[Metabolic disorders in the treatment of malignant hematologic diseases--the acute tumor lysis syndrome].

The acute tumor-lysis syndrome is a potentially fatal complication which characteristically arises during initial chemotherapy of malignant hematological diseases with large tumor burdens. The syndrome is characterized by hyperphosphatemia, hypocalcemia, hyperuricemia and often acute renal failure. Prior to chemotherapy the patient should be treated for 12-24 hour with intensified diuresis to ensure optimal renal function. The treatment of the fully developed syndrome is hemodialysis. Three cases of TLS which developed during initial chemotherapy of patients with acute lymnphoblastic leukemia …

Pathogenesis of hypocalcemia in neonatal polycythemia.

Hypocalcemia (HC) is a frequent complication of neonatal polycythemia (NP): 9.93% in the present study. Its pathogenesis is not clear. We suggest that one of the main pathogenetic explanation of HC in NP is the high level of phosphate significantly different (P less than 0.01) than serum phosphate of normal normocalcemic non-polycythemic newborns. The intense postnatal erythrocyte breakdown will increase the serum level of phosphate. This hyperphosphatemia will decrease calcium ion concentration by their combination and bone deposition, a process allowed by inadequate postnatal parathyroid gla…

Complex biochemical syndrome of hypocalcemia and hypoparathyroidism during cytotoxic treatment of an infant with leukemia.

Patients with cancer are especially vulnerable to the development of hyper- and hypocalcemia. These disorders may be the basic clinical manifestation of cancer or may result from vigorous cancer treatment. During cytotoxic treatment of a child with leukemia we describe the longitudinal course of ionized calcium in a complex syndrome of hypocalcemia and hypoparathyroidism. The results suggest that the development of severe hypocalcemia can be explained on the basis of hypoparathyroidism in relation to hyperphosphatemia, hypomagnesemia, and acute renal failure. The low ionized calcium was unsucc…

Hyperphosphatemic Tumoral Calcinosis: Pathogenesis, Clinical Presentation, and Challenges in Management.

Hyperphosphatemic familial tumoral calcinosis (HFTC) is a rare and disabling disorder of fibroblast growth factor 23 (FGF23) deficiency or resistance. The disorder is manifest by hyperphosphatemia, inappropriately increased tubular reabsorption of phosphate and 1,25-dihydroxy-Vitamin D, and ectopic calcifications. HFTC has been associated with autosomal recessive pathogenic variants in: (1) the gene encoding FGF23; (2) GALNT3, which encodes a protein responsible for FGF23 glycosylation; and (3) KL, the gene encoding KLOTHO, a critical co-receptor for FGF23 signaling. An acquired autoimmune for…

Hypervitaminosis D has no positive effects on goat tuberculosis and may cause chronic renal lesions.

BACKGROUND: There is evidence for a link between vitamin D deficiency and active tuberculosis (TB). In human beings, several trials have evaluated the role of vitamin D supplementation in TB treatment with conflicting results. However, the role of vitamin D supplementation in animal TB control has received less attention. The authors evaluated the benefit of vitamin D supplementation for preventing mycobacterial infection or reducing TB lesions (TBL) in a controlled trial with goats naturally exposed to Mycobacterium caprae. METHODS: Two groups of goats, a vitamin D-supplemented group and a no

Is chronic kidney disease-mineral bone disorder (CKD-MBD) really a syndrome?

The concept of chronic kidney disease-mineral bone disorder (CKD-MBD) does not appear to fulfil the requirements for a syndrome at first glance, but its definition has brought some clear-cut benefits for clinicians and patients, including wider and more complex diagnostic and therapeutic approaches to the management of this challenging set of issues. Admittedly, not all components of CKD-MBD are present in all patients at all times, but these are highly interrelated, involving mineral and bone laboratory abnormalities, clinical and histological bone disease and finally, cardiovascular disease.

Coexistence of chronic renal failure, hashimoto thyroiditis and idiopathic hypoparathyroidism: a rare case report.

Hypoparathyroidism is an uncommon disease and its coexistence with chronic renal failure is quite rare. Hypocalcemia and hyperphosphatemia are seen in both diseases. Diagnosis of hypoparathyroidism may be overlooked when parathormone response is not evaluated in patients with chronic renal failure. A 19-year-old female patient who had been receiving hemodialysis for 3 years because of chronic renal failure was diagnosed as idiopathic hypoparathyroidism and hashimoto thyroiditis. When her medical records on the first admission and medical history were evaluated, hypoparathyroidism and hashimoto…

[Mineral and bone disorders in chronic heart failure].

In many patients, chronic heart failure (CHF) is associated with chronic kidney disease (CKD). Virtually all patients with terminal CKD and many patients with early CKD display various disorders of mineral and bone metabolism (MBM) related with all-cause mortality and high risk of cardiovascular complications. This review addressed disorders of mineral and bone metabolism in patients with CHF, including hypocalcemia, hyperphosphatemia, vitamin D insufficiency/deficiency, secondary hyperparathyroidism, changed levels of FGF23 and Klotho, osteoporosis, osteopenia, their clinical and prognostic s…

[Albright's hereditary osteodystrophy: report of three cases].

