Joint detection of risk for psychotic disorders or bipolar disorders in clinical practice in the UK: development and validation of a clinical prediction model.

BACKGROUND: Efficient detection of individuals at risk of developing psychotic disorders or bipolar disorders is a crucial step to improving mental health outcomes in young people. A novel, transdiagnostic approach to jointly detect individuals at risk for either psychotic disorders or bipolar disorders would maximise the effect of prevention. The aim of this study is to develop and validate an individualised prediction model to detect the risk of developing psychotic disorders or bipolar disorders in the UK. METHODS: This RECORD Statement and TRIPOD+AI compliant study describes the developme…

Case report: Co-occurring obsessive-compulsive disorder and affective psychotic disorder complicated by catatonia in an adolescent female patient.

We present the case of a 16-year-old female patient who experienced the loss of her mother to suicide, leading to post-traumatic stress disorder and prominent mood symptoms. She developed catatonic features during her inpatient psychiatric hospitalization following her own suicide attempt. Over her hospital course, she began to demonstrate signs of co-occurring obsessive-compulsive disorder (OCD) and affective psychotic disorder obfuscated by the severity of her catatonia. After initial workup including neurologic evaluation, laboratory tests, imaging (EEG, MRI), the patient was stabilized on

Association of Attention-Deficit/Hyperactivity Disorder in Childhood and Adolescence With the Risk of Subsequent Psychotic Disorder: A Systematic Review and Meta-analysis.

IMPORTANCE: Growing evidence supports an association between attention-deficit/hyperactivity disorder (ADHD) in childhood and subsequent psychotic disorders. Both disorders share physiopathological features such as attention deficits, dopaminergic imbalance, and genetic susceptibility. However, the results of epidemiologic studies have been conflicting. OBJECTIVE: To provide a quantitative synthesis of studies exploring the association between ADHD and the risk of subsequent psychotic disorder. DATA SOURCES: A systematic literature search of the MEDLINE, Scopus, PsycInfo, and Web of Science …

Identifying dynamic functional connectivity biomarkers using GIG-ICA: Application to schizophrenia, schizoaffective disorder, and psychotic bipolar disorder.

Functional magnetic resonance imaging (fMRI) studies have shown altered brain dynamic functional connectivity (DFC) in mental disorders. Here, we aim to explore DFC across a spectrum of symptomatically-related disorders including bipolar disorder with psychosis (BPP), schizoaffective disorder (SAD), and schizophrenia (SZ). We introduce a group information guided independent component analysis procedure to estimate both group-level and subject-specific connectivity states from DFC. Using resting-state fMRI data of 238 healthy controls (HCs), 140 BPP, 132 SAD, and 113 SZ patients, we identified

Determinants of spontaneous extrapyramidal symptoms in elderly psychiatric inpatients diagnosed with Alzheimer's disease, major depressive disorder, or psychotic disorders.

Extrapyramidal symptoms (EPS) occur more frequently in dementia of the Alzheimer's type (DAT) than in normal aging. Other late-life mental disorders, however, have also been associated with EPS. To examine whether EPS are increased in DAT patients relative to neuropsychiatric control subjects, the authors compared EPS in 127 neuroleptic-free elderly patients diagnosed with either DAT, major depressive disorder (MDD), or a psychotic disorder (SCHIZ/DELUS). They also examined whether depressive or psychotic symptoms were associated with EPS independently of diagnosis. Severity of parkinsonian ri

Characterizing Altered Cardiac Autonomic Dynamics in Psychotic Disorders Through Multidomain Heart Rate Variability Analysis and Deep Learning.

Recent developments in wearable devices have opened the possibility of heart rate variability (HRV), a quantitative measure of autonomic nervous system (ANS) regulation, as a potential indicator for the early detection of psychiatric disorders. This study explores HRV for the detection of psychotic disorders by integrating multidomain HRV analysis with a hybrid deep learning technique. Polar H10 electrocardiogram (ECG) device was employed to collect RR interval data from 30 patients diagnosed with either schizophrenia (SZ) or bipolar disorder (BD), and 30 similar age healthy controls, with a d…

Safety behaviors maintain persecutory ideation in individuals with psychotic disorders: evidence from an ecological momentary assessment study.

BACKGROUND: Safety behaviors are a common response to persecutory ideation in psychosis. They potentially contribute to the maintenance and exacerbation of symptoms and emotional distress and are therefore a promising treatment target. However, empirical evidence for this hypothesized maintenance role is scarce. We therefore examined cross-sectional and micro-longitudinal associations between safety behaviors, negative affect, and persecutory ideation in the daily lives of individuals with psychotic disorders. METHODS: We assessed safety behaviors, persecutory ideation, and negative affect at…

Inflammatory protein markers and treatment outcomes across mood and psychotic disorders: a systematic review and meta-analysis.

