Dynamic functional connectivity in borderline personality disorder: associations with trauma, emotion regulation and symptom severity.

BACKGROUND: Altered intrinsic functional connectivity is a well-established marker of borderline personality disorder (BPD). However, recent research suggests that investigating brain dynamics may offer a more detailed perspective on the neural signatures of BPD-related symptoms. METHODS: Resting-state fMRI data were analyzed in female patients with BPD (n = 47) and healthy controls (n = 28) to derive dynamic functional network connectivity (dFNC) indices using both meta-state and cluster-state approaches. Between-group comparisons assessed BPD-related dFNC alterations, while dimensional anal

Hippocampal volume in women victimized by childhood sexual abuse.

BACKGROUND: Several prior studies have found reduced hippocampal volume in victims of psychological trauma with post-traumatic stress disorder (PTSD). We were interested to determine if this finding was evident in women who were victimized by severe sexual abuse in childhood. METHODS: In this study, hippocampal volume was measured using quantitative magnetic resonance imaging (MRI) in 21 women who reported being severely sexually abused in childhood and 21 socio-demographically similar women without abuse histories. RESULTS: Women who reported sexual victimization in childhood had significan

Penetrating colon injuries: exteriorized repair vs. loop colostomy.

Eighty-five patients with penetrating colon injuries, treated either by exteriorized repair (39) or loop colostomy (46), were analyzed. Missile wounds accounted for 75.3% of the injuries. The Penetrating Abdominal Trauma Index (PATI) was the scoring method employed to assess quantitatively the severity of injuries in each patient. Of 21 patients with right colon injuries, eight were treated by exteriorized repair and the remainder by loop colostomy. PATI and other variables were comparable in both groups. Suture line leaks occurred in two patients (25%) with exteriorized repair. The morbidity

Comparison of trauma scores and total prehospital time in the prediction of clinical course in a plane crash: Does timing matter?

OBJECTIVE: To investigate how the total prehospital time (TPT), Abbreviated Injury Scale (AIS), Injury Severity Score (ISS), and Trauma Score-Injury Severity Score (TRISS) affect the outcome of plane crash victims from anatomical, physiological and psychological perspectives. The accuracy or strength of these scores and TPT in predicting hospitalization and surgery, sequelae development and psychiatric complications [permanent temporary disability (PoTDs)] and PTSD can allow medical professionals to direct and prioritize management efforts of the victims of mass casualties in general. The design of a Bayesian adaptive clinical trial of tranexamic acid in severely injured children.

BACKGROUND: Trauma is the leading cause of death and disability in children in the USA. Tranexamic acid (TXA) reduces the blood transfusion requirements in adults and children during surgery. Several studies have evaluated TXA in adults with hemorrhagic trauma, but no randomized controlled trials have occurred in children with trauma. We propose a Bayesian adaptive clinical trial to investigate TXA in children with brain and/or torso hemorrhagic trauma. METHODS/DESIGN: We designed a double-blind, Bayesian adaptive clinical trial that will enroll up to 2000 patients. We extend the traditional

A retrospective register study comparing fibrinogen treated trauma patients with an injury severity score matched control group.

BACKGROUND: Fibrinogen concentrate (FC) is frequently used to treat bleeding trauma patients, although the clinical effects are not well known. In this study we describe demographic and clinical outcome data in a cohort of trauma patients receiving FC, compared to a matched control group, who did not receive FC. METHODS: This retrospective, single-center, observational study included adult trauma patients admitted to a level 1-trauma center in Sweden between January 2013 and June 2015. The study population consisted of patients to whom FC was administrated within 24 h (n = 138, "Fib+"). Patie

Blunt chest wall trauma: an overview.

Trauma affecting the chest wall, even in isolation, can carry a significant morbidity and mortality and thus appropriate management is vital. Consequences of chest wall trauma may include significant pain, altered chest wall mechanics, hypoventilation, infection and respiratory failure. In order to best determine the appropriate management, risk stratification tools have been developed to identify patients at highest risk of complications who would most benefit from more invasive management strategies. Early optimization of analgesia is vital both for patient experience and to reduce the risk

Contextualising Safety in Numbers: a longitudinal investigation into change in cycling safety in Britain, 1991-2001 and 2001-2011.

INTRODUCTION: The 'Safety in Numbers' (SiN) phenomenon refers to a decline of injury risk per time or distance exposed as use of a mode increases. It has been demonstrated for cycling using cross-sectional data, but little evidence exists as to whether the effect applies longitudinally -that is, whether changes in cycling levels correlate with changes in per-cyclist injury risks. METHODS: This paper examines cross-sectional and longitudinal SiN effects in 202 local authorities in Britain, using commuting data from 1991, 2001 and 2011 censuses plus police -recorded data on 'killed and seriousl

Practice Bulletin No. 165: Prevention and Management of Obstetric Lacerations at Vaginal Delivery.

Lacerations are common after vaginal birth. Trauma can occur on the cervix, vagina, and vulva, including the labial, periclitoral, and periurethral regions, and the perineum. Most of these lacerations do not result in adverse functional outcomes. Severe perineal lacerations, extending into or through the anal sphincter complex, although less frequent, are more commonly associated with increased risk of pelvic floor injury, fecal and urinary incontinence, pain, and sexual dysfunction with symptoms that may persist or be present many years after giving birth. The purpose of this document is to p

Exposure to sub-concussive head injury in boxing and other sports.

BACKGROUND: Current characterizations of chronic traumatic brain injury (CTBI) in boxing, football and other sports are reviewed in the context of the history of research on sub-concussive brain trauma in athletes. METHODS: The utility of exposure models for understanding CTBI in boxers is examined and concerns regarding the paucity of findings supportive of an exposure model for CTBI in football players are discussed. RESULTS AND CONCLUSIONS: Recommendations for development of exposure models for sport-specific phenotypic characterizations of CTBI are presented.

[Spontaneous spinal epidural hematoma as cause of incomplete spinal cord injury].

Spontaneous spinal epidural hematomas are rare but are of differential diagnostic importance due to the potentially dramatic progression through to irreversible neurological deficits. At the beginning the clinical symptoms are non-specific and the development of neurological deficits leads to the diagnosis. We present the case of a 73-year-old female patient who initially reported uncharacteristic neck pain and developed incomplete quadriplegia during the next day as well as the case of a 78-year-old male patient, who complained of acute back pain and developed paraplegia a short time afterwar

Sport Concussion Assessment Tool-2: baseline values for high school athletes.

BACKGROUND: Concussion head injuries are common in high school athletes. The Sport Concussion Assessment Tool-2 (SCAT2) has been recommended and widely adopted as a standardised method of evaluating an injured athlete with a suspected concussion. Sideline return to play decisions can hinge on the results of a SCAT2 score. However, most athletes will not have had baseline testing performed for comparison if injury occurs. Therefore, establishing of age-, sex- and sport-matched normative data for the high school athlete population is critical. PURPOSE: To determine baseline scores in all SCAT2

Damage control orthopedics: current evidence.

PURPOSE OF REVIEW: There is still an ongoing debate whether damage control orthopedics (DCO) or other treatment strategies should be favored in the treatment of multiply injured patients. This review gives an overview of the current literature concerning this important question in the treatment of severely injured patients. RECENT FINDINGS: Several studies could show that DCO can reduce the inflammatory burden due to surgery (second hit). The only randomized study showed a benefit for borderline patients treated by DCO in comparison to early total care. Other studies showed advantages for ear
