PURPOSE: To describe a case of keratoconus and Fuchs' corneal endothelial dystrophy in the left eye with no corneal disease in the right eye.
METHODS: A 64-year-old woman presented with visual impairment in her left eye; computer-assisted topographic analysis and specular microscopy were performed in both eyes and left cornea was histopathologically examined.

PURPOSE: To study the results of penetrating keratoplasty for keratoconus analyzing the differences in ultimate spherical equivalent according to graft size and the trephine used in the recipient.
MATERIAL AND METHODS: Retrospective study of 38 patients that were divided into three groups. Group A is composed of patients with a 0.50 mm donor-recipient difference, 0.25 mm in group B and same-sized in group C. A 53-year-old man with keratoconus and an axial length of 32.59 mm had cataract extraction by phacoemulsification. The Holladay II formula called for -14.00 diopters (D) of power. Two negative-power intraocular lenses (IOLs) were implanted to optimize visual results. A 1 day postoperative refraction of +1.50 D sphere necessitated an exchange of the anterior IOL. Six days after the exchange, the patient had a refraction of -1.25 D sphere and best corrected visual acuity of 20/50.

PURPOSE: To develop a videokeratoscopic-based keratoconus detection scheme that avoids the ambiguity of dioptric power definitions and videokeratoscope design.
METHODS: Corneal height data obtained with a commercial videokeratoscope are decomposed into the set of orthogonal Zernike polynomials. Expansion coefficients of a "normal" group and a keratoconus group are compared to find significant differences. Elevated Zernike terms are used to detect the disease in these populations. We have observed spontaneous development of striae and an enhanced visibility of them after placing rigid gas permeable contact lenses on six patients suspected of having early keratoconus. These fine folds subside upon removal of the rigid lens. The levels of alpha 1-proteinase inhibitor (alpha 1-antitrypsin) in keratoconus, normal human, and other diseased corneas were examined.
