Integrating a Parenting Assessment into Practice: Pediatric Providers' Time and Perspectives.

PURPOSE: To integrate a parenting assessment into primary care and assess pediatric providers' time needed to review it and their perceptions of the process. DESCRIPTION: The Quick Parenting Assessment (QPA) is a validated, 13 item parent support tool that assesses for healthy and unhealthy parenting practices. Higher QPAs indicate more unhealthy parenting being used. In a clinic serving low-income parents, the QPA was integrated into the 15 month, 30 month, 5 year, and 8 year well child visits. After each well child visit in which the QPA was administered, providers were invited to complete

Smartwatch assessment of CPR performance administered to an instrumented mannequin.

Cardiac arrest is a life-threatening emergency that requires effective cardiopulmonary resuscitation (CPR) to preserve life. Smartwatches equipped with accelerometer sensors and worn close to the wrist joint may offer an accessible way to provide CPR feedback. This study determined whether accelerometer data from a smartwatch could provide valid estimates of CPR parameters, including compression, decompression, frequency, and cycle time. Nineteen medical school students performed CPR on an instrumented mannequin while wearing a smartwatch. CPR parameters were derived from the smartwatch’s acce

Quality indicators for the diagnosis and management of chronic rhinosinusitis.

BACKGROUND: Chronic rhinosinusitis (CRS) has been identified as a high-priority disease category for quality improvement. To this end, this study aimed to develop CRS-specific quality indicators (QIs) to evaluate diagnosis and management that relieves patient discomfort, improves quality of life, and prevents complications. METHODS: A guideline-based approach, proposed in 2012 by Kötter et al. was used to develop QIs for CRS. Candidate indicators (CIs) were extracted from 3 practice guidelines and 1 international consensus statement on the diagnosis and management of CRS. Guidelines were eval

Development of a core outcome set for use in determining the overall success of gastroschisis treatment.

BACKGROUND: Gastroschisis research is limited in quality by the presence of significant heterogeneity in outcome measure reporting (PloS One 10(1):e0116908, 2015). Using core outcome sets in research is one proposed method for addressing this problem (Trials 13:103, 2012; Clin Rheumatol 33(9):1313-1322, 2014; Health Serv Res Policy 17(1):1-2, 2012). Ultimately, standardising outcome measure reporting will improve research quality and translate into improvements in patient care. METHODS/DESIGN: Candidate outcome measures have been identified through systematic reviews. These outcome measures w

Assessing health status and quality of life of Q-fever patients: the Nijmegen Clinical Screening Instrument versus the Short Form 36.

BACKGROUND: The aim of the study was to assess the use of the Nijmegen Clinical Screening Instrument (NCSI) and Short Form 36 (SF-36) in providing a detailed assessment of health status of Q-fever patients and to evaluate which subdomains within the NCSI and SF-36 measure unique aspects of health status. FINDINGS: Patients received a study questionnaire, which contained the NCSI and SF-36. Pearson correlation coefficients between subdomains of the instruments were calculated. The response rate was 94% (309 out of 330 eligible patients). Intercorrelations between subdomains of the NCSI were ge

Adding voucher-based incentives to community reinforcement approach improves outcomes during treatment for cocaine dependence.

This study compares the efficacy of the Community Reinforcement Approach (CRA) with and without an incentive program for cocaine-dependent patients in Spain. A total of 58 patients were randomly assigned to the CRA or CRA plus vouchers condition. In the CRA plus vouchers group, mean percentage of cocaine-negative samples was 97.07%, versus 79.76% in the no-voucher group. Those treated in the CRA plus vouchers condition also achieved greater improvements in psychosocial functioning than those treated in the CRA condition. The present results show that treatment outcome is better if incentives a

Optimised electronic patient records to improve clinical monitoring of people living with HIV (MONART trial) intervention: a qualitative process evaluation.

BACKGROUND: Viral load monitoring systems enable early detection of antiretroviral therapy failure, allowing for timely interventions. Systemic gaps and infrequent viral load monitoring contribute to delayed identification of antiretroviral treatment failure, increased risk of drug resistance, and poorer health outcomes for people living with HIV. METHODS: We conducted qualitative research to assess the delivery and quality of the MONART Trial intervention, which utilised a VL champion model comprising an upgraded TIER.net platform and a quality improvement process. The upgraded TIER.net plat

A model to facilitate implementation of the International Classification of Functioning, Disability and Health into prosthetics and orthotics.

BACKGROUND: The International Classification of Functioning, Disability and Health is a classification of human functioning and disability and is based on a biopsychosocial model of health. As such, International Classification of Functioning, Disability and Health seems suitable as a basis for constructing models defining the clinical P&O process. The aim was to use International Classification of Functioning, Disability and Health to facilitate development of such a model. Proposed model: A model, the Prosthetic and Orthotic Process (POP) model, is proposed. The Prosthetic and Orthotic Proce

A preventive program for the newly separated: initial evaluation.

A 6-month long preventive intervention program for newly separated persons was designed on the basis of an analysis of the literature that identified the major stressful elements in the separation experience. Following the implementation of the program, its impact was assessed by contrasting persons who were assigned to the program (n = 100) with newly separated persons who were randomly selected to serve as a no-treatment control group (n = 50). Of the nine dependent measures of adjustment used in this evaluation, five significant posttreatment differences were found, in each case favoring th

Improved assessment of mass drug administration and health district management performance to eliminate lymphatic filariasis.

Lymphatic filariasis (LF) elimination as a public health problem requires the interruption of transmission by administration of preventive mass drug administration (MDA) to the eligible population living in endemic districts. Suboptimal MDA coverage leads to persistent parasite transmission with consequential infection, disease and disability, and the need for continuing MDA rounds, requiring considerable investment. Routine coverage reports must be verified in each MDA implementation unit (IU) due to incorrect denominators and numerators used to calculate coverage estimates with administrativ

Assessment of quality of obstetric care in Zimbabwe using the standard primipara.

BACKGROUND: To improve maternity services in any country, there is need to monitor the quality of obstetric care. There is usually disparity of obstetric care and outcomes in most countries among women giving birth in different obstetric units. However, comparing the quality of obstetric care is difficult because of heterogeneous population characteristics and the difference in prevalence of complications. The concept of the standard primipara was introduced as a tool to control for these various confounding factors. This concept was used to compare the quality of obstetric care among district
