Assessment of the informed consent process in the provision of dental care in Mulago hospital, Uganda.

BACKGROUND: Informed consent is grounded in the principle of autonomy and represents patients' right to participate in clinical decisions regarding their treatment. It is equally an ethical and legal requirement in dental care. The dental practitioner must offer appropriate information about all aspects of the treatment and ensure that a patient understands and makes an informed decision. There is limited literature on informed consent for dental care in Uganda. This study assessed patients' comprehension of the informed consent process and dental practitioners' practices in obtaining informed

Oral and cutaneous pemphigus vulgaris: an atypical clinical presentation.

Pemphigus vulgaris (PV) is a rare painful and blistering autoimmune mucocutaneous disorder that appears in middle-aged adults. Oral lesions typically precede cutaneous involvement and tend to be more recalcitrant to therapy. The objective of this article is to present a case of oral and cutaneous PV in an atypical patient, a 23-year-old woman. The case was distinguished by the patient's age, which was 2 to 3 decades younger than the reported mean age of onset, and its coincidence with celiac disease, an immunopathologic process rarely seen in association with PV. Intravenous administration of

Community pharmacists' perceptions on their role in counseling patients on lifestyle modifications.

BACKGROUND: Patients need an accessible health care provider who can assist in lifestyle counseling for chronic disease states. Pharmacists are easily accessible in the community, as a recent study found that nearly 90% of Americans live within 5 miles of a community pharmacy. Thus, pharmacists are ideally positioned to offer counseling on lifestyle modifications at this health care access point. OBJECTIVES: The study's primary objective was to identify common barriers that community pharmacists may face in regard to provision of lifestyle counseling. Secondary objectives included determining

Radiation dosimetry analyses of radiographic imaging systems used for orthodontic treatment: comparison among child, adolescent, and adult patients.

OBJECTIVES: The aim of this study was to compare the effective doses of orthodontic radiographs in children, adolescents, and adults. METHODS: We exposed a child, an adolescent (simulated by an adult female phantom), and adult male phantoms using common scanning protocols for panoramic radiography, cephalography, and cone-beam computed tomography (CBCT). Glass dosimeters were placed in the organs of the phantom to measure the absorbed doses. The effective doses were deduced using tissue weighting factors as defined in the ICRP Publication 103. RESULTS: For panoramic imaging, the parotid glan

Osseointegration of Extremity Prostheses: A Primer for the Plastic Surgeon.

Interest in amputation surgery has increased in conjunction with rising public awareness about amputee care. To date, plastic surgeons have impacted the quality of life and functional potential of amputees through novel strategies for sensory feedback and prosthesis control and various techniques for neuroma treatment and prevention. Osseointegration, which involves the direct skeletal attachment of a prosthesis to bone, has the ability to further maximize amputee function. There exists a critical role for plastic surgeons to help optimize techniques for extremity osseointegration through impr

Clinical Reasoning: A State of the Science Report.

Clinical reasoning is the cognitive process that nurses use to gather and incorporate information into a larger bank of personal knowledge. This incorporated information guides therapeutic actions, and helps determine client care. Since the process guides therapeutic actions regarding client care, failure to use the process effectively leads to poor clinical decision-making, inappropriate actions, or inaction. Because of the criticality of this process, this paper presents an analysis of the literature that reveals the current state of the science of clinical reasoning, identifies gaps in know

Physical Therapists' Role in Solving the Opioid Epidemic.

An estimated 116 million Americans suffer from chronic pain, at a cost of over $600 billion per year, or roughly $2000 per person per year. In this Viewpoint, the authors highlight the challenges of the current opioid epidemic and outline strategies that the physical therapy profession may adopt to be part of the solution. These strategies include facilitating and providing patient education, early access to physical therapy services, and the promotion of health, wellness, and prevention. J Orthop Sports Phys Ther 2018;48(5):349-353. doi:10.2519/jospt.2018.0606.

Tobacco use and mental illness: a wake-up call for psychiatrists.

Tobacco use results in numerous consequences for individuals with mental illnesses and other substance use disorders, yet it is not adequately addressed by behavioral health professionals, including psychiatrists. This column describes current inaction among behavioral health professionals and some possible reasons for it and recommends next steps. Psychiatrists should provide treatment for all patients with a co-occurring tobacco use disorder and provide leadership to change policies and practices in treatment centers. Psychiatrists can be vital leaders of the effort to reduce the toll of tob

Symptomatic Overlap and Therapeutic Opportunities in Primary Headache.

