Enteral tube feeding in older people with advanced dementia: findings from a Cochrane systematic review.

AIM: Enteral tube feeding for people with advanced dementia who have difficulty swallowing and poor nutritional intake is common. Potential benefits or harms of this practice are unclear and the authors aimed to evaluate the outcomes of this intervention. METHODS: A full literature review was undertaken in April 2008. Randomized controlled trials (RCTs), controlled clinical trials, controlled before and after studies, interrupted time series studies and controlled observational studies that evaluated the effectiveness of enteral feeding via a nasogastric tube or via a tube passed by percutane

[Reconstruction of the tongue with lateral arm free flap after hemiglossectomy].

OBJECTIVE: To investigate the value of microsurgical reconstruction of the tongue after hemiglossectomy with lateral arm free flaps in the treatment of tongue cancer. METHODS: From October 2006 to April 2007, ten patients underwent simultaneous reconstruction of the tongue and oral floor defects with lateral arm free flaps after resection of squamous cell carcinoma of tongue in our hospital. The flaps ranged from 8 cm x 5 cm to 9 cm x 6 cm in size, and were adjusted to the defect of the tongue. The vascular pedicle included the posterior radial collateral artery and the accompanying veins. Th

Reconstruction of the hypopharynx: current trends.

PURPOSE OF REVIEW: Reconstruction of the hypopharynx continues to present a clinical challenge for the reconstructive surgeon. Trends continue to evolve as many viable options exist, depending on individual patient characteristics and institutional preferences. The article reviews the most current literature available on the subject. RECENT FINDINGS: Surgical resections resulting in pharyngoesophageal defects require timely reconstruction to minimize patient morbidity while optimizing quality of life, specifically in regards to speech and swallowing. Contemporary reconstructive options includ

Esophageal thermal injury by hot adlay tea.

Reversible thermal injury to the esophagus as the result of drinking hot liquids has been reported to generate alternating white and red linear mucosal bands, somewhat reminiscent of a candy cane. This phenomenon is associated with chest pain, dysphagia, odynophagia, and epigastric pain. Here, we report a case of thermal injury to the esophageal and oral cavity due to the drinking of hot tea, including odynophagia and dysphagia. A 69-year-old man was referred due to a difficulty in swallowing which had begun a week prior to referral. The patient, at the time of admission, was unable to swallow

[Perforation of the hypopharynx as a rare life-threatening complication of endotracheal intubation].

BACKGROUND: Endotracheal intubation is regarded as the gold standard technique to secure the airway with a low complication rate, however, perforating tracheal or esophageal injuries are rare but severe complications. MATERIALS AND METHODS: Two cases of hypopharyngeal perforation after intubation are presented and discussed. RESULTS: While intubation of the first patient was anticipated to be difficult, the second patient did not present any risk factors. One patient developed a typical clinical pattern of difficult swallowing, soft tissue emphysema of the neck, pyrexia, and leukocytosis. Th

The lower oesophageal sphincter.

The lower oesophageal sphincter (LOS) is a specialized segment of the circular muscle layer of the distal oesophagus, accounting for approximately 90% of the basal pressure at the oesophago-gastric junction. Together with the crural diaphragm, it functions as an antireflux barrier protecting the oesophagus from the caustic gastric content. During swallowing or belching, the LOS muscle must relax briefly in order to allow passage of food or intragastric air. These swallow-induced and prolonged transient lower oesophageal sphincter relaxations (TLOSRs) respectively result from activation of the

[Terminal care in the Department of Neurology].

Patients in the Department of Neurology undergoing treatment for disorders such as cerebrovascular disease, dementia, metabolic disease, neuromuscular disease and intractable disease, are included as subjects requiring terminal care. Intractable diseases ware defined by the Ministry of Health and Welfare (Ministry of Health, Welfare and Labor) in 1972 as being of unknown etiology, untreatable, chronically progressive and sometimes worsened by the care provided when nursing these patients. Intractable diseases in the Department of Neurology rank with those seen in other departments. Amyotrophic

Maturation and transformation of reflexes that protect the laryngeal airway from liquid aspiration from fetal to adult life.

Several reflexes are initiated in the fetus and newborn when hypochloremic or strongly acidic solutions contact the epithelium that surrounds the entrance to the laryngeal airway. These reflexes, known collectively as the laryngeal chemoreflex (LCR), include startle, rapid swallowing, apnea, laryngeal constriction, hypertension, and bradycardia. Many studies have shown that prolonged apnea associated with the LCR may be life threatening and might conceivably be a cause of sudden infant death syndrome. This certainly may be true, but the concept of a lethal LCR paradoxically contrasts with the

Effect of pre-emptive analgesia on self-reported and biological measures of pain after tonsillectomy.

