Developmental dysplasia of the hip.

The essential points regarding screening for and detection of developmental dysplasia of the hip are emphasized. Background information is provided to understand why this entity presents in a variable fashion. This article is not intended to comprehensively cover treatment methodology for the orthopedic surgeon. Instead, the information provided is meant to aid the primary care practitioner in identification of the problem and in supporting the family during the treatment process when an abnormality is detected.

Tuberculosis of the hip in children.

We have reviewed 74 tuberculous hips in 73 children treated from 1950 to 1991. From 1979 to 1991 we treated 28 patients with rifampicin, isoniazid and pyrazinamide given for nine months (series A), using active mobilisation for the more recent cases. Before this, 46 hips had been treated with streptomycin and isoniazid with or without para-aminosalicyclic acid given for a mean of 18 months (series B), and all these patients were immobilised for a mean of 2.2 years. The radiological appearances at presentation as classified by Shanmugasundaram (1983) predicted the outcome. Most hips were of the…

Hip in infants and children.

When the clinical picture is unclear, ultrasound determination of the presence or absence of an effusion can guide the clinician in the diagnosis and need for further evaluation. Sonography can be used to guide arthrocentesis, thus avoiding the ionizing radiation accompanying fluoroscopically guided aspiration.

Congenital dislocation of the hip.

Neonatal hip dysplasia remains a difficult condition to diagnose and treat. Early periodic infant hip examination is the primary screening tool, but subtle hip instabilities can go undetected until fixed dislocations are discovered when the child begins to walk. Early nonoperative intervention in the neonatal period is usually effective using a Pavlik-type harness. For infants in whom this disorder is discovered after walking age, surgery is often needed to successfully reduce the hip. Radiographic evaluation of neonatal hip dysplasia is often difficult, but the evolving use of ultrasound, com…

Developmental dysplasia of the hip.

OBJECTIVE: The definition and early treatment of congenital dysplasia of the hip are controversial. The purpose of this study was to discuss the reasons for changing the acronym to developmental dysplasia of the hip (DDH) and to address its early detection and treatment. DESIGN: This multicenter study was designed to provide an updated assessment of the definition, pathologic anatomy, prevalence, etiology, natural history, early detection, and treatment of DDH. RESULTS: DDH more accurately describes the condition previously termed congenital dysplasia of the hip. The disorder is not always p…

Synovial schistosomiasis of the hip.

Schistosomiasis is a common parasitic infestation in Egypt. We describe the case of a 24-year-old Egyptian man who presented with the signs of acute septic arthritis of the hip and in whom biopsy subsequently revealed schistosome ova in the synovium.

[Capsular retraction of the hip].

The diagnosis of capsular retraction of the hip is based on measurement of the capacity of the joint cavity on arthrography: it is reduced by at least 25% (normally 15 ml +/- 2 ml). The opaque area is only visible reduced in the more severe forms with a capacity of 5 ml or less. The arthrographic image is therefore not the key to the diagnosis. The major clinical sign is restriction of joint movement, especially in abduction and rotation. Secondary, "surgical" capsular retraction of the hip is the most common form. It is associated with synovial chondromatosis in more than one half of cases. T…

The myelodysplastic hip and scoliosis.

To investigate the relationship between unilateral hip dislocation or subluxation and scoliosis, the records were examined of 31 patients aged 10 to 26 years with myelomeningocele and an initial neurosegmental level of L3 or L4. 11 patients had deteriorated neurologically over time; the remainder had stable neurological examinations, with an average follow-up of 14.3 years. Scoliosis occurred in three of the nine patients with unilateral hip instability, compared with five of the 11 remaining patients. Only two patients had the expected pattern of unilateral hip instability on the high side of…

Transient osteoporosis of the hip.

