Left renal vein graft for vascular reconstruction in abdominal malignancy.

BACKGROUND: Advanced abdominal malignancies are occasionally invasive for the major blood vessels, such as the portal vein (PV), inferior vena cava (IVC), and major hepatic veins (HVs), and complete removal of the tumors is required for patients undergoing vascular resection and reconstruction. We used left renal vein (LRV) grafts for vascular reconstruction in patients with these malignancies and evaluated their clinical relevance. METHODS: A total of 113 patients underwent vascular resection including the PV (42 patients), IVC (68 patients), and HV (3 patients) for hepatobiliary-pancreatic

Transradial approach for challenging vascular access interventions.

BACKGROUND: Percutaneous interventional procedures for vascular access are usually performed using the draining cephalic or basilic vein. The transradial approach, which has been extensively investigated for coronary angiography and intervention, could be an attractive new technique for peri-anastomotic arteriovenous fistula stenosis. METHOD: From June 2012 to February 2013, 30 patients with end-stage renal failure were evaluated for transradial vascular access intervention. A 4-French (Fr) micropuncture kit was used to access the radial artery and then subsequently upgraded to a 5-Fr sheath.

Comparison of preperfusion and postperfusion magnetic resonance angiography in acute stroke.

BACKGROUND AND PURPOSE: The multimodal magnetic resonance imaging study in acute stroke includes perfusion-weighted imaging (PWI) after administration of contrast and magnetic resonance angiography (MRA). However, MRA may overestimate the degree of vessel obstruction caused by limitations to detect low flow states. Our aim was to determine the usefulness of a new fast imaging protocol combining classical MRA, PWI, and postperfusion MRA to improve the diagnostic management in acute ischemic stroke. METHODS: We studied 31 patients with a middle cerebral artery (MCA) infarction within the first

Outcomes of long storage times for cryopreserved vascular grafts in outflow reconstruction in living donor liver transplantation.

The outflow reconstruction of the right anterior sector in a right liver graft (RLG) with cryopreserved vascular grafts (CVGs) is crucial for preventing graft congestion in living donor liver transplantation (LDLT). The impact of the duration of cryopreservation has not been evaluated so far. From 2006 to 2009, 250 LDLT were performed: 47 of these patients (group 1) received CVGs stored for ≦1 year, and 33 patients (group 2) received CVGs stored for >1 year. Single or multiple segment 8 hepatic veins were reconstructed. The number of anastomoses did not affect vascular graft patency (P = 0.21)

Determinants of graft patency after coronary artery bypass grafting.

Coronary artery bypass grafting (CABG) remains central for multivessel coronary artery disease, but durable benefit depends on graft patency. Patency reflects interacting anatomical, technical, haemodynamic, biological and patient-level determinants. The endothelium offers a useful, metabolically active interface because it regulates vascular tone, thrombosis, inflammation, leukocyte trafficking and repair. This Review examines how conduit choice, harvesting, preservation, anastomotic geometry, target anatomy, operative strategy and comorbidity drive early thrombosis, intermediate intimal hype

The effect of successful lower extremity revascularization on aortic stiffness in patients with peripheral arterial disease.

OBJECTIVE: Peripheral arterial disease (PAD) is a global health problem and associated with poor outcomes. It causes increased arterial stiffness. The association of PAD with aortic arterial stiffness was investigated in previous studies. However, there is limited data regarding the effect of peripheral revascularization on arterial stiffness. The aim of our study is to investigate the effect of peripheral revascularization on aortic stiffness parameters in patients with symptomatic PAD. METHODS: A total of 48 patients with PAD who underwent peripheral revascularization were included in the s

Conduits in Vascular Pancreatic Surgery: Analysis of Clinical Outcomes, Operative Techniques, and Graft Performance.

OBJECTIVES: We analyze successes and failures of pushing the boundaries in vascular pancreatic surgery to establish safety of conduit reconstructions. BACKGROUND: Improved systemic control from chemotherapy in pancreatic cancer is increasing the demand for surgical solutions of extensive local vessel involvement, but conduit-specific data are scarce. METHODS: We identified 63 implanted conduits (41% autologous vessels, 37% allografts, 18% PTFE) in 56 pancreatic resections of highly selected cancer patients between October 2013 and July 2020 from our prospectively maintained database. Medium Term Outcomes of Deep Venous Stenting in the Management of Venous Thoracic Outlet Syndrome.

OBJECTIVE: Venous thoracic outlet syndrome (vTOS) is a relatively rare condition associated with significant morbidity. Its management continues to evolve, with increasing use of endovascular adjuncts, such as percutaneous thrombectomy and angioplasty, in addition to first rib resection. The utility of stenting residual venous stenotic lesions is poorly defined within the literature. This study sought to review the medium term patency rates of upper limb deep venous stenting in the management of vTOS. METHODS: A single centre, retrospective review of patients managed for vTOS with first rib r

Novel technique of percutaneous bypass in a brachiocephalic arteriovenous fistula with cephalic arch occlusion: A case report.

