[Possibilities of neurotrophic therapy for delayed speech development].

The article provides an analysis of the possibility of neurotrophic therapy of delayed speech development (DSD) in 266 children who underwent dynamic clinical, laboratory, functional and neuropsychological examinations and received neurotrophic therapy. This study confirmed the effectiveness of neurotrophic therapy in the treatment of as isolated speech disorder (ISD) as well speech disorder in the structure of the general psychomotor developmental delay (PDD). The lack of effect in the use of neuroprotective therapy in every sixth child of the latter group can be a marker for the individualiz

De novo HDAC8 mutation causes Rett-related disorder with distinctive facial features and multiple congenital anomalies.

We present a unique 11-year-old girl showing clinical features of Rett-related disorder with distinctive facial features and multiple congenital anomalies including ocular hypertelorism, arched eyebrows, a broad nose, dental anomalies, congenital heart disease, truncal obesity, and epilepsy. A novel de novo mutation in histone deacetylase 8 (HDAC8) (c.652G > T, p.Gly218Cys) was confirmed by whole exome sequencing and Sanger sequencing. X-chromosome inactivation analysis on DNA isolated from peripheral blood lymphocytes revealed a completely skewed pattern associated with an inactive maternal a

[Schizophrenic psychoses with bipolar course--implications for nosology and treatment].

Based on a description of the psychopathological symptoms and clinical course of three patients, the diagnostic classification of schizophrenia, schizoaffective disorder and bipolar affective disorder is discussed. The reported cases are characterised by a rapid change of cognition, affect and psychomotor domain with fluctuations in two opposite directions and an incomplete remission of symptoms between acute episodes of illness. Such cases are not adequately represented in the current diagnostic systems ICD-10 and DSM-IV. This renders both the diagnostic classification and the development of

ERCC6 founder mutation identified in Finnish patients with COFS syndrome.

Cerebro-oculo-facio-skeletal (COFS) syndrome is an autosomal recessive disorder characterized by microcephaly, congenital cataracts, facial dysmorphism, neurogenic arthrogryposis, growth failure and severe psychomotor retardation. We report a large consanguineous pedigree from northern Finland with six individuals belonging into four different sibships and affected with typical COFS syndrome phenotype. Two deceased patients have been published previously in 1982 as the first cases exhibiting cerebral calcifications typical for this disorder. Two living and one of the deceased patients were all

Posterior vault encephaloceles: from antenatal management to post-surgical follow-up-a cooperative study.

PURPOSE: Encephalocele is a herniation of intracranial structures associated with a skull anomaly. In Western countries, posterior encephaloceles are more common than anterior encephaloceles and may occur in the parietal (parietal encephalocele, PE) or occipital (occipital encephalocele, OE) region. Although those entities are relatively common in pediatric neurosurgery, large clinical series are scarce, and their clinical outcomes are poorly documented in the literature. METHODS: We retrospectively analyzed the clinical and radiological findings, post-operative long-term outcomes of consecut

Neuropsychological Determinants of Impairment in Activities of Daily Living in Patients With Lewy Body Dementia.

Background Understanding the specific cognitive domains associated with activities of daily living (ADLs) impairment in Lewy body dementia (LBD) may help identify which targeted therapeutic interventions to pursue in future research. This study aimed to investigate the neuropsychological determinants of impairment in ADLs in LBD patients. Methods We conducted a cross-sectional study of LBD patients referred for a clinical neuropsychological evaluation within the Parkinson's Disease and Movement Disorders Center at Virginia Commonwealth University. The presence or absence of impairment in eight

Self-Reported Sleep Need, Subjective Resilience, and Cognitive Performance Following Sleep Loss and Recovery Sleep.

OBJECTIVE: Individuals vary in response to sleep loss: some individuals are "vulnerable" and demonstrate cognitive decrements following insufficient sleep, while others are "resistant" and maintain baseline cognitive capability. Physiological markers (e.g., genetic polymorphisms) have been identified that can predict relative vulnerability. However, a quick, cost-effective, and feasible subjective predictor tool has not been developed. The objective of the present study was to determine whether two factors-"subjective sleep need" and "subjective resilience"-predict cognitive performance follow

A novel human Cdh1 mutation impairs anaphase promoting complex/cyclosome activity resulting in microcephaly, psychomotor retardation, and epilepsy.

The Fizzy-related protein 1 (Fzr1) gene encodes Cdh1 protein, a coactivator of the E3 ubiquitin ligase anaphase-promoting complex/cyclosome (APC/C). Previously, we found that genetic ablation of Fzr1 promotes the death of neural progenitor cells leading to neurogenesis impairment and microcephaly in mouse. To ascertain the possible translation of these findings in humans, we searched for mutations in the Fzr1 gene in 390 whole exomes sequenced in trio in individuals showing neurodevelopmental disorders compatible with a genetic origin. We found a novel missense (p.Asp187Gly) Fzr1 gene mutation

A novel variant of DHH in a familial case of 46,XY disorder of sex development: Insights from molecular dynamics simulations.

OBJECTIVE: Disorders of sex development (DSD) are a heterogeneous group of conditions affecting the differentiation and development of the internal and external genitalia. Here, we aimed at identifying the genetic cause of DSD in two 46,XY sisters from a consanguineous family. DESIGN: We performed a whole-exome sequencing of two 46,XY female individuals. Sanger sequencing was used to validate the most likely candidate variant, affecting the desert hedgehog (DHH) gene. Molecular dynamics simulations were performed to get insights into the impact of the variant on protein structure and on its i

Deletion 21q22.3 and duplication 7q35q36.3 in a Colombian girl: a case report.

BACKGROUND: Genetic disorders are a major cause in the etiology of cases with intellectual disability; however, analysis by a conventional technique such as cytogenetic karyotyping only allows the detection of chromosomal alterations in approximately 9.5 % of cases. The inclusion of new technologies such as high resolution microarray analysis has allowed the study of alterations in chromosomal segments that are less than 5 Mb in length; this has led to an increase in the diagnosis of these patients of up to 25 %. CASE PRESENTATION: We report the first case of an 8-year-old Colombian girl of m

Mechanisms of the psychostimulant effects of caffeine: implications for substance use disorders.

BACKGROUND: The psychostimulant properties of caffeine are reviewed and compared with those of prototypical psychostimulants able to cause substance use disorders (SUD). Caffeine produces psychomotor-activating, reinforcing, and arousing effects, which depend on its ability to disinhibit the brake that endogenous adenosine imposes on the ascending dopamine and arousal systems. OBJECTIVES: A model that considers the striatal adenosine A2A-dopamine D2 receptor heteromer as a key modulator of dopamine-dependent striatal functions (reward-oriented behavior and learning of stimulus-reward and rewa

CHD8-related disorders redefined: an expanding spectrum of dystonic phenotypes.

BACKGROUND: Heterozygous loss-of-function variants in CHD8 have been associated with a syndromic neurodevelopmental-disease spectrum, collectively referred to as CHD8-related neurodevelopmental disorders. Several different clinical manifestations, affecting neurodevelopmental and systemic domains, have been described, presenting with highly variable expressivity. Some expressions are well established and comprise autism spectrum disorders, psychomotor delay with cognitive impairment, postnatal overgrowth with macrocephaly, structural brain abnormalities, gastrointestinal disturbances, and beha
