Reversal of central sleep apnea with oxygen.

OBJECTIVE: To examine the effect of oxygen on apneas and sleep quality in patients with frequent central apneas during sleep. DESIGN/SUBJECTS: Prospective intervention study of 20 consecutive patients with predominant central apnea identified from 570 patients referred for suspected sleep apnea syndrome. Sixteen patients had congestive heart failure and seven of them had a previous stroke. Three of the remaining four patients without heart failure had experienced a previous stroke, and one was being treated with morphine. SETTING: The Department of Pulmonary Medicine at Umeå (Sweden) Univers…

Effect of surgery on obstructive sleep apnea.

The statistical analysis of data obtained by monitoring pre- and post-operative conditions of patients treated for sleep apnea syndrome is reported. In the past 3 years, surgery was performed on 149 patients (55 children and 94 adults), who complained of snoring and symptoms related to the sleep apnea syndrome at Fujita Health University, The Second Hospital. Treatment for children was adeno-tonsillectomy. Treatment for adults was uvulo-palato-pharyngoplasty (UPPP), midline laser glossectomy (MLG) and/or nasal surgery. Apnea hypopnea index was defined as frequency of apnea hypopnea per hour. P…

Treatment of sleep apnea and snoring in children.

Sleep apnea syndrome in children is usually caused by adenoidal and tonsillar hypertrophy. Here, we report on a typical case of obstructive sleep apnea syndrome, and also discuss recent indications for tonsillectomy in children. A 6-year-old boy came to our hospital suffering from serious obesity and somnolescence. He could not sleep lying down and thus started to sleep in a sitting position. Even during daytime he slept while standing, and exhibited hypersomnolescence. He was 133 cm tall and weighed 80 kg, and tonsillar hypertrophy was recognised. Consequently, tonsillectomy and a weight redu…

Sleep apnea in patients with hemispheric stroke.

Sleep pattern and breathing in humans are altered following cerebrovascular accidents involving the brainstem. Sleep apnea is a well-established complication of stroke involving the brainstem. On the other hand, the effect of cerebral stroke on sleep and breathing has not been well defined. The diffuse cerebral symptoms such as cognitive deficits, depression or fatigue, after hemispheric stroke mimic those present in patients with sleep apnea. To define the breathing pattern in patients with stroke involving cerebral hemispheres without brainstem lesion and without the prior history of sleep-d…

[The sleep apnea syndrome in patients with acromegaly].

The authors examined 19 nonselected patients with acromegaly (8 women and 11 men), incl. 16 where the disease was hormonally active, 3 had normal growth hormone values, 11 patients had a surgical operation of the pituitary. The examination was made by the MESAM 4 method (all-night recording of the heart rate, respiratory sounds, body position and arterial oxygen blood saturation). The sleep apnoea syndrome was diagnosed in 10 patients (52.6%). In patients where the sleep apnoea syndrome was not confirmed by MESAM 4, so-called intermittent snoring was found, an indirect sign of impaired ventila

Apnea and sleep state in newborns and infants.

Many studies have shown a greater frequency of apneas in active sleep than in quiet sleep in healthy newborns and infants, both full-term and preterm. The ontogeny of increased apnea during active sleep is related to phasic inhibitory-excitatory central mechanisms occurring during active sleep. Respiratory instability in active sleep can be increased by factors which modify the newborn's or infant's homeostasis. We have shown: (1) an increase in body temperature (0.8 degrees C) significantly augments periodic breathing in preterm infants at 40 weeks postconceptional age; (2) maternal administr…

[Surgical treatment of snoring and sleep apnea].

Twenty-one patients (four females and 17 males) greatly inconvenienced by snoring were subjected to UPP with CO2-laser technique. All patients were overweight (body-mass index average 30 kg/m2). Narrowing was found at the naso/oropharyngeal border in 71%. All had either abnormal uvula or soft palate. Snoring was reduced to a degree no longer socially troublesome in 17 patients. Three patients were still inconvenienced to some extent. Snoring was completely unchanged in only one patient. UPPP is not very difficult to perform, and we did not notice any sequelae of importance. We think that UPPP

Thyroarytenoid muscle activity in sleep apneas.

