Lumbar Apophyseal Ring Fracture With Central Disc Herniation: A Case Report.

Lumbar apophyseal ring fracture (LARF) with central disc herniation is a rare but important cause of neurological symptoms in adolescent athletes, often presenting diagnostic and therapeutic challenges. We report the case of a 14-year-old male professional tennis player who developed persistent bilateral lower extremity tightness and radiating sensations after intensive training, unresponsive to initial physiotherapy. MRI revealed an S1 apophyseal ring fracture with central disc herniation. With surgery declined, the patient underwent a multimodal chiropractic protocol, including targeted spin

Intra-observer reliability study of lumbar segmental measurements utilizing ultrasonography.

BACKGROUND: Spinal Manipulative Therapy (SMT) benefits LBP patients, but its mechanism is not well documented. One hypothesis indicates that SMT restores interspinal movements. Ultrasound measurement (UM) of spinous process separation (SPS) assesses the intersegmental movements. METHODS: We used the test-retest reliability of UM between the L3, L4, L5, and S1 spinous processes on fifteen asymptomatic volunteers while lying prone on a chiropractic table. The participants then walked around for over 5 minutes, and ultrasound images were reperformed prone. UM identified the tips of the spinous p

Epidemiology, etiology, diagnostic evaluation, and treatment of low back pain.

Low back pain continues to affect a significant proportion of the younger, working population between 35 and 45 years of age. An important study has correlated macroscopic and microscopic intervertebral disc alterations starting in the second decade of life with oxidative stress manifested by the presence of N-(carboxylmethyl)lysine. Job satisfaction remains a strong predictive factor for the identification of individuals with acute back pain who will develop chronic pain. Patients with pyogenic vertebral osteomyelitis may have an increase in their erythrocyte sedimentation rate during the fir

Optimal time for manipulation of frozen shoulder may be between 6 and 9 months.

BACKGROUND AND AIMS: Idiopathic frozen shoulder can be successfully treated conservatively. Manipulation under anesthesia, in randomized controlled studies, has proven to bring no additional benefit to other treatments. However, manipulation under anesthesia still is a used method mainly among patients with unbearable pain or too longstanding restriction of shoulder motion. Manipulation under anesthesia may lead to significant improvement in shoulder motion and function. However, the best timing for manipulation under anesthesia is still unclear. We therefore investigated whether timing of man

Effect of manual manipulation on mechanical gait parameters.

CONTEXT: A variety of manual manipulation techniques are utilized in clinical practice to alleviate pain and improve musculoskeletal function. Many manual practitioners analyze gait patterns and asymmetries in their assessment of the patient, and an increasing number of gait motion capture studies are taking place with recent improvements in motion capture technology. This study is the first systematic review of whether these manual modalities have been shown to produce an objectively measurable change in gait mechanics. OBJECTIVES: This study was designed to perform a systematic review of th

Secondary causal mediation analysis of a pragmatic clinical trial to evaluate the effect of chiropractic care for US active-duty military on biopsychosocial outcomes occurring through effects on low back pain interference and intensity.

OBJECTIVE: We evaluate change in low back pain (LBP) intensity and interference as the mechanism by which chiropractic care affects other biopsychosocial factors in US active-duty military members. DESIGN: We conducted secondary, exploratory mediation analysis of pragmatic, multisite, clinical trial (NCT01692275) post results using natural effect modeling. Mediators were the 6-week values of Patient-Reported Outcomes Measurement Information System (PROMIS)-29 pain interference and intensity. Outcomes were 12-week values of other PROMIS-29 biopsychosocial subdomains. Models evaluated overall a

Treatment Patterns and Population Characteristics of Nonpharmacological Management of Chronic Pain in the United States' Medicare Population: A Scoping Review.

BACKGROUND AND OBJECTIVES: Clinical practice guidelines recommend noninvasive nonpharmacological pain therapies; however, reviews that assess the literature pertaining to nonpharmacological pain management among older adults and people with long-term disabilities who are disproportionately affected by pain are lacking. This scoping review aimed to systematically map and characterize the existing studies about the receipt of noninvasive, nonpharmacological pain therapies by Medicare beneficiaries. RESEARCH DESIGN AND METHODS: We conducted a literature search in MEDLINE (PubMed), CINAHL (EBSCO)

Database coverage and their use in systematic reviews regarding spinal manipulative therapy: an exploratory study.

BackgroundSystematic reviews (SRs) of randomized controlled trials (RCTs) are considered one of the most reliable study types. Through a systematic and thorough literature search, researchers aim to collect all research relevant to their purpose. The selection of databases can be challenging and depend on the topic of interest. The Cochrane Handbook suggests searching at least the following three databases: Cochrane Library, MEDLINE, and EMBASE. However, this is not always sufficient for reviews on the musculoskeletal field in general. This study aimed to examine the frequency and choice of da

Spinal manipulation for primary and secondary dysmenorrhoea.

BACKGROUND: Dysmenorrhoea refers to the occurrence of painful menstrual cramps of uterine origin and is a common gynaecological condition. One possible treatment is spinal manipulation therapy. One hypothesis is that mechanical dysfunction in certain vertebrae causes decreases spinal mobility. This could affect the sympathetic nerve supply to the blood vessels supplying the pelvic viscera, leading to dysmenorrhoea as a result of vasoconstriction. Manipulation of these vertebrae increases spinal mobility and may improve pelvic blood supply. Another hypothesis is that dysmenorrhoea is referred p

Improved Chronic Low Back Pain, Radiographic Alignment, and Patient Reported Outcomes Following Postural Rehabilitation Protocols: A Case Series of Two Patients with 18- and 26-Months Follow-Up.

Background/Objectives: We describe a case series of two patients with non-specific chronic low back pain (CLBP) and measurable decreased quality of life, who showed improvements after a specific multi-modal conservative spine and postural therapy regimen. CLBP is the leading cause of years lived with disability and disability-adjusted life years. This case series adds observational data to the medical literature on conservative treatment of CLBP and potentially improves diagnostic and treatment understanding of how conservative therapies can benefit patients suffering with CLBP. Methods: Two p

Pain, Progress, and Price: A Review of Conservative and Complementary Treatments for Low Back Pain.

Low back pain (LBP) is a leading cause of global disability, with a 7.2% prevalence and up to 80% lifetime risk, substantially impairing functionality and imposing a major economic burden. This review evaluates the cost-effectiveness trade-offs between pharmacological and non-pharmacological therapies for non-specific LBP, drawing on recent, high-quality evidence from a comprehensive PubMed search. Evaluated approaches included physical therapy, pharmacological management, cognitive behavioral therapy (CBT), chiropractic manipulation, acupuncture, yoga, and massage therapy. Non-steroidal anti-
