Epithelioid sarcoma presenting as pulmonary cysts with cancer antigen 125 expression.

A 39-year-old Japanese woman presented with a swollen right hand and a right-sided pneumothorax. Chest CT revealed bilateral multiple pulmonary thin-walled cysts measuring<or=1 cm in diameter and small nodules. An initial skin biopsy led to a misdiagnosis of metastatic adenocarcinoma, as tumour cells were positive for cytokeratin, epithelial membrane antigen, carcinoembryonic antigen and cancer antigen 125. However, chemotherapy proved ineffective, and the skin biopsy was repeated. A final diagnosis of epithelioid sarcoma (ES) was made. Open lung biopsy showed that the pulmonary nodules repres

Rheumatoid arthritis associated recurrent pleural effusion.

Pleural effusions and pulmonary nodules are known complications of rheumatoid arthritis (RA) though pleural extra-articular manifestations without clinical arthritis are rare. We present a 66-year-old male with dyspnea and weight loss, whose imaging revealed pleural effusions and lung nodules, multiple exudative pleural effusions, and a thoracoscopic biopsy showing chronic pleuritis and granulomatous inflammation. Subsequent joint stiffness and positive rheumatoid markers led to a RA diagnosis. Patient received prednisone and methotrexate, leading to symptomatic improvement but with recurrent

[Six cases of primary pulmonary cryptococcosis].

Six patients with asymptomatic primary pulmonary Cryptococcosis are reported. In all of the patients, the disease was detected by annual chest X-ray during mass screening for lung cancer or during follow-up for pulmonary tuberculosis or gastric cancer. The chest X-ray findings consisted of a solitary pulmonary nodule in 4 patients and multiple pulmonary nodules in 2. Only one patient who could not be histologically diagnosed by bronchofiberscopy underwent surgical resection. However, the other 5 patients were histologically diagnosed by transbronchial biopsy with bronchofiberscopy. They were t…

Coexistence of Sjögren's syndrome and pulmonary nodular amyloidosis.

There are very few cases in the literature on the coexistence of Sjögren's syndrome and pulmonary nodular amyloidosis being treated with rituximab. When nodules with central calcification and cystic lesions are seen on computed tomography, amyloid lung should be considered. Biopsy is recommended as it can be confused with malignancies. In this article, we present a 66-year-old female patient who has been followed up for Sjögren's syndrome for 26 years. Multiple cystic lesions with central calcification in the lung were detected and it was evaluated as amyloid nodule in the biopsy performed. Th

Uterine carcinosarcoma metastatic to the lung as large-cell neuroendocrine carcinoma with synchronous sarcoid granulomatosis.

A 58-year-old woman underwent a curative total abdominal hysterectomy with bilateral salpingo-oophorectomy for a uterine carcinosarcoma. Fifteen months later, PET and CT scan revealed three pulmonary nodules and multiple lymphadenopathy. An atypical lung segmentectomy with dissection of the regional lymph nodes was performed. Two of the three nodules showed morphological and immunohistochemical profiles consistent with the diagnosis of large-cell neuroendocrine carcinoma (LCNEC). The third nodule, the surrounding pulmonary parenchyma and lymph nodes showed diffuse sarcoid granulomatosis. Re-re

Multiple diagnostic task performance in CT examination of the chest.

OBJECTIVES: In three experiments, we studied the detection of multiple abnormality types using the satisfaction of search (SOS) paradigm, the provision of a computer-aided detection (CAD) of pulmonary nodules and a focused nodule detection task. METHODS: 51 chest CT examinations (24 that demonstrated subtle pulmonary nodules and 27 that demonstrated no pulmonary nodules) were read by 15 radiology residents and fellows under two experimental conditions: (1) when there were no other abnormalities present except test abnormalities in the exams (non-SOS condition), and (2) when other abnormalitie

Case report: multiple lesions during navigation bronchoscopy; seen one, seen them all?

BACKGROUND: Peripheral pulmonary nodules are often detected as multiple nodules in one patient. Computed tomography (CT) guided transthoracic biopsy (TTNB) is the most widely implemented method for minimal invasive biopsy of pulmonary nodules, but generally only one nodule is sampled per procedure. Navigation bronchoscopy is an endobronchial procedure with very low complication rates, and uses high-end image guidance which allows for the sampling of multiple nodules in one session, while also allowing inspection of the central airways and endobronchial ultrasound (EBUS) guided staging in one s

Volume and location of screen-detected lung nodules associated with lung cancer within two-year follow-up: Post hoc analysis from the UK Lung Cancer Screening (UKLS) trial.

