Validating a functional near-infrared spectroscopy diagnostic paradigm for Major Depressive Disorder.

Reduced haemodynamic response in the frontotemporal cortices of patients with major depressive disorder (MDD) has been demonstrated using functional near-infrared spectroscopy (fNIRS). Most notably, changes in cortical oxy-haemoglobin during a Japanese phonetic fluency task can differentiate psychiatric patients from healthy controls (HC). However, this paradigm has not been validated in the English language. Therefore, the present work aimed to distinguish patients with MDD from HCs, using haemodynamic response measured during an English letter fluency task. One hundred and five HCs and 105 p

Potential Roles of G Protein-Coupled Receptor 30 (GPR30) in Migraine Pathophysiology.

Migraine is a highly prevalent and disabling neurological disorder with multiple hypotheses regarding its pathogenesis. G protein-coupled receptor 30 (GPR30), a member of the G protein-coupled receptor family, was once considered a membrane estrogen receptor. GPR30 is highly expressed in migraine-associated core tissues such as the trigeminal ganglion and exerts an indirect regulatory effect on the pathophysiological processes of migraine, yet there remain controversies over its specific molecular mechanisms and whether it is involved in estrogen-mediated regulation. This review systematically

Erectile Dysfunction: A Review on Prevalence, Perceptions, Diagnosis and Management in India.

Erectile dysfunction (ED) is defined as the inability to achieve or maintain penile erection sufficient to permit satisfactory sexual activity. The prevalence increases with age. Basic and clinical research is identifying the neurovascular and humoral control of the mechanisms. The initial evaluation should differentiate erectile dysfunction from premature ejaculation and loss of libido. Myocardial insufficiency, hypogonadism and peripheral neuropathy should be looked for. Initial laboratory investigations should be restricted to identifying previously undetected medical illness that may direc

Feasibility of a novel multi-device robotic system for multiple neurointerventional procedures: pre-clinical validation in porcine models.

While the application of a robotic system in neurointerventional procedures has gained notable attention, the current robotic assistance is unsatisfactory. Therefore, we developed a novel robotic system, which was capable of simultaneous coaxial control of three catheters and one guidewire, coupled with rapid-exchange functionality. To evaluate the feasibility of the multi-device robotic system for diagnostic angiography, carotid artery stenting (CAS), vertebral artery (VA) stenting, flow diverter (FD) deployment and aneurysm coiling through preclinical study. The robotic system was tested for

Roles and therapeutic prospects of the laminin family in disorders of the nervous system.

BACKGROUND: Neurological disorders comprise a heterogeneous spectrum-including cerebrovascular disease, neurodegeneration, autoimmune demyelination, neuropsychiatric conditions, and trauma-whose incidence and prevalence are rising with population aging, creating sustained healthcare and socioeconomic burdens. Recent work highlights laminin, a major non-collagenous BM glycoprotein, as a key regulator of central nervous system structure and function. As a core extracellular matrix scaffold and signaling hub, laminin orchestrates matrix remodeling, barrier disruption/repair, neuroinflammation, an

Immune-Metabolic Interactions in the Degenerative Retina: A Systems Biology Approach Based on Diabetic Retinopathy.

Diabetic retinopathy (DR) is increasingly recognized as a complex neurovascular degenerative disorder driven by intertwined immune and metabolic disturbances within the retinal microenvironment. Chronic hyperglycemia induces metabolic stress, mitochondrial dysfunction, and oxidative imbalance, which, in turn, activate innate and adaptive immune pathways. Key mechanisms-including complement dysregulation, microglial activation, leukostasis, cytokine and chemokine signaling, and advanced glycation end-product-mediated inflammation-contribute to endothelial injury, barrier breakdown, and progress

Combined heat and mental stress alters neurovascular control in humans.

This study examined the effect of combined heat and mental stress on neurovascular control. We hypothesized that muscle sympathetic nerve activity (MSNA) and forearm vascular responses to mental stress would be augmented during heat stress. Thirteen subjects performed 5 min of mental stress during normothermia (Tcore; 37 ± 0°C) and heat stress (38 ± 0°C). Heart rate, mean arterial pressure (MAP), MSNA, forearm vascular conductance (FVC; venous occlusion plethysmography), and forearm skin vascular conductance (SkVCf; via laser-Doppler) were analyzed. Heat stress increased heart rate, MSNA, SkVC

Surgical exposures of the humerus.