In normal subjects the thyroarytenoid muscle (TA), a vocal cord adductor, has phasic expiratory activity during wakefulness that disappears during non-rapid-eye-movement (NREM) sleep. Fiber-optic studies have reported absent or irregular vocal cord movements during obstructive apneas and vocal cord adduction during central apneas. This study was designed to investigate TA activity during NREM sleep in 14 subjects with sleep apnea by means of intramuscular wire electrodes. During central apneas, which were recorded in three subjects, continuous TA activity was observed. During obstructive apnea…

Practical aspects of managing the child with apnea.

Sleep apnea is a common problem in children and probably more common than currently realized. Apnea in children may be central, obstructive or mixed. Otolaryngologists are called upon to diagnose and treat obstructive apnea. The most common cause of obstructive apnea in children is adenotonsillar hyperplasia, and several conditions predispose children to sleep apnea. The most severe, and occasionally only, signs occur during sleep. The majority of children can be diagnosed by a careful history from parents or caretakers. However, sleep sonography, pulse oximetry and polysomnography may be need…

Bialaphos poisoning with apnea and metabolic acidosis.

A 64-year-old man with ethanol intoxication, ingested a bottle of Herbiace (100 ml, 32 w/v% of bialaphos, CAS #35597-43-4, Meiji Seika Kaisha, Tokyo, Japan). He had severe metabolic acidosis and was treated with infusions of sodium bicarbonate and furosemide, plus gastric lavage and enema. The metabolic acidosis improved 15 hours after treatment but nystagmus, apnea and convulsions were progressive. Although his sensorium was clear, spontaneous respirations were not observed for 64 hours. The electroencephalographic findings of atypical triphasic waves and slow waves suggest a unique response …

Morphology of the uvula in obstructive sleep apnea.

Alterations in pharyngeal structure and function are considered fundamental in the pathogenesis of obstructive sleep apnea (OSA). However, little is known about morphologic features of the pharynx in patients with OSA. We therefore studied the tissue composition of the uvula (midsagittal section) in patients with OSA, using a quantitative, morphometric point-counting technique. Uvula tissue was obtained by uvulopalatopharyngoplasty (UPPP) in 33 patients (mean number of apneas per hour of sleep = 32.7 +/- 5.2) and by autopsy in 22 normal subjects not known to have OSA. All statistical compariso…

Effects of cilazapril on hypertension, sleep, and apnea.

Epidemiologic studies revealed that up to 10 percent of middle-aged men show more than 10 cessations of breathing of more than 10 seconds' duration. In these patients, increased morbidity and mortality rates have been proved. More than 50 percent of apnea patients exhibit arterial hypertension, and up to 50 percent of hypertensive patients experience sleep apnea. Patients with sleep apnea and essential hypertension need special attention paid to their antihypertensive therapy because the following side effects of drugs have to be avoided: increases of cardiac insufficiency, hyperviscosity of t…

Therapeutic effect of posture in sleep apnea.

Four patients with mild to moderate obstructive sleep apnea were monitored first in the supine posture to establish a diagnosis of obstructive sleep apnea (apnea indices 33, 12, 22, 36). A second polysomnogram, obtained while the patients slept in the lateral posture, showed a dramatic decrease in apnea (apnea indices 5, 0, 0.2, and 0) and snoring. Each patient had an enlarged uvula, which moved to the side in the lateral posture. We conclude that sleeping in the lateral posture may be therapeutic in some patients with obstructive sleep apneas.

The effects of oxygen in patients with sleep apnea.

The effects of 6 h of continuous low flow, nasally administered oxygen were compared with the effects of breathing air in 10 men and 2 women with obstructive sleep apnea and daytime hypersomnolence. The overall quality of sleep, sleep fragmentation, the pattern of breathing, nocturnal oxygenation, and the clinical effects on daytime hypersomnolence determined by multiple sleep latency testing were evaluated. We found that in non-REM sleep, breathing 3 L/min of oxygen increased baseline percent arterial oxyhemoglobin saturation and decreased both the rate of sleep-disordered breathing from 69 +…

Sleep apnea and nocturnal myoclonus in the elderly.

Insomnia, daytime sleepiness, and nocturnal wandering, so common in the elderly, are caused largely by two specific pathophysiologic processes. Sleep apnea is a condition where respiration pauses during sleep, leading to arousal. Sleep apnea is due either to obstruction in the throat or failure of the central respiratory center. Periodic movements in sleep are characterized by frequent ankle and leg flexions, leading to arousal. Sleep apnea and periodic movements in sleep require specific diagnoses and treatments. Each process occurs in 20%-30% of people over 65, and perhaps the majority of ol…
