Congenital femoral deficiency (CFD) or proximal femoral focal deficiency (PFFD) has been a controversial topic to present, describe, classify, and treat. Since 1939, many classifications have been proposed to describe the disease and plan possible treatment - however, no single widely accepted classification. The currently available classification could not fill the gap between description and surgical planning. Some provided a graphical description of the disease with possible treatment plans, yet there is minimal evidence of its reliability and reproducibility. We propose to develop a new cl

Endemic water borne fluorosis is a public health problem in Isparta, a city located in southern Turkey. In order to investigate the association between osteosclerosis and fluorosis, we retrospectively screened the results of lumbar spine and femur neck bone mineral density (BMD) of 1500 patients who were examined before, for any reason in between 2001-2003. Sixty nine patients (67 females and 2 males, mean age 52.6+/-10.2) with vertebra T-scores>or=+2 were found only except a patient with osteoid osteoma in the femur neck (femur T-score+6.64). Thirty-four of the patients could be reexamined wi…

OBJECTIVES: To determine whether flexion improves radiographic visualization of the femoral neck when the femur is externally rotated.
METHODS: Five human femora, with varying neck-shaft and anteversion angles, were measured and immobilized. Degree of flexion required to bring the femoral neck horizontal was measured, varying the rotation. Next, one bone was radiographed in 16 positions, varying rotation in 15 degrees and flexion in 10 degrees increments. Radiographs were presented in randomized blinded fashion to 15 staff radiologists for scoring of femoral neck visualization. Following this

We report two cases of fatigue subcapital fracture of the femur after the removal of the hip plate used for fixation in transtrochanteric rotational osteotomy for osteonecrosis. Two patients, a 42-year-old man and a 43-year-old man, underwent transtrochanteric rotational osteotomy, and bony union was achieved in both patients. However, fatigue subcapital fracture of the femur occurred in both patients 15 months after the removal of the hip plate. Transtrochanteric rotational osteotomy greatly changes the trabecular bone structure in the proximal femur, thus affecting the strength of the femora

The next step to fix the shaft and distal femur fracture was to perform the distal femur locking plate. The EQ5D and Harris Hip Score questionnaires sho …

The femoral neck axis (FNA) is an important reference in femoral neck rotational osteotomy, which is a hip preservation procedure. The purpose of this study was to propose a method for determining the FNA with high accuracy and reliability and to evaluate the effect of FNA determination accuracy on the stress and strain distribution in the proximal femur. Femoral computed tomography data from 50 patients were reconstructed, and the FNA was fitted by the centroid iterative method. The fitting accuracy was evaluated in terms of the distance between the femoral head centre and the FNA. The reliab

INTRODUCTION: Unicameral (simple) bone cysts (UBCs) are fluid-filled intramedullary cavities commonly seen in the metaphysis of long bones in young patients under the age of 20 years. Pathological hip fractures associated with UBCs in the proximal femur are rare, especially in non-osteoporotic elderly patients. The management of such tumors, such as benign lesions of bone causing pathological fractures at the biomechanically critical load-bearing zone of the neck of the femur, remains a clinical challenge due to limited literature on optimal treatment strategies.

BACKGROUND: Conventional cannulated screws are commonly used for internal fixation in the treatment of vertical femoral neck fractures. However, the noticeably high rates of undesirable outcomes such as nonunion, malunion, avascular necrosis, and fixation failure still troubled the patients and surgeons. It is urgent to develop new cannulated screws to improve the above clinical problems. The purpose of this study was to design a novel magnesium-titanium hybrid cannulated screw and to further evaluate its biomechanical performance for the treatment of vertical femoral neck fractures.

Patients with transfemoral amputation (TFA) are up to six times more likely to develop hip osteoarthritis (OA) in either or both the intact and residual limb, which is primarily attributed to habitually altered joint loading due to compensatory movement patterns. However, joint loading patterns differ between limbs, which confounds the understanding of loading-induced OA etiology across limbs. It remains unknown if altered loading due to amputation results in bony shape changes at the hip, which is a known etiological factor in the development of hip OA. Retrospective computed tomography image

OBJECTIVE: To optimize a measuring method of osteonecrotic area by analyzing the average osteonecrotic areas and osteonecritic volume using multi layer MR images.
METHODS: The MRI images of 87 cases (120 hips) of ONFH(ARCO II) were collected retrospectively from January 2011 to January 2012 in Wangjing Hospital of China Academy of Chinese Medicine Science. PHILIPS Achieval 1.5T MR was used to obtain coronal TSE T1 weighted (T1W) images. The scanning parameters were shown as follows:slice thickness, 3.5 mm; gap, 0.3 mm; images repetition time(TR), 500 ms;echo time(TE), 20 ms;field of view (FOV

Quantitative computed tomography and 3 dimensional modeling were used to portray the deformity of the proximal femur in 24 Japanese adults with low subluxations to high dislocations secondary to developmental dysplasia of the hip. Periosteal and canal bony contours were extracted, 3 dimensional models generated, and morphologic parameters were calculated for each femur. Three dimensional illustrations of the average deformity and variability were created. Morphologic parameters were not found to be statistically correlated with the degree of the disease. Interestingly, the major axis of the ca

BACKGROUND AND OBJECTIVE: Medical imaging-based finite element methods are more accurate tools for fracture risk prediction than the traditional aBMD based methods. However, these methods have drawbacks like geometric errors, high computational cost, mesh-dependent results, etc. In this article, the authors have proposed an isogeometric analysis-based nonlocal gradient-enhanced damage model to overcome some of these issues. Moreover, there are uncertainties in the values of input parameters for such analysis due to various measurement errors. Hence, stochastic analysis is performed to quantify

INTRODUCTION: Bipolar hemiarthroplasty (BHA) is a commonly performed procedure for elderly patients with an intra-capsular fracture of the femoral neck. However, surgeons performing BHA worry about the rate of acetabular wear, and acetabular prosthesis protrusion can pose a challenging surgical problem. The number of periprosthetic femur fractures is expected to increase. Generally, well-fixed stems require open reduction and internal fixation (ORIF), whereas loose stems require revision arthroplasty.

OBJECTIVE: To analyze the impact of femoral neck cortical bone defect induced by core decompression on postoperative biomechanical stability using the finite element method.
METHODS: Five finite element models (FEMs) were established, including the standard operating model and four models of cortical bone defects at different portions of the femoral neck (anterior, posterior, superior, and inferior). The maximum stress of the proximal femur was evaluated during normal walking and walking downstairs.
RESULTS: Under both weight-bearing conditions, the maximum stress values of the five models w

It is the role of the hip joint to support the superincumbent body weight while its complex range of movement, under a variety of stresses, allows the individual to undertake a wide range of physical activities. It is then perhaps not surprising that a recent anatomical study shows that the upper femur has a structure commensurate with the complexity of its movements. Mechanically, the upper femur consists of two separate bones: the combined femoral head and neck and the femoral shaft. Both bones consist of a mixture of two different types: cancellous bone and cortical bone, each with its own
