. 5.5 years). Follow-up

The Effect of Patellar Positioning on Femoral Component Rotation when Performing Flexion Gap Balancing Using a Tensioning Device for Total Knee Arthroplasty.

There is an increasing interest in new devices such as tensiometers for flexion gap balancing during total knee arthroplasty (TKA). The purpose of this study was to determine the influence of patella positioning during flexion gap balancing on femoral component rotation. We prospectively evaluated 32 consecutive knees in 31 patients who underwent primary TKA for degenerative osteoarthritis and where soft tissue balancing was performed using the same tensiometer. Preoperative measurements included valgus/varus deformation, mechanical axis, epicondylar axis, and tibial slope. Intraoperatively, m

Patella re-alignment in children with a modified Grammont technique.

BACKGROUND AND PURPOSE: In skeletally immature patients, surgical options due to recurrent patella dislocation are limited, because bony procedures bear the risk of growth disturbances. In this retrospective study, we report the long-term functional and radiographic outcome in skeletally immature patients using the modified Grammont surgical technique. PATIENTS: Between 1999 and 2004, 65 skeletally immature knees (49 children) were treated with a modified Grammont procedure: an open lateral release and a shift of the patella tendon insertion below the growth plate on the tuberositas tibia, al

Prospective long-term results of operative treatment in primary dislocation of the patella.

Among 53 prospectively studied patients who had undergone operative treatment of primary patellar dislocation, 9 (17%) suffered recurrence within an average follow-up of 6.5 years. All the redislocations occurred in female patients. The patients with recurrence also suffered first-time patellar dislocation in their unoperated knee during the follow-up significantly more often (P < 0.01) than the patients with stable knees. Patients with redislocations were more often dissatisfied with their outcome than the stable group, mainly because of annoying anterior knee pain (P < 0.05). Preoperatively

Empty Sunrise Sign: An Arthroscopic Finding in Recurrent Patellar Dislocation.

Patellar dislocation is a relatively common knee injury and can be challenging to diagnose. We describe an arthroscopic finding we noted in recurrent patellar dislocation. Arthroscopic photos were evaluated from 50 knees with a history patellar dislocation and 100 control patients. We quantified the amount of patellar subluxation seen on a single arthroscopic view and coined it Empty Sunrise Sign if the patella did not overlap the trochlea. Empty Sunrise Sign was found in 82% of 50 cases with recurrent patellar dislocation and none of the 100 control cases. The finding was 100% specific for id

Associations Between Patient Characteristics and Cartilage T1ρ Relaxation Times Vary Over Time Following Patellar Dislocation.

The characteristics of patients that promote progressive cartilage degradation following patellar dislocations are not well understood. The study hypothesis is that associations relating cartilage degradation to low BMI, high activity and youth in the early months following patellar dislocation are not exhibited for contralateral knees or a longer time from injury. Quantitative MRI-based evaluation of injured and contralateral knees occurred within 5 months of a unilateral patellar dislocation (baseline evaluation, 36 subjects) and beyond 5 months (follow-up evaluation, 20 subjects). Linear re

Trochleaplasty for recurrent patellar dislocation in association with trochlear dysplasia. A 4- to 14-year follow-up study.

We investigated the clinical and radiological outcome of trochleaplasty for recurrent patellar dislocation in association with trochlear dysplasia in 38 consecutive patients (45 knees) with a mean follow-up of 8.3 years (4 to 14). None had recurrence of dislocation after trochleaplasty. Post-operatively, patellofemoral pain, present pre-operatively in only 35 knees, became worse in 15 (33.4%), remained unchanged in four (8.8%) and improved in 22 (49%). Four knees which had no pain pre-operatively (8.8%) continued to have no pain. A total of 33 knees were available for radiological assessment.

The extensor mechanism of the knee.

Disorders related to the knee extensor mechanism are common and rarely require imaging. Non specific anterior knee pain, fracture, dislocation, overuse tendinopathy and chronic patellofemoral instability are the commonest conditions encountered. Imaging is used in acute trauma, and for the assessment of cases of anterior knee pain resistant to conservative measures. The role of the radiograph is now largely restricted to cases of suspected fracture. Ultrasound is the optimum technique for suspected tendon and bursal pathology and MRI is widely used for the assessment of dysplasia and instabili

Medial dislocation of the patella.

Medial dislocation of the patella is a previously unreported entity. This disorder can be disabling to the patient and may require a hospital visit for reduction. Three cases are presented in this article in which computed tomography demonstrated the dislocation. All three patients had undergone a lateral retinacular release to the involved knee for treatment of chronic knee pain or recurrent lateral patellar subluxation.

[Arthroscopic reconstruction of medial patellofemoral ligament with hamstring tendon autografts for treatment of recurrent patellar dislocation].

OBJECTIVE: To investigate the effectiveness of reconstructing medial patellofemoral ligament with hamstring tendon autografts for the treatment of recurrent patellar dislocation under arthroscopy. METHODS: Between January 2005 and January 2010, 22 cases of recurrent patellar dislocation were treated by lateral retinacular release and reconstruction of the medial patellofemoral ligament with hamstring tendon autografts under arthroscopy. There were 5 males and 17 females, aged 15-19 years (mean, 17.3 years). The average number of dislocation was 4 (range, 3-8). The main clinical symptoms were

Patellar tendon length--the factor in patellar instability?

Patellar tendon length has been compared in 42 knees with a history of patellar dislocation, and 51 control knees. A lateral X-ray and a magnetic resonance image (MRI) were taken of each knee. The mean radiological patellar tendon length was 46 mm in the controls and 53 mm in the dislocation group. From MRI images, the mean was 44 mm in controls and 52 mm in the dislocation group. This means that the patellar tendon is significantly (P<0.0001) longer in patients with a history of patellar dislocation on both MRI and X-ray. There is no significant difference (P=0.52) between X-ray and MRI measu

The influence of directional preference on lateral patellar dislocation: a case report.

BACKGROUND: There is little consensus on the conservative management of lateral patellar dislocations (LPD). Mechanical diagnosis and therapy (MDT) is an established classification system in the spinal and extremity population. This case report describes the use of MDT in the management and classification of a patient with LPD. CASE DESCRIPTION: The patient was a 20-year-old female with a 3-month history of left knee pain precipitated by a lateral patellar dislocation. The patient described pain and a feeling of instability with standing and walking and limitations in work and recreational ac
