Perforation of a viscus with a mesh product either during or subsequent to pelvic floor reconstruction can be associated with devastating outcomes. If surgeons are going to place mesh, they also need to be familiar with symptoms concerning for perforation. The index of suspicion should always be present, as these patients can present years after initial mesh placement. The best opportunity for intervention in these serious complications is the first intervention. As bits of mesh are chipped away during attempted interventions, residual mesh fragments become disjointed, frayed, and scarred furt

This study investigated the long-term effectiveness and safety of a variant of the transobturator vaginal tape inside-out technique for acquired urinary incontinence. Twelve spayed female dogs were operated over a 2 yr period. No intraoperative complications were encountered. Transient dysuria was the most common postoperative complication (7 out of 12 dogs). On the 12th day postoperatively, incontinence was completely cured in 11 out of 12 dogs (92%). At the time of the second evaluation (median follow-up time was 21 mo), patients classified as "cured," "greatly improved," or "improved" were

AIMS: There is a lack of data describing the current state of stress urinary incontinence (SUI) procedures in academic centers. Urologists, gynecologists, and urogynecologists perform these operations, but the relative volume each group accounts for is unknown. The purpose of this study was to evaluate the distribution of cases in academic centers between specialties and associated patient characteristics.
METHODS: A hospital consortium database was used to identify patients treated surgically for SUI between 2009 and 2014. Patient and surgeon variables were evaluated. Patient variables (age,

BACKGROUND: There are many opinions but little firm knowledge about the optimal treatment of neurogenic stress urinary incontinence (NSUI).
OBJECTIVE: To scrutinize the quality and surgical outcomes of the available treatment modalities in the published literature.
EVIDENCE ACQUISITION: A systematic review of the published literature from Pub Med and Web of Science was undertaken for studies describing surgical treatment of NSUI between 1990 and 2013. A checklist of criteria of methodological and reporting quality of interventions in urological publications was applied to assess quality of t

INTRODUCTION: There are few data concerning the results of the treatment of recurrent stress urinary incontinence (SUI) after redo mid-urethral sling (MUS) procedure.
METHODS: Retrospective study concerning 34 patients presenting with recurrent SUI following the placement of a first MUS procedure and who have undergone a second MUS procedure. Results were evaluated objectively (cough stress test) and subjectively using international consultation on incontinence questionnaire-short form (ICIQ-SF).
RESULTS: The surgical technique for the placement of the first MUS was a retropubic approach in

The risk of postoperative stress incontinence (SI) is increased in women with occult stress incontinence (OSI) but the majority of patients will not develop troublesome SI postoperatively or need further SI surgery. This risk reported in current studies does not warrant a mid-urethral sling at the time of surgery for pelvic organ prolapse in women with OSI. However, if performed, the risks as well as the benefits need to be discussed with the patient.

INTRODUCTION AND HYPOTHESIS: A previous version of the Cochrane review for prolapse surgery in 2008 provided two conclusions: abdominal sacrocolpopexy had lower recurrent vault prolapse rates than sacrospinous colpopexy but this was balanced against a longer time to return to activities of daily life. An additional continence procedure at the time of prolapse surgery might be beneficial in reducing post-operative stress urinary incontinence; however, this was weighed against potential adverse effects. The aim of this review is to provide an updated summary version of the current Cochrane revie

OBJECTIVE: To determine whether the position of the tension-free vaginal tape (TVT) has an effect on postoperative flow rates and voiding function in women successfully cured of urodynamic stress incontinence.
METHODS: Postoperatively 72 women who had undergone TVT surgery had a transperineal ultrasound examination to assess the position of the TVT on the urethra. The tape was categorized as proximal, mid or distal urethral. The effect on voiding was assessed using the maximum flow rate (MFR) centiles corrected for voided volume and detrusor pressure at maximum flow.
RESULTS: Forty-six women

BACKGROUND: The tension-free vaginal tape (TVT) procedure has become standard for the treatment of stress urinary incontinence in women. The procedure carries a risk of vaginal erosion and exposure of the mesh. When this occurs, most surgeons recommend removal of the tape for immediate relief of symptoms. However, this poses a risk of recurrence of urinary incontinence.
CASE: A premenopausal woman had an exposed vaginal mesh after a TVT procedure. After failed conservative treatment, she was treated successfully using a Martius graft, with preservation of the mesh.
CONCLUSION: The Martius pr

TVT-Secur is a new "less" invasive derivative of the tension-free vaginal tape (TVT). We report an unusual case of dyspareunia for both the patient and her husband resulting from a retained finger pad from the TVT-Secur introducer. The sling was also explanted because of malposition.

PURPOSE: We evaluated the efficacy of the transurethral resection (TUR) for the management of intravesically exposed polypropylene mesh after midurethral sling (MUS) procedures.
PATIENTS AND METHODS: Fourteen patients had a TUR for intravesical mesh after MUS procedures (tension-free vaginal tape, 11; transobturator tape, 3). In six patients with large stones, cystoscopic division and cystolitholapaxy were needed before the TUR.
RESULTS: Patients presented with dysuria (9), hematuria (7), urgency incontinence (5), urinary frequency (2), and pelvic pain (2). Thirteen of 14 (92.9%) patients ha

Severe urinary tract injury is a relatively uncommon occurrence after the placement of a tension-free vaginal tape sling. Bladder perforation is the most common urinary tract injury, but bladder drainage for a few days is usually the only intervention that is required. Urethral erosions of synthetic sling tapes are much rarer, but can result in more significant sequelae such as voiding dysfunction or fistula formation. A 50-year-old woman had an erosion of a synthetic, tension-free midurethral sling through the urethra. She underwent resection of the sling with closure of the urethral defect.

We studied safety and topographic characteristics of a transobturative approach in application of an "inside out" tension free tape (TFT) for management of stress urinary incontinence (SUI) in women. The anatomic sections were made on 6 fresh female cadavers (mean age of the deceased 78.5 +/- 4 years). After the tape placement a femoral section was made with its extension to the perineum and retropelvic section. We found that the TFT passes far from the major femoral structures. The distance from the obturator canal to the TFT averaged 24.4 mm +/- 1.8 mm (from 23.9 to 30.4 mm, median 21.4 mm).

Infection is a recognised complication of using synthetic mesh tapes in continence surgery. It has only been reported alongside erosion after trans-obturator sling insertion for urodynamic stress incontinence. A 41-year-old patient developed mesh infection. This infection was diagnosed 16 months after insertion, when it became an abscess and discharged through her groin. The condition was not suspected earlier, despite having examination under anesthesia and repeat cystoscopy. The patient recovered upon removing the mesh and remains continent. This case documents the possible development of me

Although rare, serious vascular injuries after tension-free vaginal tape (TVT) insertion have been well known. We present a case of external iliac artery injury after TVT insertion, managed successfully by surgical intervention. This article also reviews the literature regarding major vascular injuries and their management. We conclude that clinicians should be able to suspect rare but serious vascular injuries, and patients have to be fully counselled regarding their potential consequences.
