Cerebrolysin enhances spinal cord conduction and reduces blood-spinal cord barrier breakdown, edema formation, immediate early gene expression and cord pathology after injury.

Spinal cord evoked potentials (SCEP) are good indicators of spinal cord function in health and disease. Disturbances in SCEP amplitudes and latencies during spinal cord monitoring predict spinal cord pathology following trauma. Treatment with neuroprotective agents preserves SCEP and reduces cord pathology after injury. The possibility that cerebrolysin, a balanced composition of neurotrophic factors improves spinal cord conduction, attenuates blood-spinal cord barrier (BSCB) disruption, edema formation, and cord pathology was examined in spinal cord injury (SCI). SCEP is recorded from epidura…

Spinal Meninges and Their Role in Spinal Cord Injury: A Neuroanatomical Review.

Current recommendations support early surgical decompression and blood pressure augmentation after traumatic spinal cord injury (SCI). Elevated intraspinal pressure (ISP), however, has probably been underestimated in the pathophysiology of SCI. Recent studies provide some evidence that ISP measurements and durotomy may be beneficial for individuals suffering from SCI. Compression of the spinal cord against the meninges in SCI patients causes a "compartment-like" syndrome. In such cases, intentional durotomy with augmentative duroplasty to reduce ISP and improve spinal cord perfusion pressure (…

The timing of spinal stabilization in polytrauma and in patients with spinal cord injury.

PURPOSE OF REVIEW: Despite improved care of multiple injured patients, controversial data about the best time point for decompression and stabilization of spine fractures and spinal cord injuries are published. Some studies even question the value of surgical management. RECENT FINDINGS: In several mostly retrospective studies and a meta-analysis, there is growing evidence that early decompression and stabilization of spine injuries seems to be beneficial, especially in more severely injured patients. Early stabilization seems to reduce length of stay and pulmonary complications. Some studies…

Spinal cord regulation of sympathetic activity in intact and spinal rats.

Excitatory amino acid (EAA) and cholinergic neurotransmission in the spinal cord of urethan-anesthetized rats was investigated to assess mechanisms regulating sympathetic activity after spinal cord injury. Blockade of EAA transmission by intrathecal injection of kynurenic acid decreased arterial blood pressure by 24 +/- 4 mmHg, heart rate by 15 +/- 10 beats/min, and renal sympathetic nerve activity (RSNA) by 85 +/- 4% in intact rats. In rats with cervical spinal transections, this blockade decreased RSNA by 51 +/- 5% and had no effect on arterial pressure and heart rate. Muscarinic blockade by…

Case Report: Beyond chronicity: sustained multidimensional response to spinal cord stimulation three decades after traumatic spinal cord injury.

INTRODUCTION: Spinal cord stimulation (SCS) is an established treatment for refractory neuropathic pain conditions, yet evidence for its effectiveness in spinal cord injury (SCI)-related neuropathic pain, particularly after decades of persistent symptoms, remains limited. It remains unclear whether unusually prolonged pain chronicity and established neurological deficits affect the potential for a meaningful response to neuromodulation. CASE PRESENTATION: We report a 60-year-old man with chronic incomplete SCI, bilateral spastic paraparesis markedly more pronounced on the left, and severe chr…

A Novel Case Report of Closed-Loop Spinal Cord Stimulation for Neuropathic Pain in Cervical Spinal Cord Injury.

BACKGROUND: Neuropathic pain after spinal cord injury (SCI) is common and often refractory. Closed-loop spinal cord stimulation (CL-SCS) is a novel intervention that adjusts stimulation based on evoked compound action potentials in real time, but its use for pain in complete SCI is not well described. CASE REPORT: A 45-year-old man with complete C6-C7 SCI developed severe, diffuse neuropathic pain below the injury level refractory to multiple medications, an intrathecal fentanyl pump trial, and ketamine infusions. With extensive epidural scarring, a staged buried CL-SCS trial with a combined …

Functional Recovery Following Spinal Cord Stimulation in a Patient Utilizing Prospera Spinal Cord Stimulation System with Multiphase Stimulation.

Spinal cord stimulation (SCS) is an established treatment for chronic neuropathic pain, particularly in patients who have failed conservative therapy or prior spine surgery. This case highlights significant pain reduction and functional recovery in a patient with severe lumbar radiculopathy and weakness following prior lumbar surgery. A 39-year-old female with multilevel disc bulges causing spinal stenosis, and chronic lumbar radiculopathy presented with severe low back and left leg pain. She had undergone lumbar decompression surgery in 2023 but continued to experience debilitating neuropathi…

Brain inflammation and cognitive decline induced by spinal cord injury can be reversed by spinal cord cell transplants.

Spinal cord injuries (SCIs) impact between 250,000 and 500,000 people worldwide annually, often resulting from road accidents or falls. These injuries frequently lead to lasting disabilities, with the severity depending on the injury's extent and location. Emerging research also links SCIs to cognitive impairments due to brain inflammation. From a treatment perspective, various approaches, including cell therapy, have been investigated. One common mechanism across cellular transplant models is the modulation of inflammation at the injury site, though it remains uncertain if these effects exten…

CD47-blocking antibody interferes with neutrophil extracellular traps formation after spinal cord injury to reduce spinal cord edema.

