Ectopia lentis continues to be a therapeutic challenge for ophthalmologists. It can occur as an isolated condition, after ocular trauma, in association with other ocular disorders, as part of a systemic mesodermal disease or as a complication of general metabolic disorders. Minimal subluxation of the lens may cause no visual symptoms, but in more advanced cases serious optical disturbances arise. The most important is amblyopia. Surgical treatment options include iris manipulation, lens decision, aspiration, intracapsular or extracapsular extraction, and pars plana lensectomy. The choice of su

PURPOSE: To report a case of a giant iris cyst with corneal perforation.
CASE REPORT: A 43-year-old woman presented with repeated episodes of pain and decreased vision in her left eye for 3 years. There was no history of trauma. Slit lamp examination showed a nasal limbal corneal perforation and a mass in the anterior chamber. Based on clinical examination, B-scan ultrasonography, and anterior segment optical coherence tomography, a presumed diagnosis of anteriorly subluxated lens was made. However, intraoperatively, the patient's own crystalline lens was found behind the anterior chamber mas

BACKGROUND: Traboulsi syndrome is a sporadic autosomal recessive disorder. Very few cases reported and no surgical treatment has been described.
PURPOSE: The purpose of the study was to describe the surgical treatment of a case of Traboulsi syndrome with intercalary staphyloma.
SYNOPSIS: Traboulsi syndrome[1-3] is a type of spontaneous filtering bleb. The case was of a 31-year-old lady with a flat anterior chamber and large intercalary staphyloma, with a subluxated clear lens. Liberal peritomy was done. The staphyloma was cut and the displaced lens was delivered through it. It was closed wit

Aniridia is a congenital eye disorder with a variable degree of hypoplasia or absence of iris tissue. It is caused by loss of function of the PAX6 gene and may be an isolated ocular abnormality or part of a syndrome. WAGRO refers to a rare genetic condition leading to Wilms tumor, aniridia, genitourinary anomalies, mental retardation, and obesity and is caused by a deletion of the short arm of chromosome 11 (11p), where the PAX6 gene is located. Here, we report on an 8-year-old boy with aniridia, polar cataract, and lens subluxation along with neuropsychomotor and speech delays. Karyotype eval…

PURPOSE: To examine the usefulness of 23-gauge pars plana vitrectomy (PPV) in removing retained lens fragments and to determine which cases require 20-gauge ultrasonic fragmentation.
DESIGN: Retrospective comparative case series.
METHODS: Twenty eyes in 20 consecutive patients who had 23-gauge PPV for retained lens fragments after complicated cataract surgery at Wills Eye Institute were retrospectively reviewed. Retained lens fragments were graded based on percentage of total nuclear size. Every surgical case started with 23-gauge PPV, but 1 sclerotomy was enlarged to facilitate 20-gauge ult

PURPOSE: To evaluate results of surgical complex treatment of lens dislocation in Marfan syndrome cases.
MATERIAL AND METHODS: Nineteen eyes of 13 patients were operatively treated for lens dislocation. Pars plana vitrectomy was done in all eyes. Dislocated lenses were removed by lensectomy in 5 eyes and with an intracapsular method in 14 eyes. The scleral fixation technique was used for primary posterior chamber intraocular lens implantation in 18 eyes, and one eye remained aphakic. Time of observation ranged between 6 and 71 months (mean 3.5 years).
RESULTS: All eyes achieved improvement i

The objective was to describe a new surgical technique to manage a posteriorly dislocated crystalline lens. Four patients with posteriorly dislocated lenses were studied. Two patients had dislocated lenses secondary to trauma, 1 had undergone retinal detachment surgery, and 1 had an idiopathic lens dislocation. Pars plana vitrectomy was carried out on all 4 eyes, followed by an injection of perfluoro-n-octane to float the lens off the retina. The lens was phacoemulsified through a limbal incision, and an intraocular lens was positioned in the ciliary sulcus with suture fixation. Perfluoro-n-oc

A moderately retarded institutionalized patient diagnosed with Sturge-Weber syndrome and bilateral retinitis pigmentosa underwent enucleation OD because of intractable pain secondary to glaucoma in 1965. Twenty-three years later, the patient had a dislocated lens OS. Although the lens dislocation might have been caused by trauma, it also might be associated with either Sturge-Weber syndrome or retinitis pigmentosa. The role of protective eye wear in one-eyed institutionalized retarded patients is discussed. To our knowledge, this combination of Sturge-Weber syndrome, bilateral retinitis pigmen

Pars plana lensectomy by ultrasonic fragmentation appears to be the treatment of choice for extraction of subluxated and dislocated lenses. The closed system technique allowed extraction of 11/11 subluxated lenses without operative or postoperative complications with an average observation period of 4.5 years. The technique was employed in four traumatically dislocated lenses in eyes with accompanying traumatic pathology requiring additional surgical procedures, i.e., cryoretinopexy and scleral buckling. There were no complications in 4/4 eyes with an average observation period of 1.5 years. P

CASES REPORT: We report the cases of 2 women with a pigmented tumour in the ciliary body, one a melanocytoma and the other a melanoma, with different clinical manifestations. The first one presented with decreased visual acuity associated with recent growth of the tumour, as well as sectorial opacities of the lens and subluxation. The second one is asymptomatic and has been kept under observation for more than 30 years.
DISCUSSION: Although the definitive diagnosis of a pigmented tumour of the ciliary body is only achieved by the histopathology study, the group of clinical features is a deter

The threat of vision loss increases with age owing to a combination of the accumulation of untreated or partially managed eye disease (for example, immune-mediated keratoconjunctivitis) as well as the late-in-life onset of certain ocular disorders such as lens subluxation/luxation syndrome and senile/age-related cataracts. In addition, the ophthalmic effects of age-related systemic diseases such as systemic hypertension can be vision threatening as well. The veterinarian must always weigh the potential ophthalmic benefits of contemporary therapy against the potential patient risk, not only whe…

We report a modified technique of direct scleral fixation of a posterior chamber intraocular lens (PC-IOL) using a specially designed intraocular needle-holder. The procedure was carried out in three eyes with posttraumatic subluxated cataracts; one with aphakic bullous keratopathy and with an accidental posterior capsular rent, and one with a superior zonulodialysis. The needle-holder greatly facilitated the procedure, enabling us to place the PC-IOL precisely in the ciliary sulcus with no subsequent decentration. This method avoids iris fixation and provides more secure scleral fixation of t
