Congenital diaphragmatic hernia in Minnesota. Impact of antenatal diagnosis on survival.

OBJECTIVE: We characterized the natural history and true mortality of congenital diaphragmatic hernia (CDH) in newborn patients by identifying all infants born with this condition in a fixed geographic region over a 2-year period. We examined this population to determine the frequency of intrauterine diagnosis, the outcome of prenatally diagnosed infants, and the impact of deaths in infants with an unsuspected diagnosis (the "hidden mortality") on the overall outcome of this condition. DESIGN: This was a retrospective population survey of all infants born with CDH in Minnesota between June 19

Nitrosative and oxidative injury to premyelinating oligodendrocytes in periventricular leukomalacia.

Periventricular leukomalacia (PVL), the major substrate of cerebral palsy in survivors of prematurity, is defined as focal periventricular necrosis and diffuse gliosis in immature cerebral white matter. We propose that nitrosative and/or oxidative stress to premyelinating oligodendrocytes complicating cerebral ischemia in the sick premature infant is a key mechanism of injury interfering with maturation of these cells to myelin-producing oligodendrocytes and subsequent myelination. Using immunocytochemical markers in autopsy brain tissue from 17 PVL cases and 28 non-PVL controls, we found in t

French validation of the sleep disturbance scale for children (SDSC) in young children (aged 6 months to 4 years).

UNLABELLED: The sleep disturbance scale for children (SDSC) has been translated and adapted into several languages and its psychometric properties are good. Notably, a French version has been validated for 4- to 16-year olds. The objective of the current study was therefore to adapt and validate the SDSC for French young children (aged 6 months to 4 years). METHOD: 421 French-speaking mothers of children aged between 6 months and 4 years completed the SDSC and the Brief Infant Sleep Questionnaire (BISQ) which is specifically for young children. Of these, 105 children were diagnosed with sleep

Variation in the Phototherapy Practices and Irradiance of Devices in a Major Metropolitan Area.

BACKGROUND: Phototherapy (PT) is widely used to prevent and treat severe hyperbilirubinemia and its associated risks for both acute and chronic bilirubin encephalopathy. Intensive PT, recommended for inpatient treatment of hyperbilirubinemia in term and near-term infants, is defined as having a spectral irradiance of ≥30 μW/cm2/nm. OBJECTIVES: We aimed to assess local PT practices by measuring the irradiance of PT devices in local neonatal intensive care units and newborn nurseries. METHODS: The irradiance footprint, including maximum irradiance at the center of the footprint, of 39 PT devic

A descriptive analysis of gut microbiota composition in differentially reared infant rhesus monkeys (Macaca mulatta) across the first 6 months of life.

The gastrointestinal microbiome is recognized as a critical component in host immune function, physiology, and behavior. Early life experiences that alter diet and social contact also influence these outcomes. Despite the growing number of studies in this area, no studies to date have examined the contribution of early life experiences on the gut microbiome in infants across development. Such studies are important for understanding the biological and environmental factors that contribute to optimal gut microbial colonization and subsequent health. We studied infant rhesus monkeys (Macaca mulat

Normative wideband absorbance measures in healthy neonates in Korea: A preliminary study.

INTRODUCTION: The usefulness of wideband absorbance (WBA) in newborns is well-demonstrated. However, it is still not clear whether there might be a difference according to ethnicity with respect to ambient WBA; therefore, further investigation is necessary to evaluate ethnic-specific normative WBA values in newborns. METHODS: Twenty-one newborns (41 ears) were recruited from the well-baby nursery at a tertiary referral center. All newborn infants who were born at 38 weeks' to 41 weeks' gestation with a normal birth weight (range: 2.5-4.5 kg) and who passed a newborn hearing screening test wit

The impact of architectural design upon the environmental sound and light exposure of neonates who require intensive care: an evaluation of the Boekelheide Neonatal Intensive Care Nursery.

OBJECTIVES: To evaluate the differences in environmental sound, illumination and physiological parameters in the Boekelheide Neonatal Intensive Care Unit (BNICU), which was designed to comply with current recommendations and standards, as compared with a conventional neonatal intensive care unit (CNICU). STUDY DESIGN: Prospectively designed observational study. RESULT: Median sound levels in the unoccupied BNICU (37.6 dBA) were lower than the CNICU (42.1 dBA, P<0.001). Median levels of minimum (6.4 vs 48.4 lux, P<0.05) and maximum illumination (357 vs 402 lux, P<0.05) were lower in the BNICU

Risk factors for incomplete vaccination in children less than 18 months of age attending the nurseries of day-care centres in Sao Paulo, Brazil.

