[Evidence of indocyanine green fluorescence in robotically assisted colorectal surgery : What is the status?].

BACKGROUND: Indocyanine green (ICG) fluorescence imaging is increasingly being used in various areas of abdominal surgery. The constant improvement in the technology enables easy intraoperative use and progressively influences operative decision-making, also in robotically assisted colorectal surgery. OBJECTIVE: Summation of current evidence on the use of ICG fluorescence imaging in robotically assisted colorectal surgery. MATERIAL AND METHODS: The assessment of evidence is based on a comprehensive literature search (PubMed). RESULTS: First individual studies (feasibility, case matched, pro

The proposed use of radiofrequency ablation for the treatment of fistula-in-ano.

Fistula in ano is a very common presentation to colorectal clinic. Embarrassment due to the symptoms makes accurate estimations of incidence difficult. It is estimated that up to 40% of peri-anal abscess will be accompanied by or preceded by a fistula. Fistulae can be classified into simple fistulae that involve no or minimal sphincter muscle and complex, which involve significant amounts of the anal sphincter muscle, possibly with multiple tracts. For complex fistulae a seton suture is usually placed through the tract and out through the anus to form a loop allowing pockets of sepsis to drain

Anterior sagittal transrectal approach (ASTRA) for vaginoplasty after complications of posterior sagittal anorectoplasty.

Transperineal exposure of the high portion of the vagina is limited. These limitations can be circumvented using ASTRA (anterior sagittal transrectal approach). We report the use of this surgical strategy for the treatment of a case of acquired vaginal atresia after posterior anorectoplasty due to anorectal malformation.

From resection to preservation: redefining the surgical paradigm in Crohn's disease.

Surgery is a pivotal component of the management of Crohn's disease (CD), particularly in cases of disease-related complications or failure of medical therapy. However, the risk of short bowel syndrome following multiple large resections makes bowel preservation strategies a major therapeutic priority. This review gathers insights from a diverse group of inflammatory bowel disease specialists, exploring the full spectrum of bowel-preserving strategies, from conservative surgical techniques and peri-operative optimization to emerging innovations in robotic surgery. The therapeutic paradigm of t

[Visualizing the evidence of robotic gastrointestinal surgery based on guideline recommendations: an evidence mapping study of gastric and colorectal cancer].

Robotic surgery, as an increasingly widespread application in the treatment of gastric and colorectal cancer, still faces obvious discrepancies in recommendations, indications, and evidence strength across existing guidelines. This study systematically analyzed 31 relevant guidelines and consensus statements (retrieved from Chinese and English databases from January 2010 to May 2025) from two dimensions: feasibility (effectiveness, safety, etc.) and training quality control. The results showed that colorectal cancer guidelines had a higher proportion (4 guidelines) of "clear recommendations" fo

Critical care challenges in hepatobiliary and pancreatic surgery.

PURPOSE OF REVIEW: The aim of this review is to provide an update on some key aspects of the perioperative management of patients undergoing hepatopancreatobiliary (HPB) surgery. RECENT FINDINGS: Recent studies underline the importance of performing these surgeries in specialized centers to improve outcomes. In these centers of excellence, the high volume of HPB-performed surgery, the existence of enhanced recovery after surgery (ERAS) programs, and the use of minimally invasive surgery techniques all help to improve patient outcomes. In liver surgery, the main evolving challenges remaining a

Robotic low anterior resection plus transanal natural orifice specimen extraction in a patient with situs inversus totalis.

BACKGROUND: Situs inversus totalis (SIT) refers to an unusual condition involving reversal of abdominal and thoracic viscera, with an incidence rate of 1/5000-20,000 adults. Minimally invasive surgeries for SIT patients are technically challenging, while the surgical experience for SIT patients is quite limited. CASE PRESENTATION: A 61-year-old man, previously diagnosed as SIT, came to our hospital for 6 months history of hematochezia and altered bowel habit. A diagnosis of rectal cancer was made in view of colonoscopic biopsy which confirmed an irregular circumferential lump of well differen

[Endoscopic management of postoperative ureteral wound. Retrospective unicentric study from October 2003 to June 2014].

BACKGROUND: Ureteral wounds are rare with an incidence of 0.5 to 1% of pelvic surgeries. Their supports and their prognosis remain dependant of the period of support and the level of ureteral lesion. The importance of early treatment reduces morbidity and improves patient prognosis. METHODS: A retrospective study from October 2003 to June 2014 was performed in a university hospital using a systematic chart review of patients' urology, digestive surgery, vascular surgery and gynecology. RESULTS: Forty-six wounds were found in 43 patients. The majority of the ureteral wound was found at the pe

Multivariate analysis suggests improved perioperative outcome in Crohn's disease patients receiving immunomodulator therapy after segmental resection and/or strictureplasty.

BACKGROUND: Medical management of moderate to severe Crohn's disease (CD) using immunomodulator agents has not eliminated surgical treatment of disease complications. The effect of improved medical treatment on perioperative CD surgical outcome is not known. We analyzed the impact of immunomodulator therapy on the rate of intraabdominal septic complications (IASC) in CD patients undergoing bowel reanastomosis or strictureplasty. METHODS: Surgical outcome was reviewed in 100 consecutive CD patients who underwent segmental resection with primary anastomosis or strictureplasty between 1998 and 2

The effect of initial operative repair on the recovery of intestinal function in gastroschisis.

Recent reports concerning the treatment of gastroschisis suggest that primary closure results in more rapid return of gastrointestinal function, a shortened hospitalization, diminished perinatal complications, and improved long-term survival. A 4-year retrospective review of infants treated for gastroschisis at the University of Florida yielded 30 infants requiring repair of this abdominal wall defect. The series included 19 males and 11 females, and the average abdominal wall defect measured 4 cm in its greatest dimension. Nine infants (mean weight, 2,275 gm) were repaired using a staged clos

Intestinal dehiscence and mortality in cats undergoing gastrointestinal surgery.

OBJECTIVES: The aim of this study was to determine the incidence of and risk factors for both gastrointestinal (GI) incisional dehiscence and mortality in a large cohort of cats undergoing GI surgery. We hypothesized that cats with preoperative septic peritonitis (PSP), systemic inflammatory response syndrome (SIRS) or sepsis would have higher GI dehiscence and mortality rates than unaffected cats. METHODS: A medical records search identified cats with surgically created, full-thickness incisions into their stomach, small intestines or large intestines. Preoperative data, including signalment
