Distribution of fast heart rate episodes during paroxysmal atrial fibrillation.

OBJECTIVE: To investigate the defibrillator waiting time (time between the recognition of atrial fibrillation and the actual shock) by studying paroxysmal atrial fibrillation episodes with RR intervals shorter than a certain limit (that is, episodes during which defibrillation should not be attempted). METHODS: Long term 24 hour Holter recordings from a digoxin v placebo crossover study in patients with paroxysmal atrial fibrillation were analysed. In all, 23 recordings with atrial fibrillation episodes of at least 1000 ventricular cycles and with < 20% Holter artefacts or noise were used (11

Heart rate variability and apnea during sleep in Down's syndrome.

Autonomic system dysfunction has been reported to occur frequently in patients with Down's syndrome (DS) and is constituted mainly by an imbalance between the sympathetic and vagal systems. The analysis of heart rate variability (HRV) during sleep is a quantitative reliable method for studying such a mechanism, but it has not yet been extensively and adequately applied in DS. In this study, HRV during sleep was evaluated in seven DS patients and in six normal controls, by also controlling for the presence of sleep apnea or arousal. The main results were an increased sympathetic function (low-f…

[Heart rate changes in propofol anesthesia--relation with excitatory movements].

Acceleration of heart rate simultaneously with excitatory movements was observed during anesthesia with propofol in four patients. Anesthesia was induced with propofol 2.5 mg.kg-1 i.v. and maintained with propofol 10 mg.kg-1.hr-1 i.v. About ten minutes after the induction, the patients showed jerking movements such as pronation or flexion of their arms, and/or plantar flexion. These movements lasted one or two seconds and appeared two or three times a minute. Three patients received vecuronium 0.1 mg.kg-1 and even after muscular relaxation acceleration of heart rate persisted. In one of two pa…

Tachycardia as the sole fetal heart rate abnormality after funipuncture.

BACKGROUND: Fetal bradycardia is a common complication of funipuncture. We present a case of fetal exsanguination in which fetal tachycardia was the sole fetal heart rate abnormality. CASE: Funipuncture was performed at 32 weeks' gestation for evaluation of Rh isoimmunization. A persistent fetal tachycardia ensued and, although there was no immediate ultrasound evidence of bleeding, repeat ultrasonography revealed active bleeding at the puncture site. A neonate with an initial hematocrit of 42% was delivered by cesarean. Despite aggressive replacement of blood products, a repeat hematocrit wa…

Heart rate variability: technique and investigational applications in cardiovascular medicine.

OBJECTIVE: To describe heart rate variability (HRV) analysis, especially time- and frequency-domain analyses, and some of its investigational applications in clinical cardiovascular medicine. DESIGN: We provide a brief introduction to the magnitude of sudden cardiac death and the factors that influence life-threatening ventricular arrhythmias as a backdrop to the potential importance of the autonomic nervous system and how this system might be assessed by the analysis of HRV. MATERIAL AND METHODS: We reviewed the literature from 1973 to 1994 that described beat-to-beat changes in heart rate,…

Are affective disorders associated with alterations of heart rate variability?

A standardized heart rate analysis was carried out in unmedicated patients with major depression, melancholic type (n = 16), panic disorder (n = 16), reactive depression with suicide attempts during the preceding 24 h (n = 16) and in 16 normal control subjects. The investigations included time- and frequency-derived measurements of heart rate variability. In the patients with reactive depression, no differences could be detected as compared with the control group. The patients with panic disorder showed a significantly increased low-frequency band of spectral analysis (P < 0.01) and a marginal…

Effect of drafting on heart rate in cross-country skiing.

The purpose of this study was to compare the heart rate responses during cross-country skiing as a leading skier, as well as in a drafting situation, 2 m behind the leader. Six male cross-country ski racers, paired for VO2max skied a 2-km course (two loops of 1 km) using the skating techniques at a fixed speed (5.6 m.s-1) on two different occasions, once as a leading skier, the other as a drafter. A recovery period of 30 min was allowed between the two trials. Heart rates (HR) were monitored every 5 s during all performances. Capillary blood lactate concentrations were taken before and at 5 mi…

Fetal heart rate patterns and the timing of fetal injury.

We studied the nonstress test (NST) results and other perinatal features of 44 children with cerebral palsy, who had fetal heart rate (FHR) patterns during labor suggesting preexisting injury. This was a retrospective, descriptive study. All fetuses persistently showed absent variability and small, variable decelerations, with overshoot from the onset of monitoring during labor. During the initial NST, 84.1% of fetuses revealed normal reactive NST patterns (three with decelerations). Six fetuses (15.9%) had nonreactive NST results (three with decelerations). The conversion of the reactive NST …

Should a VVIR pacemaker increase the heart rate with standing?