Poor treatment outcomes worsen prognosis in schizophrenia, bipolar disorder (BD), and major depressive disorder (MDD) and likely involve shared mechanisms, including alterations in inflammatory protein markers. However, the role of these markers on treatment outcomes was not previously studied cross-diagnostically, and to fill this gap, we performed a systematic review and meta-analysis. We searched major scientific databases for studies providing quantitative data on the association between peripheral inflammatory protein markers and psychopharmacological treatment outcomes in schizophrenia,

Long-term antipsychotic use and cardiometabolic adverse effects in adults with non-psychotic disorders: A systematic review.

BACKGROUND: Off-label antipsychotic prescribing for non-psychotic disorders is increasing, often at low doses and over extended periods. While cardiometabolic adverse effects are documented among individuals treated with antipsychotics for psychotic disorders, the consequences of long-term off-label antipsychotic use are not well-known. This systematic review synthesises evidence for cardiometabolic adverse effects of long-term antipsychotic therapy in adults and the elderly with non-psychotic disorders. METHODS: We searched electronic databases for randomised controlled trials or observation…

Association between psychotic-like experiences and common mental disorders in a general adult population in Lagos, Nigeria.

BACKGROUND: Psychotic-like experiences (PLEs) are subclinical phenomena associated with psychiatric vulnerability. While linked to common mental health symptoms in high-income countries, their patterns in sub-Saharan Africa are underexplored. OBJECTIVES: This study examined associations between clinically significant PLEs (CS-PLEs) and three domains of common mental health symptoms: depressive symptoms, anxiety symptoms, and somatic symptom burden among Nigerian adults. Secondary aims included exploring symptom severity gradients, demographic moderators, and functional impairment. METHODS: D

Smartwatch-Derived Digital Phenotypes Relate to Psychopathology Dimensions in Patients With Psychotic Spectrum Disorders: Longitudinal Observational Study.

BACKGROUND: Digital phenotyping refers to the objective measurement of human behavior via devices such as smartphones or watches and constitutes a promising advancement in personalized medicine. Digital phenotypes derived from heart rate, mobility, or sleep schedule data have been used in psychiatry to either diagnose individuals with psychotic disorders or to predict relapse as a binary outcome. Machine learning models so far have achieved predictive accuracies that are significant but not large enough for clinical applications. This could hinge on broad clinical definitions, which encompass …

Structural Racism and Inequities in Incidence, Course of Illness, and Treatment of Psychotic Disorders Among Black Americans.

Psychotic disorders (e.g., schizophrenia, schizoaffective disorder) are a leading cause of morbidity and premature mortality and an overlooked health inequity in the United States. European data indicate inequities in incidence, severity, and treatment of psychotic disorders, particularly for Black communities, that appear to be primarily attributable to social adversities. The dominant US narrative is that any observed differences are primarily a result of clinician bias and misdiagnosis. We propose that employing the framework of structural racism will prompt European and US research to conv…

Dynamic interplay between life events and course of psychotic disorders: 10-year longitudinal study following first admission.

BACKGROUND: Life events (LEs) are a risk factor for first onset and relapse of psychotic disorders. However, the impact of LEs on specific symptoms - namely reality distortion, disorganization, negative symptoms, depression, and mania - remains unclear. Moreover, the differential effects of negative v. positive LEs are poorly understood. METHODS: The present study utilizes an epidemiologic cohort of patients (N = 428) ascertained at first-admission for psychosis and followed for a decade thereafter. Symptoms were assessed at 6-, 24-, 48-, and 120-month follow-ups. RESULTS: We examined sympto…

Motivational and cognitive factors linked to community integration in homeless veterans: study 1 - individuals with psychotic disorders.

BACKGROUND: Little is known about the determinants of community integration (i.e. recovery) for individuals with a history of homelessness, yet such information is essential to develop targeted interventions. METHODS: We recruited homeless Veterans with a history of psychotic disorders and evaluated four domains of correlates of community integration: perception, non-social cognition, social cognition, and motivation. Baseline assessments occurred after participants were engaged in supported housing services but before they received housing, and again after 12 months. Ninety-five homeless Vet…

Utility of Imaging-Based Biomarkers for Glutamate-Targeted Drug Development in Psychotic Disorders: A Randomized Clinical Trial.

IMPORTANCE: Despite strong theoretical rationale and preclinical evidence, several glutamate-targeted treatments for schizophrenia have failed in recent pivotal trials, prompting questions as to target validity, compound inadequacy, or lack of target engagement. A key limitation for glutamate-based treatment development is the lack of functional target-engagement biomarkers for translation between preclinical and early-stage clinical studies. We evaluated the utility of 3 potential biomarkers-ketamine-evoked changes in the functional magnetic imaging (fMRI) blood oxygen level-dependent respons
