BACKGROUND AND PURPOSE: The purpose of this case report is to describe a multimodal pain therapeutic concept including the adjunct use of pregabalin in a 4.5 year-old child after forefoot amputation. Phantom limb pain and sensation is a complex pain syndrome that is difficult to treat and prevent. 70-75% of all children develop such a pain syndrome after amputation. We describe here a paediatric patient who underwent forefoot amputation following traumatic foot injury and received multimodal pain therapy including pregabalin.
METHODS: A 4.5 year-old otherwise healthy girl suffered severe inju…

Tactile sensory feedback plays a key role in accomplishing the dexterous manipulation of prosthetic hands for the amputees, and the non-invasive transcutaneous electrical nerve stimulation (TENS) of the phantom finger perception (PFP) area would be an effective way to realize sensory feedback clinically. In order to realize the high-spatial-resolution tactile sensory feedback in the PFP region, we investigated the effects of electrode size and spacing on the tactile sensations for potentially optimizing the surface electrode array configuration. Six forearm-amputated subjects were recruited in

Chronic pain is usually underestimated in children, due to lack of knowledge and its specific signs. In addition to suffering, chronic pain … llows rapid implementation of intensive physiotherapy. These techniques have also shown interest in phantom limb pain after limb amputation, but they remain controversial for erythromelalgia pain or chronic abdominopelvic pain. Finally, the treatment of postdural puncture headache due to cerebrospinal fluid leak can be treated by performing an epidural injecti …

The immediate and long-term outcome of a mutilating hand injury can be positively influenced by health care professionals adopting a biopsychosocial perspective toward treatment and management. Such an injury produces a psychological and social impact that should be openly and candidly addressed with the injured individual and with the family. The earlier and the more skillfully these issues are addressed, the more likely it is that psychological factors will not impede functional outcome.

Movement disorders post-amputation are a rare complication and can manifest as the jumping stump phenomenon, a form of peripheral myoclonus. The pathophysiology remains unknown and there is currently no standardized treatment. We describe the case of a 57-year-old male with unremitting stump myoclonus, starting one month after transtibial amputation, in his residual limb without associated phantom or neurological pain. The consequence of the myoclonus was a reduction in prosthetic wearing time. Failure to respond to oral medication led us to attempt the use of botulinum neurotoxin Type A injec

This report describes a study of patients with hands rendered anesthetic by stroke or neurosurgery. Touching the normal hand of such patients triggers sensations referred contralaterally to the anesthetic hand, paralleling observations of sensory referral to phantom limbs of amputees. The referred somatic sensations are elicited by touch but not usually by other kinds of stimuli, cannot be localized precisely, and do not support spatially organized perception. These characteristics suggest that referral may depend on reorganization in parietal cortical areas other than area 3b, the primary cor…

Fixed dystonia of limb is a rare and complex disorder characterized by abnormal, fixed posturing of a limb, often accompanied by chronic pain. Its etiology remains controversial, with debates surrounding psychogenic versus organic origins. We present the case of a 22-year-old female with a longstanding history of left foot deformity and progressively worsening neuropathic pain, which persisted despite multiple pharmacological treatments, physiotherapy, and psychological interventions. The patient developed severe psychological distress, including suicidal ideation and a self-amputation attempt

This study describes the injuries, complications, functional limitations, and rehabilitative needs of amputees managed at Walter Reed Army Medical Center during the Persian Gulf conflict (1991). Fourteen amputees were treated sustaining 21 amputations with 18 lower-limb and 3 upper-limb amputations. In six casualties there were multiple amputations. Battle injuries were the cause in 79%. Nerve injuries occurred in 79%, phantom pain in 64%, and functional limitations (ambulation and activities of daily living [ADLs]) were present in all casualties. Contractures were noted in 86%. Skin traction

PURPOSE: To evaluate in children the agreement between femoral and tibial torsion measurements obtained with low-dose biplanar radiography (LDBR) and CT, and to study dose reduction ratio between these two techniques both in vitro and in vivo.
MATERIALS AND METHODS: Thirty children with lower limb torsion abnormalities were included in a prospective study. Biplanar radiographs and CTs were performed for measurements of lower limb torsion on each patient. Values were compared using Bland-Altman plots. Interreader and intrareader agreements were evaluated by intraclass correlation coefficients.

Phantom limb pain, a common sequela in lower-limb amputees, impairs their ability to accurately perceive and distinguish between haptic and pain sensations, thereby hindering residual limb function and reducing rehabilitation efficacy. Although existing prosthetic technologies seek to restore sensory function, they often fail to provide real-time localization of haptic stimuli and effectively integrate pain feedback, resulting in an inability to deliver cohesive tactile and nociceptive cues. Here, we present a bioinspired perceptual sensor (BPS) capable of spatiotemporally decoding haptic and …

Mirror visual feedback was originally devised as a therapeutic tool to relieve perceived involuntarily movements and paralysis in the phantom limb. Since this pioneering work was conducted in the mid-1990s, the technique has been applied to relieve pain and enhance movement in other chronic conditions such as stroke and complex regional pain syndrome (CRPS) type 1. This review describes how mirror visual feedback was first developed with amputees, its original application in CRPS, and how further research has demonstrated its potential benefit within graded motor imagery programs. We discuss t…

Upper-limb prosthesis control remains challenging in achieving natural and intuitive movements, especially for devices with multiple actuated degrees of freedom (DoFs), often demanding high cognitive effort. Machine learning aids in mapping phantom limb muscle patterns to prosthetic movements, but is limited by the instability of electromyographic signals over time. This study investigates two simultaneous and proportional myocontrol strategies, based on position and velocity, using incremental learning for a 3-DoFs prosthesis, allowing co-adaptation between the system and the user. Six able-b…

Radiofrequency and cryoneurolysis are promising options for managing chronic and acute pain. These minimally invasive techniques target nerve structures to alleviate pain in various conditions. This narrative review aims to provide an overview of the historical development, current techniques, applications, and existing evidence for radiofrequency and cryoneurolysis in pain management. While research is ongoing, evidence suggests their effectiveness in treating conditions like postmastectomy pain, phantom limb pain, radicular pain, and trigeminal neuralgia. However, their adoption in clinical …

Dorsal column stimulators (DCS) have been implanted in 130 patients with various chronic pain syndromes at the University of California, San Francisco, between 1969 and 1973. Preoperative psychiatric evaluation and percutaneous dorsal column stimulation testing were of value in rejecting those patients most likely to have unsatisfactory long-term results with DCS. Best results occurred in patients with phantom limb or peripheral nerve pain and worst results in patients with paraplegic pain, documented arachnoiditis, pancreatitis and arthritis. The need is stressed for careful preoperative sele…
