Determinants of the total cosine of the spatial angle between the QRS complex and the T wave (TCRT): implications for distinguishing primary from secondary T-wave abnormalities.

BACKGROUND: Contrary to intuitive expectations and dissimilar from that which occurs in the atria, left ventricular (LV) depolarization and repolarization proceed in opposite directions, creating a concordance of the spatial QRS complex and T-wave angles (QRS-T). By defining the determinants of QRS-T, it might be possible to distinguish a primary (caused by an abnormality of repolarization) from a secondary (caused by a delay in ventricular depolarization) T-wave abnormality. METHODS AND RESULTS: From a near-consecutive series of 154 patients (age, 60 +/- 16 years; 81 females) in sinus rhythm

Successful embolization of aneurysms in deep coma patients with subarachnoid haemorrhage receiving ECMO support: a case report.

ECMO is an advanced technology for extracorporeal respiratory and circulatory support. It involves the extraction of venous blood from the patient's body, which is subsequently oxygenated within an oxygenator (or membrane lung). This oxygen-rich blood is reinfused either into veins or arteries, rapidly compensating for impaired lung and heart functionalities. ECMO mirrors the essential processes of cardiac output in facilitating tissue perfusion and gas exchange, thereby expanding the therapeutic window for critically ill patients with acute circulatory insufficiency, and enabling restoration

In Vivo ETosis of Human Eosinophils: The Ultrastructural Signature Captured by TEM in Eosinophilic Diseases.

Eosinophilic diseases, also termed eosinophil-associated diseases (EADs), are characterized by eosinophil-rich inflammatory infiltrates and extensive eosinophil degranulation with clinically relevant organ pathology. Recent evidence shows that eosinophil cytolytic degranulation, that is, the release of intact, membrane-delimited granules that arises from the eosinophil cytolysis, occurs mainly through ETosis, meaning death with a cytolytic profile and extrusion of nucleus-originated DNA extracellular traps (ETs). The ultrastructural features of eosinophil ETosis (EETosis) have been studied mos

Beyond Single-Lead ECG-Derived Respiration Analysis: Use of Vectorcardiograms from the EASI-System for Breathing Frequency Estimation-A Feasibility Study.

Precise respiration assessment is crucial for heart rate variability (HRV) interpretation as respiratory components-particularly respiratory sinus arrhythmia (RSA)-provide essential information on vagally mediated regulation. Conventional single-lead electrocardiogram-derived respiration (EDR) methods measure the amplitude modulation of the QRS-waveform caused by respiratory chest movements. This causes a displacement of the electrical heart axis in relation to the ECG lead axis, typically within the 2D frontal plane of the Einthoven electrode montage. Another approach is based on heartbeat ac

Side-Specific Concordance Between Nasal Endoscopy and Computed Tomography Severity in Pediatric Chronic Rhinosinusitis.

Background/Objectives: The European Position Paper on Rhinosinusitis (EPOS) diagnoses chronic rhinosinusitis (CRS) from symptoms with nasal endoscopy and/or computed tomography (CT), but how closely a graded endoscopic score reflects CT severity in children-and whether it does so side-specifically-is poorly defined. We quantified this concordance to confirm when CT might be deferred. Methods: Retrospective analysis of 73 children (33 girls, 40 boys; aged 5-17 years) treated surgically for CRS after failed medical therapy. CT was scored with the Lund-Mackay scale (LMS); a modified score (MLMS)

Orbital reconstruction for tumor-associated proptosis: quantitative analysis of postoperative orbital volume and final eye position.

OBJECTIVE: Surgical resection of sphenoid wing tumors and intraorbital pathology carries the dual goal of appropriately treating the target pathology as well as correcting proptosis. Residual proptosis following surgery can lead to cosmetic and functional disability. The authors sought to quantitatively assess the effect of orbital volume before and after reconstruction to determine the optimal strategy to achieve proptosis correction. METHODS: All surgeries involving orbital wall reconstruction for orbital or intracranial pathology that preoperatively resulted in proptosis between 2007 and 2

Traumatic tension pneumocephalus - Two cases and comprehensive review of literature.