The neurovascular and muscular anatomy about the humerus precludes the use of a truly "safe" fully extensile approach. Working around a spiraling radial nerve at the posterior midshaft requires either a transmuscular dissection or a triceps-avoiding paramuscular technique. To gain maximal exposure, the radial nerve must be mobilized at the spiral groove. For exposure of only the proximal humeral shaft, many surgeons prefer the anterolateral approach because it uses the internervous plane between the axillary and deltoid nerves proximally and the radial and musculocutaneous nerves distally. Pro

Perfusion-dependent spreading depolarization signatures identify tissue vulnerability in stroke.

Cortical spreading depolarizations (CSDs) are large-scale disruptions of neuronal homeostasis that contribute to secondary injury after ischaemic stroke and occur across a range of acute and chronic neurological disorders. In healthy cortex, CSDs are typically coupled to compensatory vasodilation, whereas in ischaemic tissue they can evoke inverse haemodynamic responses that exacerbate metabolic stress and promote lesion progression, highlighting the need for approaches that can resolve localised electrophysiological and vascular dynamics as a function of evolving tissue metabolic state. Here,

Prospective evaluation of early postoperative male and female sexual function after radical prostatectomy with erectile nerves preservation.

Prostate cancer screening has led to the diagnosis of localized prostate cancer in increasingly young and sexually active men. Accordingly, the impact of cancer treatment on sexual function is gaining more attention. To prospectively evaluate the impact of radical prostatectomy (RP) on male, female and conjugal sexual function. Patients were prospectively assessed by an urologist and a sexologist before and 6 months after robot-assisted laparoscopic RP (RALP). RALP was performed with uni- or bilateral neurovascular bundle preservation by a single surgeon. Postoperatively, all patients were pre

Autonomic control of the vasculature in CKD: leveraging renal denervation.

Chronic kidney disease (CKD) confers disproportionate high risk for cardiovascular morbidity and mortality. Hypertension remains both a driver and consequence of progressive renal dysfunction. Beyond sodium retention, volume expansion, and renin-angiotensin-aldosterone system activation, increasing evidence supports a CKD phenotype of sustained sympathetic activation coupled with impaired autonomic buffering and altered vascular responsiveness. Baroreflex sensitivity is reduced across pre-dialysis and dialysis cohorts and is linked to arterial stiffening and vascular calcification, changes tha

Zebrafish col4a1 loss-of-function models mirror key neurovascular and ocular features of COL4A1/A2 syndrome and enable human variants assessment in vivo.

Pathogenic variants in COL4A1 and COL4A2, encoding type IV collagen α1 and α2 chains-core components of all basement membranes-cause a multisystem disorder with variable expressivity. Affected individuals commonly present with cerebral small vessel disease (cSVD), unmanageable intracerebral haemorrhage (ICH), drug-resistant epilepsy, microphthalmia, and congenital cataract. Severe phenotypes are often linked to glycine substitutions that disrupt α1/α2 heterotrimer assembly, though insertions, deletions, and haploinsufficiency seem to also be pathogenic. Limited insight into collagen IV α1 and

A Potentially Dangerous Industrial Projectile Lodged in the Leg of a Steel Factory Worker.

Penetrating injuries due to fragments energized by an explosive event are life/limb-threatening and are associated with poor clinical and functional outcomes. Penetrating injuries are commonly inflicted in attacks with explosive devices. The extremities, especially the leg, are the most commonly affected body areas, presenting a high risk of infection, slow recovery, and the threat of amputation. This report presents a case of a young factory worker who sustained an injury to the leg with a foreign body lodged near the neuro-vascular bundle. A 44-year-old gentleman sustained a projectile injur

Neurovascular unit on a chip: implications for translational applications.

The blood-brain barrier (BBB) dynamically controls exchange between the brain and the body, but this interaction cannot be studied directly in the intact human brain or sufficiently represented by animal models. Most existing in vitro BBB models do not include neurons and glia with other BBB elements and do not adequately predict drug efficacy and toxicity. Under the National Institutes of Health Microtissue Initiative, we are developing a three-dimensional, multicompartment, organotypic microphysiological system representative of a neurovascular unit of the brain. The neurovascular unit syste

Bright light produces Fos-positive neurons in caudal trigeminal brainstem.

Excessive discomfort after exposure to bright light often occurs after ocular injury and during headache. Although the trigeminal nerve is necessary for light-evoked discomfort, the mechanisms underlying this phenomenon, often referred to generally as photophobia, are not well defined. Quantitative Fos-like immunoreactivity (Fos-LI) was used to determine the pattern of neuronal activation in the caudal brainstem after bright light stimulation and, secondly, whether a neurovascular mechanism within the eye contributes to this response. Under barbiturate anesthesia, male rats were exposed to low