Albright's hereditary osteodystrophy includes an heterogeneous group of genetic diseases, including the pseudohypoparathyroidism type Ia and pseudo-pseudohypoparathyroidism. Features of this disease are: round faces, over-weight, subcutaneous calcifications and brachydactylia. Pseudohypoparathyroidism type Ia is associated with laboratory abnormalities such as hypocalcemia, and hyperphosphatemia due to parathormone resistance. Furthermore, these patients may present peripheral resistance to other hormones, such as gonadotrophins and thyroids. Pseudo-pseudohypoparathyroidism has no peripheral r…

[Novel phosphate binders under development].

Controlling serum phosphorus levels in patients with chronic kidney disease is crucial. Since hyperphosphatemia is an established risk factor for not only secondary hyperparathyroidism, but also cardiovascular mortality in these patients, the adequate control of serum phosphorus is necessary. The target levels of serum phosphorus, however, are not consistently being met. To achieve better control of serum phosphorus levels, some novel phosphate binders, which do not contain calcium or metal, have been developed. In this manuscript we discuss some novel phosphate binders, which are now in clini…

Severe calcinosis cutis associated with treatment of hypoparathyroidism in a dog.

A 6-month-old, female border collie was referred for evaluation of hypocalcemia, hyperphosphatemia, fever, and painful ventral abdominal skin. She had recently been treated intravenously and subcutaneously (SC) with a diluted 10% calcium gluconate solution. The medical evaluation supported the diagnosis of primary hypoparathyroidism, but the subsequent hospital course was complicated by severe calcinosis cutis, which caused extensive skin necrosis and marked debilitation. This patient illustrates that administration of a calcium gluconate solution SC can be associated with extensive morbidity …

Hypoparathyroidism in a patient with long-chain 3-hydroxyacyl-coenzyme A dehydrogenase deficiency caused by the G1528C mutation.

Mitochondrial trifunctional protein (MTP), an enzyme complex participating in fatty acid beta-oxidation, is the potential site of two documented defects: long-chain 3-hydroxyacyl-coenzyme A dehydrogenase (LCHAD) and MTP deficiencies. LCHAD deficiency usually manifests as hypoglycemia, with hepatopathy, hypotonia, cardiomyopathy, and retinopathy. Hypoparathyroidism has been detected in a patient with MTP deficiency. We now report on a patient with LCHAD deficiency and hypoparathyroidism, evidenced by hypocalcemia, hyperphosphatemia, and a low level of parathyroid hormone, in whom the parathyroi…

Childhood calcinosis cutis.

Calcinosis cutis, an uncommon disorder characterized by hydroxyapatite crystals of calcium phosphate deposited in the skin, has been described infrequently in childhood. Classically, it is divided into dystrophic, metastatic, and idiopathic types. We report an 8-year-old girl with hyperphosphatemia secondary to a tumor lysis syndrome, who developed a localized soft tissue calcification over a previous lesion of ecthyma gangrenosum. Intravenous infusion of calcium gluconate was probably the precipitating factor. Our case illustrates that several etiopathogenic mechanisms may be simultaneously i…

[Treatment of renal osteodystrophy: physiopathology and secondary effects].

The prophylaxis and treatment of renal osteodystrophy are based on pathophysiological principles. Development of secondary hyperparathyroidism should be averted by early prevention of hyperphosphatemia through diet and phosphate ligants, and by normalization of the calcium balance through calcium supplements and vitamin D or its analogues. This treatment requires close clinical and laboratory control in order to avoid several hazards (hypercalcemia, hypophosphatemia, and refractory constipation). In cases with severe secondary hyperparathyroidism, subtotal parathyroidectomy is sometimes requir…

[Acquired pseudohypoparathyroidism/(PHP) in adult coeliac disease (MCA)].

One case of chronic hypocalcemia and hyperphosphatemia, with an adult coeliac disease, hight P.T.H. level, very low rate of cyclic A.M.P. in urine, and normal rate of cyclid A.M.P. in plasma is described. It is the question of the pseudohypoparathyroidism acquirec as a result of an adult coeliac disease, because of after seven months of diet without gluten the clinical and biological improvement was observed with the increase of the cyclic-AMP clearance into the normal level and its great reactivity on the administration of exogenous PTH. We think to bring an additional analysis of the trouble…

[Bone metabolic disorders after prolonged treatment with antiepileptic agents in a patient with idiopathic hypoparathyroidism].

A case is reported of a patient, aged 20, with idiopathic hypoparathyroidism, manifested with the picture of epilepsy with a long treatment with antiepileptic remedies. Two years after the initiation of the antiepilertic treatment, severe skeleton disturbances occurred with manifested osteoporosis and multiple symmetric zones of bone reconstruction, hypocalcemia, hyperphosphatemia and elevated serum alkaline phosphatase. The disturbances of calcium and phosphorus level in blood are favourably affected by the application of relatively high vitamin D doses. The role of the long-term anticonvulsi…

A Quick Reference on Phosphorus.

Phosphorus, or phosphate, is the body's major intracellular anion involved in numerous biological processes. Most phosphate is intracellular, with the remaining amount contained within soft tissues and the extracellular space. Parathyroid hormone, calcitriol, calcitonin, and phosphatonins regulate normal phosphate homeostasis by adjusting renal and/or gastrointestinal absorption and/or excretion. Hypophosphatemia occurs secondary to decreased gastrointestinal absorption, transcellular shifts, increased renal excretion, or some combination of these general mechanisms. Hyperphosphatemia results …