OBJECTIVE: Arteriovenous fistulas are a principal mainstay of long-term dialysis access for patients with end stage renal failure. However, the patency of arteriovenous fistulas is limited, often requiring percutaneous transluminal angioplasty as a salvage procedure. We report a case of percutaneous method of arteriovenous fistula salvage. METHODS: A gentleman with brachiocephalic arteriovenous fistula created in 2015 was admitted under us for dialysis access issue. His fistula history was notable for recurrent and refractory venous outflow stenosis of the cephalic vein and the cephalic arch

Endovascular treatment of surgically implanted arterial graft thrombosis by using manual aspiration thrombectomy.

The purpose of this study was to present our experience with guiding catheters in manual aspiration thrombectomy of occluded infra-aortic bypass grafts. This material was designed as a guiding catheter but was also used for thrombus aspiration. Six consecutive patients (all male; mean age, 61.0±5.7 years; range, 54-68 years) who underwent manual aspiration thrombectomy at the discretion of the operator for infra-aortic bypass graft thrombosis between 2002 and 2010 were retrospectively reviewed. The angiographic success described as either stenosis or residual thrombus less than 30% was 67%. Pr

The thrombosed brachial-cephalic fistula used for renal dialysis--a cautionary note.

Arteriovenous fistulae (AVF) are commonly required for dialysis prior to renal transplantation, and are subsequently left insitu, even if thrombosed. The authors present one of two patients in whom progressive digital ischaemia occurred, and was initially overlooked, many years following formation of an AVF. The patient was surgically explored and clot protruding from the thrombosed vein into the brachial artery was noted. The arterial defect was closed with a vein patch and histological examination of the fistula confirmed clot. The patient recovered satisfactorily and remained well 6 months

Benefits of arterial reconstruction in claudication.

We conducted a midterm follow-up of 150 claudicants who underwent surgical reconstruction by assessing cumulative patency, survival, and palliation (graft patency in live patients) rates. Eighty-nine claudicants (group I) underwent direct (in situ) proximal revascularization, 33 (group II) had indirect (ex situ) proximal revascularization, while 28 (group III) had distal revascularization. The secondary patency rates at 3 years were 97.5% in group I, 97.0% in group II, and 75.0% in group III, respectively. Only one patient with limb graft thrombosis required below-knee amputation. There were 3

Omission of aspirin in patients following coronary artery bypass graft surgery.

Graft patency is a major factor contributing to the long-term results of coronary artery bypass graft (CABG) surgery. The systematic overview of the Antiplatelet Trialists' Collaboration provides unequivocal evidence that antiplatelet therapy reduces by nearly one-half the odds of coronary graft occlusion following CABG. We retrospectively reviewed patients undergoing CABG during 1993 at the Cardiothoracic Unit, Northern General Hospital, to determine the incidence of, and indications for, aspirin omission following CABG: 462 patients with isolated CABG, 75 patients with a combined CABG and a

The management of venous trauma: early and late results.

The result of a fifteen year study related to the management of the venous injuries in the iliac-femoro-popliteal segment have been assessed. The patients were divided in Group I--comprised 20 patients with venous revascularization and Group II--12 cases without revascularization. The early results at 30 days evaluated by the clinical examination revealed edema in all limbs without revascularization. The phlebographic patency of the revascularization was 70%. The difference between the two groups was significant (X2 = 14.93; X2 critical = 3.84). The late results were evaluated clinically in 14

The effect of distal anastomotic site on PTFE graft patency in lower extremity bypass.

The site of distal anastomosis of polytetrafluorethylene (PTFE) lower extremity bypass grafts may significantly affect results. We examined patency in 144 cases; 45 femoro-popliteal above knee (AK), 55 femoro-popliteal below the knee (BK) and 44 femoro-distal (D) PTFE bypasses, the groups being comparable with regard to other risk factors studied. Cumulative graft patency at 3 years was 71.3% for AK, 36.7% for BK, 16.4% for D and overall 35%. The site of distal anastomosis is an important determinant, of PTFE lower extremity bypass patency. We have abandoned the use of PTFE for BK and D, but f

Pediatric Neck Vessel Repair Following Extracorporeal Membrane Oxygenation Decannulation: Assessing Stroke Risk, Long-Term Neurologic Outcomes, and Vessel Patency.

Pediatric extracorporeal membrane oxygenation (ECMO) decannulation practices vary widely regarding ligation versus repair of the common carotid artery (CCA) and internal jugular vein (IJV) with potential implications for neurologic outcomes. We examined rates of ischemic cerebral vascular accident (CVA) and vessel patency in pediatric ECMO patients following decannulation. A retrospective review of pediatric ECMO patients (2009-2023) cannulated via cervical approach was conducted, excluding palliative decannulations. Clinical outcome measures were evaluated using descriptive and inferential st