This prospective, double-blind, randomized study assessed effect of pre-emptive peritonsillar block in 30 ASA-I children, aged 6-12 years, of both sexes, scheduled for tonsillectomy. Patients were divided into three groups: those in group I received a sham block, whereas peritonsillar blocks with bupivacaine 0.25% were given to the children before tonsillectomy (group II) or immediately after surgery had been completed (group III). Constant pain, pain on swallowing, blood glucose, serum epinephrine and norepinephrine concentrations were measured immediately after surgery and 4h after operation

Effects of tap water on esophageal burns in dogs from button lithium batteries.

Incidences of swallowing lithium batteries have been increasing. A lithium battery is more dangerous than button alkali batteries because the tissue erosion and perforation of the esophagus is more rapid than with button alkali batteries. We hypothesized that a bolus fresh water wash out or dilution of the alkali produced by the direct current flow, and/or inhibition of the alkali production by interrupting the direct current flow, might lessen the severity of the necrotic injury at a contact esophageal site with a battery. Model CR2032 lithium batteries were fixed in the esophagus of 21 adult

Static and dynamic function of the lower esophageal sphincter before and after laparoscopic Nissen fundoplication.

The means by which fundoplication protects against reflux is disputed. We studied the resting and dynamic features of the lower esophageal sphincter (LES) and 24-hour pH monitoring in 26 patients before and after laparoscopic Nissen fundoplication. Resting features were LES pressure, abdominal length, and total length. Dynamic function was assessed by the residual pressure in the LES during a swallow measured as the bolus flowed though the LES. All patients experienced near-total relief of heartburn and all but one had normal postoperative acid scores. Resting LES characteristics were restored

Feeding behaviour in infants.

20 healthy full-term infants have been studied in their 2nd, 10th, 18th and 26th weeks of life in three standard situations: before feeding, in the course of milk drinking and after feeding. A polygraphic record of sucking, breathing, swallowing, the oculomotor activity, the EEG and vocalization has been taken simultaneously with a video-recording of the infant's behaviour. Physiological patterns and developmental changes of sucking, breathing and swallowing are described. These activities are considered as fine motor activity of the earliest age. The possibility of using polygraphic recording

Content Validity and Intra-Inter Reliability of the Turkish Version of the Murray Secretion Scale.

INTRODUCTION: In healthy individuals, secretions are typically managed through reflexive swallowing or coughing. Impairments in sensory or motor function, however, may hinder laryngo-pharyngeal clearance, leading to hypopharyngeal secretion accumulation and compromised swallowing safety. This study aimed to translate the Murray Secretion Scale into Turkish (MSS-TR) and to evaluate its content validity and inter- and intra-rater reliability. The primary goal was to determine whether the Turkish version of the scale is a reliable and valid tool for clinical use in assessing pharyngolaryngeal sec

Effectiveness of allied health therapy in the symptomatic management of progressive supranuclear palsy: a systematic review.

BACKGROUND: Progressive supranuclear palsy (PSP) is an adult onset neurodegenerative condition associated with mobility, balance, speech, swallowing, vision and cognitive changes. The condition is diagnosed using the National Institute for Neurological Disorders and Stroke (NINDS) and the Society of Progressive Supranuclear Palsy (SPSP) criteria. Therapeutic interventions for PSP are important, and a healthcare team should include a physiotherapist, occupational therapist and speech therapist. Mobility, speech and swallowing problems are commonly experienced, and aspiration pneumonia is the le

The Visual Analogue Test for Anosognosia for Dysphagia: VATA-Dysphagia.

BACKGROUND: Swallowing difficulties (dysphagia) can be associated with a plethora of neurological diseases representing a life-changing condition and, in severe cases, a life-threatening situation. It is, therefore, crucial that patients and caregivers are aware of its implications to implement the best strategies for safety and functional rehabilitation. Current scales present some limitations in assessing patients' and their caregivers' awareness, making it difficult to evaluate more ecological aspects of dysphagia and hampering the assessment of patients with aphasia, attentional disorders

Repair of tracheo-oesophageal fistula secondary to button battery ingestion: A combined cervical and median sternotomy approach.

A three-year-old child developed a large tracheo-oesophageal fistula secondary to a button battery being lodged in the upper oesophagus for 36 hours. The diagnosis was confirmed with a contrast swallow. Operative access was gained through a combined right cervical incision and complete median sternotomy. Repair of the fistula required a segmental resection of both the trachea and oesophagus followed by primary anastomosis.

Perforation of Meckel's diverticulum by a button battery: report of two cases.

Ingested button battery perforation of Meckel's diverticulum is extremely rare, with only two reported cases in the recent literature. Two additional preschool children who accidentally swallowed an alkaline button battery and developed a perforated Meckel's diverticulum are reported. Surgeons should be aware of this rare complication. Careful clinical, radiologic and laboratory monitoring of children who ingest a button battery is advisable.