To assess the usefulness of incorporating a posture sensor into a ventricular inhibited rate modulated pacemaker, the hemodynamic effects of increasing the ventricular pacing rate with standing were studied in 15 pacemaker dependent patients aged 55 +/- 3.5 years. In a randomized cross-over design, the pacing rate remained at 70 or was increased to 100 beats/min immediately prior to standing. Blood pressure was monitored continuously and forearm blood flow was measured by venous occlusion plethysmography. There was no difference in supine blood pressure (117 +/- 4/63 +/- 3 compared to 118 +/-

Amniotic fluid index and prolonged antepartum fetal heart rate decelerations.

Both prolonged fetal heart rate (FHR) decelerations and decreased amounts of amniotic fluid (AF) have been associated with adverse fetal outcome. To determine whether adverse outcome could be predicted by a quantitative assessment of AF in patients with prolonged FHR decelerations, we identified 97 consecutive pregnancies found during antepartum testing to have prolonged FHR decelerations. A four-quadrant AF index was measured concurrently in 92 of these 97 patients. Women with an AF index less than 2 cm had operative intervention for fetal distress in seven of 11 cases (64%), compared with 17…

Heart rate-right ventricular stroke volume relation with myocardial revascularization.

Whether increasing pacing frequency in cardiac surgical patients effectively improves right ventricular cardiac index depends on the interrelationships between heart rate, stroke volume index, and end-diastolic volume index. If an inverse relation exists between heart rate and right ventricular volume then the decrease in right ventricular ejection fraction described after bypass may be due, in part, to changes in heart rate. We evaluated the effects of pacing at 80, 95, and 110 beats/min using a thermodilution volumetric catheter in 16 patients undergoing myocardial revascularization. End-dia…

Influences on heart rate variability in spontaneously breathing preterm infants.

To investigate the influence of maturational and physiological factors on heart rate variability in spontaneously breathing very preterm infants (n = 29) a multiparametric study was performed during the first 3 days of life in infants born at a gestational age below 33 weeks. Four times a day, RR-intervals, respiration curve and rate, transcutaneously measured blood gases and observed body movements were recorded while the infants were asleep. All data were stored simultaneously in a micro-computer. Non-invasively measured blood pressure and patency of the ductus arteriosus were documented as …

[Diagnosis of fetal heart rate without referring to uterine contraction].

The purpose of this study is to evaluate intrapartum fetal condition only with reference to fetal heart rate (FHR) change. Sixty-five cases who had no severe complications were examined at 36 weeks to 42 weeks of gestation. Major cardiotocographic abnormalities were baseline bradycardia and severe variable deceleration. Late deceleration was not observed. The conventional program of automatic FHR diagnosis and a new computer program excluding uterine contraction (UC) algorithm were used. Digitalized FHR and UC data stored in a floppy disk were reproduced and analysed by either of the programs …

Effect of vecuronium on atropine-induced changes in heart rate.

The effect of vecuronium on the heart rate response to atropine has been studied by comparing dose-response relationships in two groups of patients who underwent extracorporeal shock wave lithotripsy. One group received vecuronium (0.1 mg/kg) and the other acted as control. Incremental doses of atropine (1.8, 1.8, 3.6, 7.2 and 14.4 micrograms/kg) were administered and changes in heart rate recorded. No significant differences were observed between the two groups following each incremental dose of atropine.

The relationship between theophylline, caffeine and heart rate in neonates.

Plasma theophylline and caffeine levels were measured in neonates receiving aminophylline for apnoea. Significant levels of caffeine were present in neonates with high plasma theophylline levels and particularly in those children who had been on theophylline for 6 days, or more. Caffeine levels in plasma decreased slowly compared to theophylline. Tachycardia did not occur in any infants even at theophylline values greater than 20 mg/L. For theophylline concentrations above 10 mg/L there was no relationship between heart rate and drug level. Heart rate alone cannot be used to predict toxic drug…

Central body temperature as a guide to optimal heart rate.

Studies in man suggest that fixed-rate artificial pacemakers do not return hemodynamic function to normal, since the principal mechanism for the increase in cardiac output with exercise, increased heart rate, is not restored. Special pacemakers are available that can detect atrial activity and pace the ventricles in coordination, but nearly half of the patients receiving artificial pacemakers have abnormal atrial function (atrial fibrillation, sick sinus syndrome). This study examined the effects of exercise on the temperature of blood returning to the right atrium. Precision thermistors, plac…