Transient osteoporosis of the hip (TOH) is an uncommon but possibly underdiagnosed condition. This pattern of regional osteoporosis affects previously healthy middle-aged men, and women in the third trimester of pregnancy. Its etiology is still unclear, yet in view of similarities to regional migratory osteoporosis and reflex sympathetic dystrophy, vascular and neurologic disturbances have been proposed as the possible pathogenetic mechanisms. Pain in the hip area and functional disability of the affected limb are the main clinical signs. Diagnosis is supported by local radiological osteopenia…

[Tendinitis of the hip region].

Tendinitis and bursitis are less common around the hip than around the shoulder. Nevertheless, they must be recognized to avoid unnecessary and costly diagnostic errors. Their various clinical forms are studied in detail. Tendino-bursitis of the gluteus medius muscle is the most frequent in its subacute form, but it is rare in its acute, pseudo-gouty form. Calcification of the reflected tendon of the rectus femoris muscle often closely resembles arthritis of the hip. Synovial cysts of the psoas bursa and rupture of the gluteus medius tendon are rare but must be known. Local injections of corti…

Treatment of the irreducible hip.

"Irreducible" congenital dislocation of the hip should not be regarded as a special pathological entity. Thirty-eight cases with a previous and failed treatment attempt were reviewed. Recognition of the renal cause of "irreducibility" (iatrogenic or anatomopathological) and institution of an appropriate treatment protocol result in adequate reduction in the vast majority of cases. However, osteochondritis is not an uncommon finding when the results of secondary treatment are evaluated. The percentage of more severe cases is higher after open procedures.

[Sonography of the hip in infants].

Because of technical reasons it is rather difficult to differ Typ IIa (+) and Typ IIa (-) before the 6. week alive. In hip sonography there must be decided between dynamic examination and stress-examination. Hip instability can be objectived sonographically by measurement. Only newborn-screening by sonography with consecutive adequate therapy can improve the results remarkable.

Imaging of the pelvis and hip.

Imaging is basic in the orthopedic evaluation of hip disorders. For optimal evaluation of these images, the orthopedic surgeon must not only correlate normal anatomy and pathology with radiographic findings, but must also be familiar with the newer modalities of CT, magnetic resonance imaging and ultrasound and the role and place of these studies in the evaluation of musculoskeletal disorders. This article correlates normal anatomy of the hip with these modalities and discusses the details of these techniques, particularly CT and MRI, that are pertinent to the orthopedic surgeon.

[Sonography of the hip in infants].

Efficient hip-sonography requires adequate expertise of both patho-anatomy and technique. There are several problems concerning selection of patients, timing of the examination, technique, analysis of findings. The goal is early diagnosis of inhibited maturation or of dislocation of the hip-joint in order to begin treatment - if indicated - as early as possible. Thus therapy may be shortened and it gets less invasive. In future no gross hip-pathology should be detected for the first time after the age of three months.

Hemiarthroplasty of the hip and dislocation.

A retrospective study was undertaken on 171 patients who had suffered a subcapital fracture of the neck of the femur between 1982 and 1986. The aim was to compare the dislocation rates in bipolar and monopolar endoprostheses. The basic surgical approaches were utilized: the posterior and direct lateral. Four endoprostheses were employed: Thompson's Moore's (monopolar), Monk's 'hard top', and Charnley-Hastings' (bipolar). There was no significant difference in the dislocation rates between monopolar and bipolar devices. It is concluded that the bipolar devices do not offer any additional protec

Idiopathic chondrolysis of the hip.

Fourteen patients (16 hips) with idiopathic chondrolysis of the hip were retrospectively studied at the Shriners Hospital from 1973 to 1986. Follow up averaged 84 months (range 13-180 months). All 14 patients were female. All but one were premenstrual. Each patient presented with an insidious onset of pain in the hip, thigh, or knee and radiographic symmetrical joint space narrowing. Three of the 16 hips required fusions, and only five regained a full range of motion. Partial restoration of the joint space occurred in eight hips. Essential treatment consists of unloading the hip joint while ma…