UNLABELLED: Although traumatic pneumocephalus is not uncommon, it rarely evolves into tension pneumocephalus (TP). Characterized by the presence of increasing amounts of intracranial air and concurrent appearance or worsening neurological symptoms, TP can be devastating if not recognized and treated promptly. We present two cases of traumatic TP and a concise review of literature on this topic. Two cases of traumatic TP are presented. In addition, a literature search revealed 20 additional cases, of which 18 had sufficient information for inclusion. Literature cases were combined with the 2 re

Clear cell meningioma: a clinicopathologic study of 18 tumors and examination of the use of CD10, CA9, and RCC antibodies to distinguish between clear cell meningioma and metastatic clear cell renal cell carcinoma.

Clear cell meningiomas (CCM) can be difficult to distinguish from metastatic clear cell renal cell carcinomas by standard light microscopy. Distinction is important in deciding patient management and establishing prognosis. The purpose of this study is to evaluate the use of immunomarkers CA9, CD10, and RCC in differentiating between CCM and clear cell renal cell carcinoma. The study retrospectively reviewed the clinicopathologic features of 18 patients with CCM (9 females, 9 males; age range at the time of surgery 16 to 86 y) including immunostaining results with antibodies to CA9, CD10, and

Subcortical intracerebral hemorrhage caused by mucormycosis in a patient with a history of bone-marrow transplantation.

We report on a rare case of a patient with rhinocerebral mucormycosis that presented as intracerebral hemorrhage (ICH). A 54-year-old man who was immunosuppressed had ophthalmoplegia. Four days later, ICH developed in his left frontal lobe. The ICH was surgically removed totally. Pathology specimen surgically obtained from brain surface adjacent to hematoma cavity showed blood vessels filled with Mucor mycelium. Combined with surgical findings, venous occlusion by Mucor mycelium might be the cause of ICH in the patient.

Comparative analysis of transnasal endoscopic reconstruction techniques for managing cerebrospinal fluid rhinorrhea in skull base defects.

The nasal skull base is located into the deep position of nasal cavity and closely related to important nerves and vessels. The complete removal of tumors in this area poses a complex surgical challenge. In order to investigate the clinical efficacy of utilizing free middle turbinate mucosa (FMT), fascia lata, and pedicled nasal septum flap (known as the Hadad-Bassagasteguy flap, HBF) for the treatment of cerebrospinal fluid (CSF) rhinorrhea, a retrospective analysis was conducted on clinical data from 65 patients who underwent skull base reconstruction following endoscopic resection of nasal-s

Current Trends in Facial Feminization Surgery: An Assessment of Safety and Style.

BACKGROUND: Facial feminization surgery (FFS) plays an instrumental role in the process of gender affirmation. These procedures are becoming increasingly appreciated for their ability to improve patient satisfaction and gender identity in a way that alleviates gender dysphoria and improves the quality of life. Despite the recent surge in popularity across the US, the current literature lacks evidence on the safety profile of combined facial feminization procedures. Our goal was to determine the safety profile of facial feminization procedures registered on a national surgical database. METHOD

Lung cancer and Rosai-Dorfman's disease. A clinicopathological study.

CASE REPORT: A 60-year-old female patient underwent craniotomy for a cerebral lesion in the frontoparietal lobe. Histologically, it turned out to be a metastasis from an adenocarcinoma. The primary tumor was found in the upper lobe of the left lung. The patient had whole brain radiation therapy only, the lung tumor was not treated. 4 years later, she presented with enlarged cervical lymph nodes. A biopsy showed dilated sinuses filled with histiocytes, but no tumor cells. The diagnosis of sinus histiocytosis with massive lymphadenopathy was made (Rosai-Dorfman's disease). Without any treatment,

Trigonocephaly: surgical considerations and long term evaluation.

Trigonocephaly accounts for approximately 10% of all craniosynostosis. Severe trigonocephaly results in a triangular-shaped forehead, superior-lateral orbital depression, hypotelorism, and compensatory occipital-parietal calvarial changes. Radiographic findings include ovoid orbits with parallel medial borders, thickened keel-shaped frontal bone, small ethmoid sinuses, and a short anterior cranial fossa with pitched sphenoid wings. Our experience with 50 infantile cases of severe nonsyndromic trigonocephaly patients treated from 1987 to 2005 is clinically reviewed to assess long-term growth ba