To estimate the proportion of children in day-care centres with incomplete vaccination and to identify associated risk factors, we conducted a cross-sectional study among 258 children less than 18 months of age attending public and philanthropic day-care centres in the city of Sao Paulo, Brazil. Interviews, blood collection and anthropometry were performed. Unconditional logistic regression was adjusted for incomplete vaccination risk factors. 10.9% of children had incomplete vaccination. Children who were born prematurely (OR=4.27; p=0.004), or were malnourished (OR=4.99; p=0.049), or lived i

Establishing demand feeding in hospital.

In the summer of 1975 demand feeding was introduced for all babies born at the John Radcliffe Hospital, where previously babies were fed according to a rigid schedule to fit in with ward routine--clock feeding. Over a 10-day period before demand feeding was introduced details were collected about infant feeds concerning 42 normal babies whose mothers had decided to breast feed. 2 weeks after the introduction of demand feeding similar details were collected about 43 normal breast-fed babies. At the time of the observations 65% of all babies born in this hospital were being breast fed. Comparing

Cardiac Troponin-I Level at 24 hours of Age in Stable Newborn Infants Born at ≥35 Weeks of Gestation.

Cardiac troponin-I is a known biomarker of myocardial injury in adults and children but its diagnostic utility is unclear in newborns. This study aimed to establish normative data for troponin-I in stable newborns and assess any variation due to maternal diabetes status, mode of delivery, and Apgar scores.Prospective, observational study of stable newborn ≥35 weeks gestation admitted to a well-baby nursery at a single institution. Infants with respiratory distress, congenital infections, malformations, or syndromes were excluded. Troponin-I values were obtained by a validated point-of-care capi

Molecular epidemiology of staphylococcal scalded skin syndrome in premature infants.

BACKGROUND: Outbreaks of nosocomial staphylococcal scalded skin syndrome (SSSS) in infants have been well-described associated with the well baby nursery or delivery room. We describe two cases of SSSS in very low birth weight infants in a neonatal intensive care unit (NICU) and the success of infection control strategies used to prevent an outbreak. METHODS: Staphylococcal scalded skin syndrome was diagnosed in two infants in the NICU: Case I (a 47-day-old, formerly 530-g female); and Case II diagnosed 48 h later (a 41-day old, formerly 706-g female). Multiple infection control measures were

Staphylococcus aureus infections in Australasian neonatal nurseries.

OBJECTIVE: To study the incidence and outcome of systemic infections with methicillin sensitive (MSSA) and methicillin resistant Staphylococcus aureus (MRSA) infections in Australasian neonatal nurseries. METHODS: Prospective longitudinal study of systemic infections (clinical sepsis plus positive cultures of blood and/or cerebrospinal fluid) in 17 Australasian neonatal nurseries. RESULTS: The incidence of early onset sepsis with S aureus, mainly MSSA, was 19 cases per 244 718 live births or 0.08 per 1000. From 1992 to 1994, MRSA infections caused only 8% of staphylococcal infections. From 1

Quality Improvement Reduces Excessive Serum Bilirubin Laboratory Draws in a Community Hospital Nursery.

BACKGROUND: Newborn jaundice is common and requires appropriate monitoring and treatment to prevent acute bilirubin encephalopathy and kernicterus. Minimizing total serum bilirubin (TsB) draws has many benefits, including decreasing blood loss and painful procedures, as well as saving time and costs. OBJECTIVE: The objective of this study was to test an iterative quality improvement process to reduce the percentage of newborns receiving TsB draws by half, ie, from 30.6% to 15.3%, at a community hospital nursery over 18 months. METHODS: Baseline data were collected retrospectively, and interv

Modeling the cost and performance of early identification protocols.

This is the first in a series of three papers concerned with the early identification of hearing loss. In this paper, a simple model is presented that permits the calculation of the performance and cost of early identification protocols. In the second paper (Turner, in press), this model is used to compare four early identification protocols that differ in hearing screening strategies. The third paper (Turner, in press) examines the factors that influence the early identification protocol. The model described in this paper is sufficiently general to accommodate most early identification strate