Headache, a nearly universal experience, remains costly, disabling, and often suboptimally managed. The most common presentations in the United States are migraine, tension-type headache (TTH) and "sinus" headache, but their extensive symptomatic overlap suggests that these conditions can be approached as variations in the same underlying pathology and managed accordingly. We use case studies of patients with varying prior diagnoses (none, migraine, TTH, and sinus headache), as well as a 4-question diagnostic screening tool, to illustrate how pharmacists can use this conceptual framework to si

Diabetic retinopathy education and screening at the community pharmacy in Puerto Rico.

OBJECTIVE: Diabetic retinopathy (DR), the most common diabetes-related eye disorder, is associated with poor glucose control and is the most frequent cause of adult-onset blindness. The purpose of this study was to demonstrate that pharmacists can play an effective role in identifying individuals at risk for developing DR, educating them about DR, facilitating access to diagnostic eye exams, and referring patients diagnosed with vision problems or other conditions for treatment and continuing care. METHODS: This two-phase project was conducted at two independent community pharmacies in Puerto

Ethical tensions in genetic counselling research.

Ethical tensions are recognised as part of the everyday practice of conducting research and practising genetic counselling. What are the conceptual frameworks that can assist researchers and genetic counsellors to deal with ethical tensions when conducting research? How might the overlap that arises from being a researcher and clinical genetic counsellor be dealt with? This article uses a case study to consider the ethical tensions between conducting research in genetic counselling and maintaining a clinical practice as a genetic counsellor. It examines the reconciliation of the dual roles of

What are the concerns of prospective responsible officers about their role in medical revalidation?

INTRODUCTION: The Health and Social Care Act 2008 sets out the requirement for the introduction of responsible officers for the NHS. The paper states that these individuals will be responsible for ensuring that doctors who are revalidated meet the required standards. It is not known how well prepared prospective responsible officers are for their new role. METHODS: Semi-structured interviews were conducted with 12 medical directors from Scottish health boards who agreed to participate. RESULTS: Prospective responsible officers remain uncertain about their role in medical revalidation. Specif

Promoting prevention and early recognition of malignant melanoma.

Research continues to support several of the traditional risk factors for melanoma: a personal history of melanoma, basal or squamous cell carcinomas, the presence of moles, sun sensitivity, occupational exposure to certain substances, or a depressed immune system (ACS, 2007; Batailee et al., 2007; Batistatou et al., 2007). These are now joined by several new risk factors, namely, a history of dysplastic nevi, nevi persisting into adulthood, the use of pesticides, and not being vaccinated with BCG or vaccinia vaccines (Fortes et al., 2007; Krone et al., 2005; Shors et al., 2006). Down syndrome and sleep-disordered breathing: the dentist's role.

BACKGROUND: Sleep apnea is a common sleep disorder characterized by sleep fragmentation, oxygen desaturation and daytime somnolence. It is common in people with Down syndrome, occurring in at least one-half of this population. METHODS: The authors review the clinical presentation of obstructive sleep apnea and its association with Down syndrome. RESULTS: The diagnosis and treatment of sleep-disordered breathing in the Down syndrome population reinforce the important role that dentists play in recognizing its association with comorbidities and daily cognitive and behavioral function. CONCLUS

Dyslexia: a review of two theories.

Optometrists will frequently see patients, who may have a diagnosis or a suspected diagnosis of dyslexia (specific reading disorder) and will need to manage and counsel such patients. There are many propounded theories on the cause(s) of dyslexia. Although most professionals in this area consider that dyslexia is chiefly a linguistic disorder, the possibility of a visual component is contentious. This article is a selective review of two commonly discussed theories that suggest a visual component in dyslexia; the magnocellular deficit theory and Meares-Irlen syndrome.

A randomized controlled trial of consensus-based child abuse case management.

OBJECTIVE: This study evaluates the effects of expert-assisted child abuse and neglect case management in the German child welfare and healthcare system as perceived by the case workers themselves. METHODS: Case workers with different professions (social workers, counselors, clinic-based and office-based psychotherapists, and physicians) participated in the study. They were responsible for 80 child protection cases which were enrolled for the study and randomly assigned either to expert-assisted case management or to case management as usual. The sample represented a broad range of child prot