OBJECTIVE: Our goal was to investigate the role of neutrophil extracellular traps (NETs) in the disruption of the blood-spinal cord barrier (BSCB) following spinal cord injury (SCI) and to evaluate the therapeutic efficacy of CD47-blocking antibodies in mitigating the disruption. METHODS: We utilized Evans blue extravasation to evaluate BSCB permeability and immunofluorescence to evaluate the formation of NETs and the expression of ZO-1, CD31, S100A8/A9, CD68, GFAP, Iba-1, and NeuN. Spinal cord edema was quantified by comparing the dry and wet weights of tissue samples. We used enzyme-linked …

Extracellular vesicle-mediated spinal cord-brain crosstalk induces hippocampal neurogenesis impairment and cognitive deficits post-spinal cord injury.

Rationale: Spinal cord injury (SCI) is well-documented for its devastating impact on motor and sensory functions. However, its potential effects on cognitive function remain underexplored. This study aims to investigate the mechanisms of SCI-induced cognitive dysfunction, focusing on spinal cord-hippocampal communication mediated by extracellular vesicles (EVs). Methods: Cognitive function and hippocampal neurogenesis were assessed in mice subjected to either SCI or sham surgery. EVs were isolated from spinal cord tissues of SCI and sham groups and stereotactically injected into the hippocampu…

Effects of spinal cord injury associated exosomes delivered tRF-41 on the progression of spinal cord injury progression.

BACKGROUND: Spinal cord injury (SCI) is a devastating neurological and pathological condition. Exosomal tsRNAs have reported to be promising biomarkers for cancer diagnosis and therapy. This study aimed to investigate the roles of SCI-associated exosomes, and related tsRNA mechanisms in SCI. METHODS: The serum of healthy controls and SCI patients at the acute stage were collected for exosomes isolation, and the two different exosomes were used to treat human astrocytes (HA). The cell viability, apoptosis, and cycle were determined, and the expression of the related proteins were detected by w…

Changes in respiratory structure and function after traumatic cervical spinal cord injury: observations from spinal cord and brain.

Respiratory difficulties and mortality following severe cervical spinal cord injury (CSCI) result primarily from malfunctions of respiratory pathways and the paralyzed diaphragm. Nonetheless, individuals with CSCI can experience partial recovery of respiratory function through respiratory neuroplasticity. For decades, researchers have revealed the potential mechanism of respiratory nerve plasticity after CSCI, and have made progress in tissue healing and functional recovery. While most existing studies on respiratory plasticity after spinal cord injuries have focused on the cervical spinal cor…

Successful spinal cord stimulation for neuropathic below-level spinal cord injury pain following complete paraplegia: a case report.

INTRODUCTION: Neuropathic pain is common in patients with spinal cord injury (SCI) and often difficult to treat. We report a case where epidural spinal cord stimulation (SCS) below the level of injury has been successfully applied in a patient with a complete spinal cord lesion. CASE PRESENTATION: A 53-year-old female presented with neuropathic below-level SCI pain of both lower legs and feet due to complete SCI below T5. Time and pain duration since injury was 2 years. Pain intensity was reported on numeric rating scale with an average of 7/10 (0 meaning no pain, 10 meaning the worst imagina…

An Autonomic Neuroprosthesis: Noninvasive Electrical Spinal Cord Stimulation Restores Autonomic Cardiovascular Function in Individuals with Spinal Cord Injury.

Despite autonomic dysfunction after spinal cord injury (SCI) being the major cause of death and a top health priority, the clinical management options for these conditions are limited to drugs with delayed onset and nonpharmacological interventions with equivocal effectiveness. We tested the capacity of electrical stimulation, applied transcutaneously over the spinal cord, to manage autonomic dysfunction in the form of orthostatic hypotension after SCI. We assessed beat-by-beat blood pressure (BP), stroke volume, and cardiac contractility (dP/dt; Finometer), as well as cerebral blood flow (tra…

A Rare Intramedullary Spinal Cord Metastasis from a Retroperitoneal Leiomyosarcoma Presenting as a Non-Traumatic Spinal Cord Injury.

Leiomyosarcoma (LMS) is a rare but well-recognized malignant soft tissue sarcoma of smooth muscle origin. Metastases commonly occur in the lungs, liver, kidney, brain, and bone. Cases of metastatic osseous lesions or other extradural space-occupying masses secondary to LMS leading to neurologic compromise are relatively commonplace in the literature. Conversely, cases of intramedullary spinal cord metastasis (ISCM), an unusual entity as a sequela of any cancer, are exceedingly rare as a consequence of LMS. Only 2 cases of an ISCM from LMS are currently documented in the literature, and to the …

Measurement of Intraspinal Pressure After Spinal Cord Injury: Technical Note from the Injured Spinal Cord Pressure Evaluation Study.

Intracranial pressure (ICP) is routinely measured in patients with severe traumatic brain injury (TBI). We describe a novel technique that allowed us to monitor intraspinal pressure (ISP) at the injury site in 14 patients who had severe acute traumatic spinal cord injury (TSCI), analogous to monitoring ICP after brain injury. A Codman probe was inserted subdurally to measure the pressure of the injured spinal cord compressed against the surrounding dura. Our key finding is that it is feasible and safe to monitor ISP for up to a week in patients after TSCI, starting within 72 h of the injury. W…
