Treatment of distal radius fractures using a cemented K-wire frame.

BACKGROUND: This retrospective study included an alternative treatment for types A2, A3, and B1 distal radius fractures using percutaneous fixation with a cemented K-wire frame. METHODS: From January 2017 to January 2020, 78 patients with distal radius fractures were treated with percutaneous internal fixation using a cemented K-wire frame. There were 47 male patients and 31 female patients. The fractures were classified into types A2 (n = 10), A3 (n = 46), and B1 (n = 22). X-rays were taken immediately after surgery and after the bone had healed. Wrist function was assessed using the Mayo Wr…

Kirschner Wire Fixation of Distal Radius Fractures, Indication, Technique and Outcomes.

Distal radius fractures are the most common upper extremity fracture that present to US emergency departments. Given the variable presentation, including age and fracture pattern, there are many techniques that have been utilized to treat them. Closed reduction and percutaneous pin (CRPP) fixation remains a viable option in extra-articular and simple intra-articular fracture patterns. CRPP techniques flourished in the mid-twentieth century until the treatment patterns shifted to primarily volar-locked plating in the twenty-first century. Although several meta-analyses have been performed compa…

Distal radius fractures: an evidence-based approach to assessment and management.

Distal radius fractures account for one in five bony injuries in both primary and secondary care. These are commonly the result of a fall on outstretched hands or high-energy trauma. On assessment, clinicians should determine the mechanism of injury, associated bony or soft tissue injuries, and neurovascular symptoms. Investigations should always include radiographs to evaluate for intra-articular involvement and fracture displacement. Owing to the heterogeneous injury patterns and patient profiles, the preferred management should consider the severity of the fracture, desired functional outco…

Radiographic Outcomes and Complications of Delayed Fixation of Distal Radius Fractures.

BACKGROUND: Surgeons are sometimes presented with patients with distal radius fractures who present in a delayed fashion or lose reduction after several weeks of attempted closed management. There are limited studies on delayed surgical treatment of distal radius fractures to assist providers in decision-making. METHODS: We conducted a matched cohort study to compare radiographic outcomes and complications for patients with a distal radius fracture treated with delayed (3-5 weeks) or early (0-2 weeks) surgical fixation. Patients ages 18+ who underwent open reduction and internal fixation of d…

The Effect of Distal Radius Fractures on 3-Dimensional Joint Congruency.

PURPOSE: Whereas the goal of distal radius fracture treatment is anatomical restoration, controversy exists as to whether it is the severity of the disruption of joint alignment at the time of fracture or residual malalignment after healing that determines outcome. The objective of this study was to compare joint contact mechanics bilaterally a minimum of 3 years following a distal radius fracture. Our hypothesis was that 3-dimensional joint space would be reduced in individuals following wrist fracture and that these changes in joint load lead to the development of degenerative arthritis. To

Surgical Decision-Making in Median Neuropathy Associated with Distal Radius Fractures.

Objectives  A lack of conclusive evidence on the treatment of acute median neuropathy (AMN) in patients with distal radius fractures has led to inconsistent surgical guidelines and recommendations regarding AMN in distal radius fractures. There is a wide variation in surgical decision-making. We aimed to evaluate international differences between surgical considerations and practices related to carpal tunnel release (CTR) in the setting of distal radius fractures. Methods  We approached surgeons who were a member of the Orthopaedic Trauma Association (United States) or of the Dutch Trauma Soci…

Intraoperative three-dimensional imaging in the treatment of distal radius fractures.

INTRODUCTION: In operative treatment of distal radius fractures satisfying outcome mainly relies on anatomical fracture reduction and correct implant placement. Examination with two-dimensional fluoroscopy may not provide reliable information about this. The aim of this study was to determine the effectiveness of additional intraoperative three-dimensional imaging in the operative treatment of comminuted distal radius fractures. MATERIALS AND METHODS: From August 2001 to June 2015, patients with a distal radius fracture who were treated operatively and received intraoperative three-dimensiona…

Epidemiology of distal radius fractures in a geographically defined adult population.

Distal radial fractures are the most common adult orthopaedic fracture. We sought to determine whether the incidence of this injury is changing and identify trends in its occurrence. We analysed data for all adult patients presenting to University Hospitals of Leicester with a distal radial fracture from 2007-2016. Incidence rates were calculated using United Kingdom population data. Poisson regression techniques were used to analyse weekly, seasonal and annual variation in fracture incidence. There was no significant change in average age or incidence of fracture. Increased incidence was asso

Treatment Trends, Complications, and Effects of Comorbidities on Distal Radius Fractures.

Background: Distal radius fractures (DRFs) are the most common upper extremity fractures in adults. This study seeks to elucidate the impact age, fracture type, and patient comorbidities have on the current treatment of DRFs and risk of complications. We hypothesized that comorbidities rather than age would relate to the risk of complications in the treatment of DRFs. Methods: A retrospective review of data was performed for patients treated between 2007 and 2014 using Truven Health MarketScan Research Databases. Patients who sustained a DRF were separated into "closed" versus "open" treatment…

How Should the Treatment Costs of Distal Radius Fractures Be Measured?

Distal radius fractures (DRFs) are common, but the best way to measure the total cost of treating these injuries is not known. We conducted a study to measure the total treatment cost of DRFs to identify which items should be measured, and for how long, to capture all major cost-drivers. Eighty-two patients with DRFs were included in this prospective, observational study. All costs, both direct and indirect, were measured. Direct costs were measured for each patient from internal billing records. Indirect costs were obtained with a customized questionnaire. The major contributors to overall co…

Safe Return to Driving After Volar Plating of Distal Radius Fractures.

PURPOSE: A major concern for patients following distal radius fracture fixation is when they can resume driving. This decision has medical, legal, and safety considerations, but there are no evidence-based guidelines to assist the surgeon. The goal of this study was to observe when patients are capable of safely resuming driving following surgical fixation of the distal radius. METHODS: Patients undergoing volar plating of a distal radius fracture were prospectively enrolled. At approximately 2 and 4 weeks after surgery, patients were administered a driving examination on a closed course and

Dorsal Plating of Distal Radius Fractures Historical Context and Appropriate Use.

The management of distal radius fractures has evolved over time from a largely nonoperative paradigm to a more commonly performed operative procedures today. Surgical trends have similarly developed, with dorsal plating falling out of favor due to complications involving extensor tendon pathology as well as due to the ubiquity of the volar plate along with the advent of locking plate technology. However, with the improvement in design of newer generation dorsal plates, this technique should be used in the appropriate clinical situation, including dorsal comminution and angulation with concomit…

Use of Postoperative Radiographs following Operative Fixation of Distal Radius Fractures.

BACKGROUND: Recent studies show that routine postoperative films after open reduction and internal fixation for distal radius fractures rarely alter clinical management. This population study evaluates the use of postoperative radiographs after distal radius fracture fixation. METHODS: The authors studied insurance claims from the Truven MarketScan databases to identify patients aged 18 years and older who underwent open reduction and internal fixation for a distal radius fracture between 2009 and 2011. Two years of postoperative data were collected to determine the number and timing of posto…

[Treatment of extra-articular distal radius fractures in active elderly patients].

Extra-articular distal radius fractures in active elderly patients are common and predominantly affect females. The high number of patients lost to follow-up compromises the evaluation of outcomes. Treatment aims to control the comminution and allow fast recovery of pre-injury activity levels. Fixation with volar locking plates is the gold standard. The role of bone substitutes in this type of injury is unclear.
