Inherent smoothness of intensity patterns for intensity modulated radiation therapy generated by simultaneous projection algorithms.

The efficient delivery of intensity modulated radiation therapy (IMRT) depends on finding optimized beam intensity patterns that produce dose distributions, which meet given constraints for the tumour as well as any critical organs to be spared. Many optimization algorithms that are used for beamlet-based inverse planning are susceptible to large variations of neighbouring intensities. Accurately delivering an intensity pattern with a large number of extrema can prove impossible given the mechanical limitations of standard multileaf collimator (MLC) delivery systems. In this study, we apply Ci

Use of a toxicokinetic model in the analysis of cancer mortality in relation to the estimated absorbed dose of dioxin (2,3,7,8-tetrachlorodibenzo-p-dioxin, TCDD).

We performed an analysis of All cancer and Lung cancer mortality in relation to estimated absorbed dose of dioxin (2,3,7,8-tetrachlorodibenzo-p-dioxin, TCDD) in the cohort of chemical workers at 12 US plants assembled by the US National Institute for Occupational Safety and Health (NIOSH) (n = 5172). Estimates of cumulative exposure to TCDD were based on a minimal physiologic toxicokinetic model (MPTK) that accounts for inter- and intra-individual variations in body mass index (BMI) over time. Population-level parameters related to liver elimination and background (input or concentration) of T

Development and validation of an echocardiographic model for predicting progression of discrete subaortic stenosis in children.

The clinical course of discrete subaortic stenosis (DSS) varies considerably between patients. This study was performed to identify echocardiographic characteristics of DSS that distinguish progressive from nonprogressive disease. The study included 100 patients from 2 institutions and was performed in 2 stages. In phase I, a prediction model was developed based on multivariate analysis of morphometric and Doppler variables obtained from the initial echocardiogram in 52 children with DSS from Texas Children's Hospital. In phase II, the performance characteristics of the prediction model were t

Method for calculating risk associated with family history of a disease.

A method is described for estimating excess relative risks of a disease from familial factors. Beginning with population-based series of cases and controls, a cohort of each subject's relatives is formed and checked for disease against a population based registry. The disease experience of the cohort formed from each subject's relatives is summarized as a kinship-weighted familial standardized incidence ratio (FSIR). The FSIR's are used as exposure estimates in conditional linear excess relative risk models, which may be used not only to screen for significant familial disease aggregations, bu

Development and validation of a risk calculator predicting exercise-induced ventricular arrhythmia in patients with cardiovascular disease.

BACKGROUND: Mortality due to cardiovascular disease is often associated with ventricular arrhythmias. Nowadays, patients with cardiovascular disease are more encouraged to take part in physical training programs. Nevertheless, high-intensity exercise is associated to a higher risk for sudden death, even in apparently healthy people. During an exercise testing (ET), health care professionals provide patients, in a controlled scenario, an intense physiological stimulus that could precipitate cardiac arrhythmia in high risk individuals. There is still no clinical or statistical tool to predict th

Effect of different impression techniques on marginal integrity of CAD-CAM milled all-on-four mandibular frameworks: an in vitro study.

BACKGROUND: To guarantee a passive fit, full arch implant supported prostheses require scrupulous impressions. The accuracy of conventional and digital impressions is still up for debate, despite several studies comparing both acquisition techniques. The present study aimed to compare mandibular full arch implant impressions by assessing the vertical misfit of implant supported frameworks obtained through conventional and digital impressions. METHODS: To simulate the "All-on-4" scenario, a completely edentulous epoxy mandibular reference model was prepared with the installment of two straight

Detection of Clinically Significant Drug-Drug Interactions in Fatal Torsades de Pointes: Disproportionality Analysis of the Food and Drug Administration Adverse Event Reporting System.

BACKGROUND: Torsades de pointes (TdP) is a rare yet potentially fatal cardiac arrhythmia that is often drug-induced. Drug-drug interactions (DDIs) are a major risk factor for TdP development, but the specific drug combinations that increase this risk have not been extensively studied. OBJECTIVE: This study aims to identify clinically significant, high-priority DDIs to provide a foundation to minimize the risk of TdP and effectively manage DDI risks in the future. METHODS: We used the following 4 frequency statistical models to detect DDI signals using the Food and Drug Administration Adverse

Novel breast tissue feature strongly associated with risk of breast cancer.

PURPOSE: Accurate, individualized risk prediction for breast cancer is lacking. Tissue-based features may help to stratify women into different risk levels. Breast lobules are the anatomic sites of origin of breast cancer. As women age, these lobular structures should regress, which results in reduced breast cancer risk. However, this does not occur in all women. METHODS: We have quantified the extent of lobule regression on a benign breast biopsy in 85 patients who developed breast cancer and 142 age-matched controls from the Mayo Benign Breast Disease Cohort, by determining number of acini

Large-scale evaluation of multimodal large language models for pneumothorax detection.

PURPOSE: Pneumothorax requires rapid recognition and accurate interpretation of chest X-rays (CXRs), particularly in acute settings where delays can have serious consequences. With the emergence of advanced image interpretation models capable of visual analysis, their diagnostic reliability in radiology practice remains to be determined. This study aimed to assess the diagnostic performance of three state-of-the-art systems in detecting pneumothorax using a large, well-annotated dataset. METHODS: A total of 10,675 CXRs from the publicly available SIIM-ACR Pneumothorax Segmentation dataset wer

Extrapolation of Time-to-Event Survival Outcomes of Histology-Independent Therapies Using a Bayesian Hierarchical Model.

IntroductionHealth technology assessment of histology-independent therapies (HITs) requires statistical methods that can capture heterogeneity in outcomes while allowing borrowing of information between tumor sites to inform cost-effectiveness analysis. In this study, we extend previous work on binary outcomes to the application of Bayesian hierarchical models (BHMs) for extrapolation of overall survival from pembrolizumab-treated patients with microsatellite instability-high/deficient mismatch repair solid tumors.MethodsWe considered BHMs based on 1- or 2-parameter distributions for extrapola

Risk prediction models for contrast-induced acute kidney injury in patients with acute coronary syndromes: a systematic review and meta-analysis.

BACKGROUND: Percutaneous coronary intervention (PCI) has become a crucial method for the treatment of acute coronary syndromes (ACS), which includes ST-segment elevation myocardial infarction (STEMI), non-ST-segment elevation myocardial infarction (NSTEMI), and unstable angina (UA). However, contrast-induced acute kidney injury(CI-AKI) is one of its serious complications. A growing number of models have been used to predict ACS patients undergoing coronary angiography (CAG) or PCI, but the predictive efficacy of these models is unclear. METHODS: We systematically searched PubMed, Web of Scien

Valley-Forecast: Forecasting Coccidioidomycosis incidence via enhanced LSTM models trained on comprehensive meteorological data.

Coccidioidomycosis (cocci), or more commonly known as Valley Fever, is a fungal infection caused by Coccidioides species that poses a significant public health challenge, particularly in the semi-arid regions of the Americas, with notable prevalence in California and Arizona. Previous epidemiological studies have established a correlation between cocci incidence and regional weather patterns, indicating that climatic factors influence the fungus's life cycle and subsequent disease transmission. This study hypothesizes that Long Short-Term Memory (LSTM) and extended Long Short-Term Memory (xLST