INTRODUCTION: Follow-up of radiologically suspicious lung nodules from screening programs remains complex and unstandardized. This study analyses distribution and characteristics of solid and part-solid non-calcified nodules ≥100 mm3 and non-solid nodules ≥8 mm detected at baseline in the UK Lung cancer Screening (UKLS) trial, with insights into their relationship to screen-detected lung cancers. METHODS: UKLS participants with baseline indeterminate and positive lung nodules were included. Nodule location was categorized by lobe and by attachment: intraparenchymal, juxta pleural or pleural-b

Tool in lesion verification of shape-sensing robotic-assisted bronchoscopy with cone beam CT in sampling peripheral pulmonary nodules.

INTRODUCTION: Our aim was to evaluate the diagnostic and safety performance of shape-sensing robotic-assisted bronchoscopy (ssRAB) with cone beam CT (CBCT) for the biopsy of pulmonary nodules. Additional analysis was performed to assess outcomes for small nodules and those close to the fissure, pleura or mediastinum. METHODS: This single arm, multicentre prospective study enrolled 200 subjects with suspicious pulmonary nodules. Each subject underwent a ssRAB procedure with CBCT and was subsequently followed up. The primary outcome was tool-in-lesion (TIL) confirmed with CBCT. Further endpoint

Effect of ultrasound-guided erector spinae plane block for preoperative CT-guided percutaneous localization of pulmonary nodules: a randomized controlled trial.

BACKGROUND: The increasing detection of pulmonary nodules through low-dose computed tomography screening has necessitated precise preoperative localization techniques. While local anesthesia is routinely used for CT-guided pulmonary nodule localization, it inadequately addresses deep tissue pain during pleural penetration. This study is to evaluate the analgesic efficacy and safety of ultrasound-guided erector spinae plane block (ESPB) during CT-guided pulmonary nodule localization. METHODS: This single-center, prospective, randomized controlled trial was conducted from June 2024 to January 2

Radiomic Feature Stability in Low-Dose CT and 3D Segmentation for Pulmonary Nodules: Phantom and Multicenter Evaluation.

Purpose To identify a stable radiomic feature (RF) subset for pulmonary nodules by analyzing test-retest and acquisition parameter variability using phantoms and to evaluate its clinical utility. Materials and Methods CT scanners at multiple centers were used to scan a chest phantom embedded with artificial pulmonary nodules under varied doses and scanning and reconstruction parameters. An artificial intelligence-based, fully automated three-dimensional segmentation method was used. Three sources of variability were examined: test-retest, inter-CT scanner, and intra-CT scanner. RF stability wa

Pulmonary Nodules.

Pulmonary nodules are classified as solid or subsolid, with subsolid nodules further classified as part-solid or pure ground-glass. Management strategies differ according to the type of nodule. A comparison of current and previous imaging studies, when available, is essential to assess the risk of the nodule being malignant. Solid nodules that have been stable for 2 years are considered to be benign, whereas subsolid nodules require a longer period of stability to be considered benign. Prediction models to stratify the risk of the nodule being malignant can be used to guide the management of s

Cone-Beam Computed Tomography-Guided Electromagnetic Navigation for Peripheral Lung Nodules.

BACKGROUND: Electromagnetic navigation bronchoscopy (ENB) is a minimally invasive technology for the diagnosis of peripheral pulmonary nodules. However, ENB is limited by the lack of real-time confirmation of various biopsy devices. Cone-beam computed tomography (CBCT) could increase diagnostic yield by allowing real-time confirmation to overcome the inherent divergence of nodule location. OBJECTIVES: The aim of this study was to assess the diagnostic yield of ENB plus CBCT as compared with ENB alone for biopsy of peripheral lung nodules. METHOD: We conducted a retrospective study of patient

Pulmonary Benign Metastasizing Leiomyoma in a Patient With a Concurrent Breast Tumor: A Diagnostic Challenge.

Pulmonary benign metastasizing leiomyoma (PBML) is a rare condition in which histologically benign smooth muscle tumors occur in distant organs, most often the lungs. Although typically discovered incidentally, differentiating BML from metastatic malignancy can be challenging in real-world clinical settings. A 53-year-old woman with a history of hysterectomy for uterine leiomyoma presented with multiple pulmonary nodules and a cystic left breast lesion containing an irregular solid component. Fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography demonstrated no signif…
